I was scrolling through reddit today and came across this post saying I have “fatphobic tendencies”. This was really hard for me to read and caused me to take a critical look back at my previous comments regarding obesity, bariatric treatments, and diet recommendations (Keto, intermittent fasting, vegan, etc.) Our culture demonizes people for their weight, from rogue marketing pushing “ideal bodies” in media to a lack of representation for large bodied people on stage and on screen. That being said, I am a doctor, and my job is to help people live longer, healthier lives, and often those results can be achieved through a variety of tactics that will reduce the weight of people who suffer from obesity. It’s a fine line between helping my patients and making sure they don’t feel judged or criticized for the way they look, and I’m working hard to get it right.

Most overweight/obese people have this mentality that if you speak about diets, exercise, weightloss you're automatically fatphobic. This isn't something new, its an issue happening daily on the internet and Dr Mike isnt the only one that has addressed this problem. Plenty of fitness content creators who have been on a weightloss journey have spoken up about this. Unfortunately those people refuse to listen because they blame it on their family history of obesity saying how they can never lose weight, theyve always been fat, and so forth. Any healthy advice they take it as fat shaming. Its quite sad honestly.

There is a small group of people in the body positivity movement who legitimately think there is nothing unhealthy with being fat. I've seen it go as far as shaming people for losing fat for their health. But body positivity isn't about saying being fat is healthy, it's about recognizing that fat people are people with value and worth. And yes, you can be, by all measures healthy and overweight... But all the evidence points towards the fact that it causes stress in all the systems in the body and eventually leads to disease.

He knows from the patients he's had the struggles of diets and whatnot which made him very sympathetic towards people who struggle with the mental part of weight loss for obese people (which is the biggst hurdle really; since dieting is the only real thing you'll need for weight loss unless say you got a couple pounds of sagging skin where physical activity will be welcome) He even sympathizes well enough for those who pretty much get harsh words from others' doctors

Solid point right here. Every time I get a check up or a doctor asks about my diet and what I eat/drink, I am always told I need to eat better and drink better, and that's me being "too skinny". Also it's so odd that this is what people think is fat phobic and his smoking example was good but also dentists always say the whole less sugar and acid stuff but nobody is out here with half of their teeth missing due to cavities complaining about dentists telling them they should work on improving their diet so the few teeth they have left don't rot away.

This is an excellent point. Fatphobia is when people say you are: lazy, disgusting, mean, stupid, ugly, worthless, don’t deserve respect or rights, should be treated unequally and should be shunned in society because you are fat. Health is important and should be sought out. I think health is less tied to the number on the scale or bmi and more about your habits. Do you eat balanced meals and exercise often? Do you avoid harmful habits like smoking and excessive drinking? If you get the recommended amount of exercise and eat with nutrition in mind, weight, shape, or size shouldnt really matter. As someone who is very anti-fatphobia, all i ask is people stop judging me, being cruel to me, prevent me from opportunities that i am well equipped for such as jobs, and constantly pressuring me to take up extreme measures that lead to eating disorders because of my size. Really, unless you are my doctor and you know my lifestyle and habits, you shouldnt be commenting on someone elses weight or what they eat. Its not your place. And i think its toxic to expect everyone to sustainably be able to be thin. Thinness does not equal health, and sometimes people cannot sustain a weight that someone else can. Yes, if we all starved, we could be thin, but that isnt sustainable. And i ask people to understand weight loss is a lot harder than it looks. It takes both mental and physical effort. It can consume your mind until its all you think about. I want everyone to be healthy, find food that is nutritious and satisfying. Find movement that makes them feel happy and do it often as possible. But that does not mean its ever ok to bully people because of how they look.

Doctors can learn a lot in their own fields. So can people in a lot of other fields. Doctor Mike agreed with this statement in the video. Initial medical training isn’t all encompassing, professionals should spend their careers continuously learning, as they are confronted with new challenges and because the field evolves constantly. I quite like Doctor Mike, and still feel he could still continue to learn about working with fat people and considering our bodies from a health at every size perspective. Not because his way is inherently or always wrong, or because I think he’s fatphobic, but because there are other approaches and considerations about obesity. There are a growing number of people who are living fat and healthy. He would do well to talk with an actual fat person/fat health advocate about this.

the problem with doctors hyperfocusing in weight is that they tend to ignore everytging else because of that. i know for me, its completely fair to bring up my weight when i talk about the issues i have because im sure it affects it. but just saying "just loose weight" completely ignores the other symptoms that i have that arent necessarily caused by weight. like i seriously doubt loosing 10 pounds will lessen my extreme migraines. i personally really struggle with exercises because my joints so badly i can barely move the next day, regardless of my intensity. weight is definitly a big factor in health, but its not always the big boogieman people think it is. sometimes its just a symptom

Exactly! And it’s an all too common experience for people. But I do know that in one of Dr.Mike’s videos he does actually call this out. He had a patient where other doctors had dismissed all their health concerns because of the patient’s weight saying it was because they were obese. But Dr. Mike was the first one to get him properly checked out and he turned out to be something incredibly serious and had nothing to do with the patient’s weight.

I understand where you’re coming from and I agree that there are cases where a doctor hyper focuses on the obesity when there are other things going on. However, what I feel that you’re missing is that a doctor will look at all potential causes of symptoms, and if there is a treatment that can overlap and help multiple things at once, which is particularly the case with obesity/losing weight, they will prioritize it over other treatments because there’s a likelihood that it will positively affect the symptom you are coming to them for, it will increase your overall quality of life, AND last but not least, even if it doesn’t change the target symptom, it makes you healthier and safer for further treatments such as medication or surgery and can also make identifying the root cause of the symptom easier. So I’m sure it’s frustrating to always hear about your weight, but the reason doctors hyperfocus on it is because it’s truly so far reaching within the human body and can snowball into so many other health issues that on the surface may not seem related that it is ONLY a positive thing to focus on and will move you forward in one way or another.

It's because some people only achieve body positivity through denial. That is the #1 reason right there. A lot of fat people are completely aware of the very real health risks that come with being over weight and for some of them that makes it really hard not go through life hating that extra weight. Some fat people want so badly to feel good about their bodies that they strive for body positivity in the most mentally (and physically) unhealthy way possible, which is denial. This denial is a process that usually starts by looking for other fat people who have already achieved some level of body positivity. They observe these people and weed out the ones that take the "body positive while working to be healthier" approach because having to address the health aspect gets in the way of the goal. They then start focusing on fellow fat people who adamantly insist that they are perfectly healthy and who tend to cite bogus medical articles as their justification. Body positivity without denial DOES exist but only when people have psychologically healthy reasons behind it. People who just want to be told that they aren't actually unhealthy so that they can feel good about what they see in the mirror are the people who throw around "fatphobic" and insist that anybody who claims to care about their health is a liar who just hates fat people. Their friends and family can have been expressing concern for their health for years but as soon as they achieve unhealthy "body positivity" through denial they suddenly accuse those friends and family of just hating fat people. It's a really big problem that's becoming more and more common as more and more false information is spread. It's very unfortunate that this problem has also started to fuel the misconception that all fat people are in denial and just don't care about their health when that's not true at all.

If I go to the doctor and speak about something that could very well be related to my weight I'm totally fine with them bringing it up or if I go to a checkup. I mean the whole point of the checkup is to talk about my overall condition. But I don't want to talk about my weight when I'm there with something acute that is not related to my weight when I know the doctor already. So if I'm there because I have an infected wound I don't want an assessment of my weight but if I'm at the gyno and they tell me I have high blood pressure than yeah absolutely do, that's the doctor's job. Just show a little compassion. So I don't get this criticism here either because as an obese person I never felt doctor Mike being fatphobic. I also don't get what's wrong with intermittent fasting. I hear you, but just to play devils advocate, a lot of fat people hear this again and again from people who don’t know anything about their lifestyle. I just think there’s a possibility that this person just took it personally because maybe they’ve heard it before from people who aren’t actually knowledgeable and since he’s not their doctor, they felt like he made blanket statements (which may or may not be true, I haven’t seen many of his lifestyle type videos). Like I said, just playing devils advocate

Hmmm...I agree, but they do have a point that some of the diets he has tried out here are quite extreme and often lead to people developing eating disorders. Something like keto, for example, ist often practiced by people who don‘t understand the risks in unsafe ways and his videos did not really deal with that as much as they could have. So I can see the angle of: Maybe a popular KZbin doctor shouldn’t be trying these because people will carelessly imitate him. Of course that doesn‘t mean he‘s fatphobic and I understand why he‘s done these videos- I just fear that they might do more harm than good.

BMI has a big issue though: It does not account for bone density, fluids or muscle mass. If i am 1.76 m and weigh 80kg, but most of that is muscle i still fall under the slight obese category, yet my only fat is around the stomach area and most of my muscle groups are well defined and developed. BMI is not everything doctor. I really liked that you pointed out that the BMI is not everything and said that our current physical health and BMI do not always corelate. Only issue with having abdominal muscles worked but not a six pack is the nutrition. You really have to cut out on the calories and sweets to really see it and that is not for everyone. Everyone should exercise though. Cycling, walking, calisthenics, weight lifting. Keep in shape and healthy to live a longer life than you would have otherwise. You raise your chances of having a well worked and maintained immune system and a longer life. Too much muscle can also be detrimental. I remember some powerlifter level individuals having to sleep with masks that help them with oxygenating their organs in case they end up short of breath at portions of their sleep.

I don't see Dr. Mike being fatphobic. I will say I have been to doctors that completely disregard the reason I came to the doctor's office only to hammer on about weight. Saw one about a ringworm infection and he just went on about weight this and that, when I had no prior relationship with him and I had not had any major changes since last time I was at a doctor, so it felt unwarranted. For years I avoided the doctor because I knew the conversation would turn into what I perceived as shame and not care. My current doctor is great! She talked to me about risks, tested me for common issues, pointed me to some programs, had me talk to a sleep Dr. and a nutritionist and voila I'm down ~100 lbs and have kept it off for about a year now. This conversation needs to happen, but trust should first be established. I'm still on a weight loss journey, but weight is only a passing mention in doctor consultations...

I think the "body positivity" thing is actually just abt appearance bc the hourglass skinny body is what women are pressured to achieve. Like that's the reason for the mouvement. It's bc of the pressure society puts onto people to achieve the desired body or "body goals" and it's talking abt appearance. Loving yourself genuinely imo is taking good care of yourself, eating what's best for you, getting exercise and sleep and just listening to your body's needs in general. Wow. This was incredibly based and I seriously believe that you're doing some insanely remarkable work here, Doc. My dad was obese for a long time. What has been helping him remarkably is a keto diet and working out, and it's helped him sleep better. He doesn't suffer from insomnia and sleep apnea in the way he used to anymore. He's the healthiest and happiest I've ever seen him. My stepmom is incredible with natural and holistic health matters and she is the healthiest person I know. She has changed my dad's life for the better and I cannot thank her enough. This all goes to say that keto isn't for everyone! And that's okay. Just in my dads experience, he's gained back so many years of his life. Overall, I'm sharing this story because it proves that people are different and different things work for different people. And as for Mike, I appreciate that you are as unbiased as you are because that is so incredibly hard to do these days without potentially receiving backlash.

I have been shamed and dismissed by many doctors when attempting to address my own weight. I don't condone the post this person made, but I get the feeling they have been shamed and probably not helped by the medical community. Sometimes, there is a disconnect between the emotional impact of this trauma when a doctor says "eat right" or whatever. For some people they've tried it all to no avail and I think there could be some benefit from doctors having more sensitivity training around this topic when addressing patients directly, and utilizing more comprehensive programs that address emotional impacts and trauma that affect someone's weight loss journey...the medical approach isn't always enough. One specific treatment or diet won't work for everyone, and it's not always someones fault if they can't lose weight. I do think doctors can be fixated on that last one...it must not work because you didn't do it right mentality. Maybe it didn't work because their weight loss journey requires something that wasn't previously considered. That being said I love Dr. Mike and I'm sure receiving that post was heart breaking on a certain level.

That is still their problem. They don't get to accost a doctor for that who is genuinely doing their job.

@@ThaFuzzwood Are you really justifying condescension on the doctors' part? I guess when patients experience forced hospitalization and physical restraint, and develop serious trauma from it, it's also the patients' fault then

He’s a freaking doctor. It’s almost universal knowledge that obesity is literally a killer. The man took vows. He spent years upon years in school. He’s not making stuff up. To quote John Mulaney, “it seems like everybody, everywhere, is really mad about everything ALL THE TIME.” Come on people... 🙄

I was fat growing up and am still losing weight but I know the struggles that comes from trying to lose weight, these people decide to not challenge themselves and decide to blame the world for trying to help them, for anybody not fully convinced just try reducing your calorie intake , it’s hard at first but eventually you will easily be dropping weight and you will feel better, you will be more confident, and you will perform better at your job your everyday life and practically anything else, trust me give it a shot there’s no better feeling than stepping on that scale and seeing it drop
