As I see it, an outside force has entered a ROM of Sonic 3 and, at the same time, accidentally made Tails self aware, becoming aware of how many times he's died, OVER AND OVER AGAIN. Now bearing the burden of knowing this, he suffers from a mental breakdown, not wanting to die any more. During the "Escape isn't an option" ending, Sonic investigates a little too much and winds up taking Tails' place, going insane and tearing off his face and torso with his bare hands, saving the fox but getting caught in limbo, midway between a "Game Over" and simply respawning with an extra life. And that's my breakdown...

You know, the ending where Sonic mutilates himself kinda looks at Pervision’s appearance. Think about it... Pervision has a hole in his eye When Sonic moles out his eyes it ends up with at huge hole.
