Glucose metabolism must also be discussed by liver function. Not enough emphasis on keeping the liver functioning optimally.

Why is it that some labs use percentage insulin sensitivity and percentage beta function in the calculation of HOMA IR ? What is that new formula?

Fasting versus Vigorous Exercise is like comparing Apples and Oranges. Both fruits are good for you.

Can this be extrapolated to apply to pre-diabetes?

Of course! The only difference between pre-diabetes and diabetes DX is your A1C, 6.4% is pre-D, 6.5% is diabetes. They are both an expression of insulin resistance and high blood sugar. It’s the same disease with the same cause, one is just a little more advanced than the other. So they would obviously both increase the risk of Alzheimer’s. However, if you have pre-Diabetes , it’s entirely likely that you haven’t yet caused all the damage which is required to happen in order to set yourself up for Alzheimer’s. There’s more chance for you to fix things , lower your A1C, and therefore your risk

It's about hyperglycemia, actually. So, if your blood sugar is high...

Low fiber/high sugar diets all restrict blood flow to microvascular tissue. That's the brain, eyes and genitals. That's why diabetes/pre-diabetes and metabolic syndrome all list them as symptoms; brain fog, blurred vision and ED. So yes.
