I love Dr. Ricucci and his invaluable work for Dentistry. He is a strong defendant of total carious tissue removal as he has proved histologically the presence of bacteria in the canal when carious tissue is left behind. However, the recent European guidelines advise against complete removal in several situations and backs this up with clinical, long-term studies. In Periodontics we always have histological inflammation but no clinical inflammation. Shouldn't we take the same approach to Endo? Having bacteria in the canal may not necessarily be a cause of clinical disease.

You are correct that Dr. Ricucci needs to update his mindset that new evidence shows that our understanding of the implications of the histology needs to be updated since new empirical evidence shows that the old understanding is incomplete
