The thing is, a lot of people on this show have experienced some sort of trauma and they use food as a coping mechanism. Yes as Dr. Now has explained many times on the show, someone at such an obese size has stretched out their stomach hence why they eat so much, it takes more for them to get full, the surgery shrinks the size of their stomach so it's supposed to help reduce how much they can physically eat. So many times he asks them to lose 50 pounds in a month on their own before he'll do the surgery to prove their committed to it, sometimes more, He can come across as harsh sometimes but this is life and death and I would imagine because so many have come to him for help he's had to turn away a lot of people too just because of demand. So it's gotta be frustrating for him when someone isnt putting in the effort and wasting his time when he could have helped someone else who would have.
