As a pharmacist, all the steroid rx I’ve seen never come with duration of use instructions. And when I ask patients, they haven’t been given verbal instructions either :( I wish more prescribers are as clear as you are. Instead, what I see is topical steroids repeats over YEARS until someone intervenes. Thank you for providing this info to patients so they’re aware of appropriate duration of use. It’s so often overlooked by prescribers in the community.

but TSW/Red Skin Syndrome isn’t rare at all…there are thousands of us who had eczema as a kid who were constantly given steroid creams and are now suffering enormously. We are just ignored.

I dont even trust doctors/ dermatologists on this- those of us who had eczema as kids know that as you grow up the doctors they just tell you to keep using steroids and give u higher strength when the flare ups keep coming back worse. How come doctors who know uve been on steroid’s for years still keep telling you to use it when everyone knows its not for longterm use but it u try to tell the doctor they just ignore u and give u higher strength steroids.

@@salahelbaroudy1932I’ve been using topical steroids for ages and it helps me immensely when no other cream for eczema did. Just keep using it as directed and if you notice anything tell ur doctor, if it works good and there’s nothing going on then your completely fine. I’ve had zero issues.

So true. Most people have not been given good guidance when it comes to how much to apply and how often. Than can cause some problems if used at high doses for prolonged periods.

I got prescribed hydrocortisone for rosacea two days ago and I’ve been so afraid of using it based on the reviews I’ve read, now after watching your video I feel a little more safe to use it, or at least knowledge as to how to use it. also when dermatologists prescribe steroids they don’t do it under “careful supervision” they are in and out of your room in five minutes with a prescription. and they say use “as needed” this is not the patients fault

EXACTLY!!! Mine are the same way. They are in and out and as needed. I wish I knew this many many years ago on how bad hydrocortisone can be for your skin when you use it excessively. I used to use fluticasone years ago since I was little all the time on my face not knowing how powerful it was. Then they switched me to hydrocortisone which is not as powerful I think but I still didn't know it was still bad for my skin. I get eczema on my face off and on. I just wish they told me how powerful this product was.

I only use hydrocortisone now if my skin is REALLY BAD and in need... Now if it's just a little flare up...I use Cereve PM on my face and it really helps the dryness I get on my face. Especially in the winter time. I can't live without it now since I started using it a little over a year ago lol

A while ago I had a rash on my leg and my dermatologist prescribed me a steroid cream which I used it twice a day for 2 weeks and it worked. This is a Godsend! Thank you so much. I've been battling ACD on my face/neck for 4 months! I was given a topical steroid but too scared to use it on my face. I've literally had anxiety attacks over my condition b/c when I go online, all I see are worst case scenarios...TSW, Red Man...and I panic. I have applied the ointment for 2 days...see it's better...stop. And, yes, it comes back. My dermatologist just told me it was safe to use as much as I want on my face until the rash is gone. I didn't feel very encouraged by that bit of instruction. I so, so needed this video.

@@pedromcintyre8943 Hello. My post was 2 yrs ago. I finally found a Dr. online who deals with TSW patients and took his advice. I cut out all steroids. I was warned it would get worse, and boy, did it ever. My face was weeping and swollen! It took me 1.5 years to finally realize I was going to be okay. Today, I'm free and clear...just back to breaking out on my fingers. I cut out chocolate as I saw a connection. I have to be careful about outdoor pollen. I can't give you advice, but for me, it was a reaction to steroids. I know if I have any more flare ups around my eyes/cheeks to just ride it out. No more steroids. Also, I refused all doctor's (allergist/ 2 dermatologists) suggestion to go on Dupixent and glad I did. May work for some, but it wasn't the answer for me.

Hi Dr Dray, thanks for this video! I had a reaction to something on the tops of my cheek once, I knew steroids would clear it up quickly so asked my doctor for something gentle that I can use on my face and she prescribed me mometasone furoate in Elocon Alcohol Free Cream 0.1%. Cleared it up really fast and only needed to use a tiny tiny bit of it. From your video it is nice to see that this comes under your "potent" heading otherwise I would have had no idea what it was. I also have super sensitive skin, especially to fragrance. I use Banana Boat Simply Protect sunscreen for babies , which is really gentle and non-irritating. I can even put on my neck.

It's ok being prescribed steroid creams however like myself who's now going through topical steroid withdrawal I was presibed a topical steroid for my eczema and because it didn't help my doctor then presibed it for longer, then another more potent steroid cream, again for longer and despite them seeing they were not helping and infact making this worse they continued to prescribe them. Being a dermatologist and doctor I trusted in them. It wasn't until I was classed as one of my dermatologists most severe patient that I researched into it all and found the tsw community. We are now in touch with the government to get this recognised and put a stop to this

My two-year old gets severe dermatitis from mosquito bites. Huge red plaques that become really hard, red and inflamed. Cortisone cream is the only thing that helps I'm afraid.

Hey doc. I’m currently dealing with acne rosacea with bad inflammation. I’ve been prescribed 3 creams: - Clindamycin - Pimecrolimus - Desonide ointment The desonide ointment is only to be used for 2 days because I’m so inflamed that I’m not leaving my home. what is considered prolonged use though? using it a few times a month for years? or does it mean every day for years? My dermatologist told me to use it for 2 weeks every time I have a flare up on my lips, though I don't like using it for that long so I tend to do 3 days each month, sometimes every other month whenever I have a flare up.

I’ve used a strong steroid cream for atleast 5 years, probably more. I’ve tried twice to stop using it and after a week my skin is the worst it’s ever been and I’ve had to go on antibiotics and now I’m having to take steroid pills because it’s so bad :( I don’t know what to do anymore. I feel like because I’ve used it for so long I’ve ruined my body.

I would love if you did a video on Fatty Alcohol’s derived from palm -coconut oil (Cetyl, Stearyl, Cetearyl) and the denatured alcohols like Benzyl, Isopropyl, tert-butyl, ethanol, etc - AND why skincare companies put the denatured ingredients in their skincare - I know it is to either stabilize the other ingredients and or to push ingredients like Retinol and VIT C further into your skin... this to me is crazy!!! I always try to find products without ANY ALCOHOL OR FRAGRANCE (except the fatty alcohol) - my main QUESTION IS: What ingredients should chemists (skincare companies) be using instead of the denatured alcohols??? Hi Dr Dray. Thanks for the very informative video, i learnt a lot from it. You mentioned that a dermatologist will usually advise different steriods for the face and body with differing potency based on the skin. I am currently using eloment on my face and dermovate on the body, i'm not too sure about the potency but my dermatologist assures me its mild to mid strength. But i have never had a dermatologist never informed me as to the duration to be used, usually i will stop once the skin has cleared. Could you advise generally how long are steriods to be used for a flare? As for TSW, I think i may have experienced it recently as i had an extremely bad flare on my face and neck where it was extremely red and patchy but it was mainly inflamed and itchy ( i dont normally have severe eczema on my face and neck). I did go to a dermatologist for consultation but she just mentioned it was a normal eczema flare. Would love to know more about this, would be great to learn more on the topic in a video from you.

Great comprehensive coverage, but one side effect I was thinking about is depression or irritability. I don’t think you mentioned this in the video. I just listen to your more recent mercury skin cream video. Someone in my household used and tried to sell mercury containing skin cream. I sent it to the cdph and they confirmed and added it to the database. She just purchased some more skin cream with a new label (no ingredient list) and some of these issues are coming back. I I realize hydrocodone on mercury and steroids are some of the common unregulated ingredients in these colorism issue skin creams. I don’t know that this new skin cream has any of those yet, but I was just wondering if steroids could have the rage/depression/anxiety side effects.

How often have you had clients come in with rashes you cannot identify? My son had a couple of rashes his dermatologist could not identify. He said he has a couple of theories and to bring him back in a couple of months or sooner if its not getting better. He was prescribed hydrocortisone for his face and clobetasol for his hands. The pharmacist said to use them only a couple of weeks. The doctor said there is refills so he can use them a couple of months. Would you recommend a client taking these to take break from them between refills? I will of course ask his doctor as well I am just curious if you have dealt with similar.

Dr. Dray: thanks to you I’m seeing a derm and have been diagnosed with lichen Planopilaris, on clobestrol propionate foam and oral antibiotics/anti-inflammitory pills. No one seems to know how often I should wash my hair. I use the foam MWF so should I wash Tue,Thurs, Sat? Should I wash it out same day? Thank you so much for your videos!

I'm sorry to hear you are also having to deal with LPP. I was just prescribed betamethasone propionate lotion and I've got the same questions regarding hair washing/application days. My rx says to apply daily on the scalp but I'm unsure of whether I need to wash it off daily before reapplication. I'm already losing soooo much hair daily... and my scalp is way too tender to wash it every day. LPP has been a real bummer for me already especially seeing more and more bald areas on my scalp after every shower day. I know the medicine might help me keep more of my hair in the long run but it's still so disheartening to even be going through this at all. Sending hopeful energy your way, fellow LPP warrior. ♥

Just the video I was hoping for! In my 20s and 30s (now 51) I used many prescription strength steroid lotions (Desowen and Diprolene, for example) to treat relentless seborrheic dermatitis of face & scalp. The steroids provided relief, no doubt, and I used them conservatively because I voraciously read all labels/inserts. I am grateful for the often overnight facial redness/itch relief that the steroids provided, however, there was sometimes a rebound effect, and I believe the broken facial blood vessels I now have are attributable to the prolonged use. I haven't used them for over 10 years now. Thanks for putting this information out there Dr. Dray. It's no wonder your reach is growing - 130K subscribers!

Thank you for this video ☺️ I was prescribed clobetasol to use on my fingers at night. I grade my compliance at F-. I am definitely phobic, because of the potential side effects if it was to get in the eyes. My provider said that after about an hour, it should no longer be an issue. I’m still scared 😟 You mentioned a while ago that you like using cotton gloves to moisture your hands at night. Are there ones that you could recommend for use with a topical steroid? Thank you!

My son's dermatologist just kept pushing prescription steroid cream for his eczema. He has lost pigmentation on his arms and leg because of long term use (they didn't tell me this could be a side effect). After changing to a new dermatologist, she started him on Eucrisa and that stuff has been a miracle for his eczema.

Thankyou Dr Dray. I avoided using the topical steroid I was prescribed because I was too scared. My eczema (never had it before) got worse and worse and was full body. I’ve been using Diprosone on it for 8 days now and it is definitely better. At times it looks nearly all the way better but then it will randomly go dark pink/purple. If I press on it it goes back to normal, almost like it’s the blood underneath? Have you ever heard of this happening? Also- do I need to taper off the topical steroids?

I am surprised you have never seen RSS. Im currently going through it now & know alot of people who are. The burn is terrible and dealing with steroid type rosace. I am also someone who hasn’t used alot. For over a year I used on & off Hydrocortisone wrongly from my gp, and they upped the steroid. Know its currently taking over my life. Im on week 4 off burning skin and itchy dermatitis. Please be wary it is very common

As an update; I have decided to do nothing, and just allow my body to heal it self. Looks like the semi-permanent coloring is worst to use than the permanent coloring, so pick your poison. I have just cut back on washing my hair every day and just doing it every two weeks; something that is difficult to do in Florida because of the heat, just have to keep my hair short. I am looking for the simplest shampoo with the least amount of chemicals, don't want to go back to regular shampoos. The scalp is still getting pink and red, but at least I am not getting the blood pressure spikes that I was getting before. I wished I had known about the problems with PPD, I guess I have to research all products before using them.

Thanks, Andrea! I had radiation to the head and neck area 5 years ago and my skin has never been the same. I often get dermatitis and it gets worse in the Summer. I've determined that I cannot use spray sunscreen on that area (nor should I since I've learned better 😉) because I break out in a terrible rash. I was given betamethasone cream which helped. Also, the Aveeno Excema cream has worked miracles in that area!

Got the most uncomfortable, itchy, raw papular eczema on my fingers/hands right now. Hydrocortisone doesn't cut it. Trying Mometasone furoate ointment now with cotton gloves and Aveeno cream. You don't appreciate how great it is to have normal skin until something like this happens! Ugh.

Ok, I have questions! I have been prescribed clobetasol for eczema. I tend to flare on large areas of the body. I have been dealing with it for going on 19 years. The only thing that has ever “cured” me is light therapy. It really works for me. I stopped it after being clear for several months. It seems to be coming back with the summer heat. What do you think about the risks of uvb light therapy? I plan to start it up again after I see my derm later this month. I am a bit concerned about using it long term due the risk of aging and such. I also have been using my cerave on my wet skin before exiting the shower. Should I apply my steroid on top of the cerave or underneath to my wet skin? Thanks for all the great info! (I can’t get into me my derm until the end of the month).

uvb phototherapy does have a risk of that, but it is actually very very safe as the wavelength employed and dosage delivered is strategically titrated

I would stay away from topical steroids if I could, especially on the face. I've used a topical steroid cream 7-8 years ago, and I've been suffering from the side effects ever since (it has very severely compromised my skin barrier, so I was left to deal with rosacea, extrem sensitivity/fragility of the skin, all sorts of inflammation, itching, redness etc.). So be extremely cautious when using a topical steroid, and avoid it if possible on the face.

My daughter used to suffer terribly with eczema, it would be inflamed, bleed, crack open the skin and cover most of her face and body as a baby and child. I would use a small amount of the steroid cream mixed in with a moisturiser which spread it out further and helped keep the skin soft and less itchy. As a young child it kept getting infected due to scratching through the night. One particular year it was very bad she was around 8 years old when I found a set of goats milk products, a wash, lotion, cream, shampoo and conditioner. Within three days of use the eczema had calmed right down, stopped itching and within a week totally gone. She is now 17 and we recently tried another goats milk product as we couldn’t get any of her usual products and the eczema came right back. Now she is almost eczema free again within the first few days of using the original product now we have it again. It is called THE GOAT SKINCARE by Coremetrics, Australian made and owned, and is our miracle in a bottle, and we were and still are completely able to stop using steroid creams. Hi Drey! What defines chronic or extended use? Weeks/months/years? Or varies case to case and strength. I was prescribed a steroid eye-drop (DEXAMETH) as part of treatment for a bad EKC I most likely caught on a long haul flight. Was told to treat for 7 days but been taking extra day cause it’s about 90% better. Definitely going for follow up. Just wondering what time length is considered “chronic” use. Hi Dr Dray, hopefully you see this comment or anyone because I’m concern for my mom’s treatment that a dermatologist has prescribed for her dark spots on her face. Before she used the cream (hydrocortisone), her dark spots were small but still visible on both sides of her face. However, after she started using the cream, the dermatologist told her to use it for 4 weeks, one time a day then stop for 2 weeks. Then again, use it 4 weeks and stop 2 weeks. After my mom used the cream for the first 4 weeks, her dark spots worsen and it expanded on her face. I want to know if this was a misdiagnosis or is this part of the treatment. I joined my mom to the clinic on December 28th 2021, but the dermatologist “left early” even though the door to enter the dermatology clinic was open. They told us we will receive a call back but after looking at the reviews on their website it’s very unlikely they will.

Thank you so much for all this wonderful information about topical steroids. You have really lessened my fear about taking Clobetasol for the last 10 years! Although my Derm. Doc. just switched me to Fluocinonide, because the Clobetsol didn't seem to be as effective as it once was. I have always feared using this for my small patch of psoriasis on the back of my head. I treat it for 7 days per month and that seems to work... but my Doc. has never told me or shown me how to use it! I actually am so afraid of it, I use a q-tip to apply it! Mine comes in liquid form, so is really hard to apply to the back of my head! I have read that Oregon Grape is effective for psoriasis...but I know it's not FDA approved, and my Dr. didn't say much about it when I asked her if I could try a natural treatment...thoughts? I just really wanted to get off this steroid! I truly appreciate your compassion and all this information. xo

oregon grape is not a treatment for psoriasis...see my psoriasis videos...it is systemic disease w/ cutaneous manifestations...treatment is important
