2 epidural shots didn't help me. Neither did nerve block and nerve ablation. My sciatica was crippling and I was using a walker the pain was that bad. 3 orthopedic drs recommended spinal fusion. I refused to believe I needed that drastic surgery. Went to HSS in Manhatten and saw my 4th orthopedic dr. He recommended micro surgery of a laminectomy. 2 hr surgery. Went home that night. 2 months after surgery I'm sciatica pain free. Still have back pain but livable. 100x's better than crippling sciatica from back to feet. Thank God for Hospital for Special Surgery. They believed what I believed that spinal fusion was not needed.

I’ve been getting epidural injections at L4-L5 for pain and sciatica from degenerative disc syndrome and spondylolisthesis for 18 months at three month intervals. Pretty good pain relief. I am fine with just local numbing. I di pilates, lots of walking and stretching. Trying to keep going and put off surgery as long as possible. These videos are very helpful in explaining the procedure.

I have suffered for years with severe Sciatica. I would get minimal relief and pain would always return 4 weeks after the epidural steroid injections. After years of receiving the maxim doses allowed my Dr suggested a Spinal Cord Stimulator. He referred me to a Neuro Surgeon and after two years I continue to be pain free. it was done in the hospital as Day Surgery. It is wonderful. No larger than a pacemaker. The best part is that I am pain free!!!!!

After suffering for over 3 years, ordering a cane, laying on acupuncture mat, hot tub, massages and drinking too much wine....taking ibuprofen, tramadol in order to work, getting ergonomic desk chair, computer stand, stopping all my dancing (ballroom and swing), I FINALLY did this procedure. I elected semi-sedation which was perfect for me. Doctor used much numbing solution which was fine until he “hit the spot”.....even with semi-sedation I about came off the table (I was in surgi-center) and I heard myself say “oh F”! I am 64 and would never ever say this in public. Of course, I felt amazing right after the procedure. Went to lunch and was able to enjoy sitting in a booth for a $65 lunch with my driver. At 16 hours and I am nearly pain free (soreness) and to me, it’s a dang miracle. Yes, I understand this won’t last but I will take what I can get! I am able to go up stairs, sit in a chair or car without being utterly miserable. I was told it usually improves over a month or so..... I am thrilled, pain went from 7 to a 1.

Had this done for L4 L5 complete herniation (blow out) in Oct of 2021 lasted until 4/6/2023 and got 100 % relief unless strenuous work was done as in outside yard work or working on vehicles. I was amazed. Hope this latest round this week works well, but pain is nowhere close to what it was before. I could not sit and barely walk. Two little toes were numb. Now only lower back pain when waking or trying to sleep but hope it goes away after injection this week.

I say that because they tried it on me sitting on the edge of a bed with the X-ray machine around me the doctor put the lidocaine in as soon as he pulled it out he tried to get the steroid shot in and was unable to because as he said the spondy is too hard I took that the spondonosis was too hard and too close together to get the needle through I never did go back to be put under for the shots but I've been looking for someone to do a laminectomy I have discs missing and pinched nerves I think that would be the best but laminectomy thank you for all your videos they're great

I had the injection about two months ago for L3-4 herniated disk with severe nerve impingement. It was absolutely the most painful process I have ever experienced. I wasn’t given any pre-procedure medication. Didn’t feel like there was any anesthetic given at the time of the injection. I was not prepared for the amount of pain I experienced. Great video, thank you for sharing. I had my 1st lumbar epidural injection on Tuesday morning (3/12/24) with no sedation, just a local anesthetic, it was quick and painless. The rest of the day I felt good, a little uncomfortable, the next day (Wednesday) I felt so good, almost completely pain free! Today I woke up and my low-back felt a little tight. Like you guys, my Doctor also told me it may take several days for the steroid medication to work itself throughout that area. I hope my first shot brings me pain relief for a long time.

Me too. I got this ESI done but it was a totally different procedure. It was done while I was in a sitting position and two nurses held me tight from the shoulders. There was no CT scan or anything it just injected blindly. I was given anesthetic prior to the injection but I was in severe pain I almost fainted and had a blackout too. Now two weeks in the injection, it cured like 75% of the pain I had. But still a slight numbness is there in my right leg and I can't walk properly. Also I can't grip tightly from my right arm.

Doctor tells me not to move as he's sticking needles in my spine ! Almost impossible. I feel like I have PTSD from my constant back pain and all the very painful procedures I've had ; a discography test , numerous lower lumbar epidurals injections, nerve blocks didn't work and 2 failed back surgeries . I had an injection L5 (in a CT facility) and would not recommend driving home. I walked from the scan room to my change room where I sat for 10 minutes. I was informed I could go and could not walk in a straight line, it felt like I was inebriated. I sat at Reception waiting for my car ride to collect me. I went outside to meet the car, had a vasodilation (chronic sweating and loss of hearing and eventually vision) and collapsed. So warning to everyone do not drive yourself, and if you can be put into a wheel chair from the scan room to reception do not hesitate to ask for one, just in case the same happens to you. H&S.

Yes some do get short term relief from radicular pain, but that group is a narrow percentage. The FDA’s 2014 AADPAC hearings on the safety and efficacy of ESI therapy found that the majority of patients (68%) fall outside this narrow window. In fact they refused to approve all ESIi’s and stated that serious adverse medical events including death have been reported. They added that fluoroscopy did not prevent dura puncture and concluded by adding a serious new WARNING to all injectable steroids that this procedure should not be attempted. All true doctor, all of it!

My experience is much like the video. I presented with L4-L5 stenosis and some other problems 10 years ago after several days of strenuous lifting heavy objects in odd position. Severe 10/10 pain - hip, leg, buttocks, sciatica - complete agony. Got some immediate relief from a course of methylprednisolone, but temporary and not complete. Had the epidural steroid injection and have been mostly pain free ever since. (One spinal orthopedist at the same practice said I had to have fusion surgery and laminectomy immediately or it would get worse). I'm retired (desk job) but have always been active in sports and now very active in a lot of home improvement projects. I occasionally experience relatively mild flareup if I lift super heavy objects or otherwise overdue but generally a day or two of ibuprofen is sufficient. Two years after the lumbar stenosis, I presented with cervical spinal stenosis, severe head and neck pain. Received cervical epidural with pain free result. Had one bad flareup a few years after but knocked it down with course of methylpred. I may be luckier than most as I have had numerous cortisone injections in multiple joints (arthritis) over the years - both shoulders, both knees, elbow, finger, thumb - and respond almost immediately, usually within six to 12 hours or less with complete relief within 24 - 48 hours. Good luck to all dealing with back issues.

I was ok, then, My second injection about 2-3 weeks later, was late as the doctor was so behind with patients. By the time I went into the operating room I jerked from the first injection because my body felt it. The doctor looked at me then proceeded to do the next injection on the same side. I felt that one too! About a month later I was having so much pain and sciatica I never had before. I was in worse shape than before. I believe I had nerve injury. And I ended up with diabetes right after that procedure too. This was done in Las Vegas.

First injection in the office setting didnt help at all. The 2nd injection done at a surgery center with the "relaxing drugs" made a HUGE difference. I think I was able to relax enough for them to get the needles into the correct position and not "rush" since I was in so much pain in the office. Give injections one more try. I have also tried acupuncture, chiropractic and physical therapy without any relief. I had my 2nd injection over 2 weeks ago and while I am not totally pain free (Still pins/needles down my toes) I am able to cope and I am not as irritable. GOod Luck!

I’m having occipital block done next Wednesday for the first time which I’m more than quite worried about but I’m so grateful to find your videos I suffer chronic lower back pain and sciatica. My local hospital past it off and just said to take naproxen when it’s too much to deal with. Sadly I’m in the uk as if I lived near to you I’d be asking for any and all help you could offer Will definitely be going back to my go to ask about this nerve block treatment for my back

Did for L5-S1 radiculopathy and transverse nerve compression. I felt so much worse after the injections. Say you may be sore for few days after, inflammation from sites. But never got better, it not only didnt get better. Stay same... It got Worse. ...Now because being tossed around im doing surgery cause i cant walk, sit, ly down, worrying about permanent nerve damage. Pain is unbearable. Even if it does work it can take weeks to months to work. So of course 0 pain meds were given to me. Not during or short term after. Gotten so bad i have nuero system issues. But i still have hope ill be in not as much pain. Anyone else relate. I know everyones situation is different even same diagnoses. Different outcome. Anyone have any info for me. Thank you all. Take care

What a totally professional procedure and accompanying description by Dr. Su. My experience was quite the opposite. I believe the injection was put into the wrong site based on my pain profile so I wasn’t surprised that it didn’t provide any relief whatsoever. Can’t believe the difference between this comprehensive procedure and what I would liken to a “fast food approach” to the epidural injection I was subjected to. Summing up it seems to have been a complete wast of time. Australian patients deserve better.

Australian here too. Pain relief lasted about a week and I was back to terrible sciatica two weeks later. Happened to be overseas where I saw a neurologist after the pain got unbearable, and basically I was injected at the wrong site and at only one level despite needing two levels. He also couldn’t believe its just a radiologist who does the procedure in Aus. Now been pain free for 6 months thankfully.

Anyone can be on their best behavior if they know they're being filmed! I do hope this worked out well....But my experience doing these injections was HORRIBLE! l was getting these injections several yrs ago every three months. Despite they did nothing for me, I was told it had to be done bc of insurance. The last time I went I was lying on the table waiting for the dr to get off his cellphone. Then when he finally came in and did the injection, I heard the nurse gasp and immediately felt warm fluid on me, then my heart was RACING! It felt like I was going to explode, my head was pounding so hard and my BP shot up 200+/100+! All they did was give me Tylenol and have me wait until my BP went down. Of course the dr was nowhere to be seen. I felt like I was going to die when that happened and I've never had it done since. Insurance procedure or not, it's my body and if it's not working I shouldn't have to expose myself to it! Do you think any of that episode was documented....of course not! I need to start recording all of my visits to dr, hospital, etc., because in the end it's their word against yours...we need a voice! And people were lined up for these injections on gurneys in the hall, in rooms, and in talking with several of these people they all said the same thing...they don't work and if by chance they do work it's not for long and that it's all about the insurance money! The older I get the more I see this type of practice and it just sickens me! Most drs seem like politicians....they're just in it for the money. This is just MY honest opinion and experience to those of you who are ready to start with the hate comments!

I hate this, it’s the worse type of pain to ever endure as a 29yr old almost 30. This kind of pain has taken almost 3 months of my life of not being able to do things I use to do. Been out of work for 2 months because standing all day on my feet and having my body in awkward positions as a dental assistant is impossible to do the work I do. I received my injection 3 weeks ago and sadly got no relief. I wish nerve and lower back pain wasn’t a thing at all.

From what I read, getting Steroids via Epidurals, deteriorate connective Tissue. I suffer with a number of Spinal Maladies, L5-S1 Spondylioethesis for years, and DDD in most of the spine. When I was getting an Epidural at L5 site, the Doctor doing it said as he was administering the shot with Flouroscopy, You have mighty Thin Discs, Mighty thin. The Epi dis nothing to help. After a round of Facet Joint injections, about 6 months earlier - did more to help (at L2 -L3) for about a week !! The second round of Facet Injections as it was being applied had My head spinning . the dr. had to STOP! We can send a Rocket to Mars, contriol a Vehicle on Mars' surface form Earth, Yet to date, This is all we have for Spinal Issues?

The steroid epidurals do deteriorate hips even though they are in the spine. They're dangerous. Steroids are not recommended to be used in the spine & it says so with the steroids pkgs. Yet the drs do them as often as possible even though they do know the damage they can cause yet deny. I originally had 3 in 2008 as well as PT etc when I first hurt my spine. Didn't help. I was told in 2015 by pain management that there was nothing they could do that would give me relief or help. In 2016 when the CDC Guidelines came out suddenly they wanted to do steroid injections & ablation. I've read a lot of reviews on the injections & researched it a bit & evidently they didn't help many people & they are dangerous. Some people ended up in much worse condition. I would rather have my meds cut off & die rather quickly from the heart attack it would cause than to participate in more unnecessary pain & deterioration from these injections.

Dr. Su, I want to thank you for this content. I found out on 12/23/22 that I have severe multilevel advanced degenerative disease with multiple herniated discs and severe multilevel stenosis from c4-5 through c6-7 AND severe multilevel degenerative disease and stenosis with herniated discs in L3-4 through S1. I also have a rare condition called osteopoikilosis, which in my case actually does cause considerable pain (I understand that it is normally benign and painless). I used to be 6’0” and now I’m barely 5’8”. I’m unable to look up, and even holding my head up normally is getting difficult. It’s reduced me to sitting in a chair, in constant pain, and I can barely move. I already have pretty severe diabetic neuropathy, so the effects combined are pretty miserable. I have to wait until the end of February for a consultation, which is a very long time to wait when it feels like you’ve got a baseball in your backside (and occasional hornet stings). I am still trying to come to terms with everything, since I only found out 3 days ago. You’ve given me a bit of hope, however small, that I won’t have to spend the rest of my life in pain. Thank you.

Dwight, I feel and Hear You. I'm in a comparable position. I have disc issues from L-2 , L3 L5-S1, T9 , T10, and up to the top of my neck C3-C7 have issues too. 6 Spinal and Neurosurgeons consulted - all told me that there is No Surgery for me. Too much arthritis in the spine vertebrea and Facet Joints So I can only deal with it with Pain management. Had facet injections which was good for 1 week, the epidurals in the Lumbar and neck - didn't do anything!. Lots of DDD too & Dextroscoliosis of 25 Degrees. I'm 71 YO, and have suffered over 45 years with back issues - progressively worse over the last 15 years. I used to 5' 11 1/2 " tall, now about 5' 10". I'm too Fat (can't lose weight !!) at 225 lbs (ranging from 220-226). My goal this year : is to continue to pursue answers for these health/ spinal issues With the InterWeb, answers are out there. You must do your own research - the Doctors have no time or no Interest to seek and treat the causes only Treat Symptoms. Try Supplements to see if they work for you Turmeric, Ginger, Boswellia, Bromelain, White Willow bark [Nature's Aspirin], MSM, etc. Chronic Pain (and sometimes to me, it's the horrific Stiffness in all my joints and back that is Worse than the Pain), SUX and destroys your Life and well being.

I just had my 3rd round of lumbar epidural injections of L1-L-4. Back in May, I had radio frequency ablation on the same vertebrae. All accomplish the same thing, a temporary easing of most of the pain. But it just puts off the need for surgery until they don’t work anymore. I’ve see 3 orthopedic spine doctors, but they all wanted to do something different and didn’t impress me that they had the desire or a definitive plan on what to do. The first one spent a total of 4 minutes with me in the exam room, never discussed the MRI and other tests I had done, and walked out of the room. I waited 6 weeks for an appointment with this clown. I’m seeing a spine neurosurgeon next week, and we will see what his thoughts are.

I didn't want to miss speak so I just looked it up again, do you tell your patients that corticosteroid injections are NOT approved by the FDA in or near the spine because i was not told anything about it when I had mine done in my cervical and lumbar spine and both made my pain worse for about 2 weeks then it calmed back down to it's "normal " level the same thing with my knee

@@DrBrianSuTheSpineGuy I understand off label use ( used for something other than the original intention) it's being used for it's intended purpose it's just not supposed to be used in that part of the body and of course all the FDA required was a label change that in my case and I'm sure most the patient never sees or is told about and in my opinion it's unethical. I'm not saying this about you and I appreciate you answering yourself but we'll just have to agree to disagree

I have received several epidurals, 3-4 months apart. Yet my anesthesiologist refuses to give me another epidural because I am diabetic. Being a diabetic for over 40 years, my body has built up a tolerance toward insulin. My A1C is 11.2. My anesthesiologists wants me A1C around 8. My primary doctor added Victoza daily injections. My glucose has been decreasing. Back pain gets really bad, sporadically. Pain shoots down my right leg. Epidurals involve steroids which increase glucose levels. Are there any other alternatives?
