Lymph Node Clinical Examination - Lymphoreticular OSCE - Clinical Skills - Dr Gill
The clinical examination of the head and neck lymph nodes is often the first clinical skills session that new medical students encounter and is a common examination station in medical school OSCEs
Most people will be familiar with the concept of having "raised glands" when they have a sore throat. This is not actually a raised "gland" but is due to enlargement of the lymph nodes of the neck as a result of the immune system reacting to facilitate the immune response to the infection.
Lymph nodes are exactly that, nodes on the network of the lymphatic system that allows the main immune system to react and to focus resources where they are needed within the body.
This lymphoreticular examination covers lymph nodes in the following regions:
- Submental
- Submandibular
- Parotid and tonsilar
- Pre-and post auricular
- Deep, superficial and posterior cervical chains
- Occipital
- Supraclavicular
- Axillary
- Epitrochlear nodes

Please note that there is no ABSOLUTE way to perform a clinical examination. Different institutions and even clinicians will have differing degrees of variations - the aim is the effectively identify medically relevant signs.
However during OSCE assessments. Different medical schools, nursing colleges and other health professional courses will have their own preferred approach to a clinical assessment - you should concentrate on THEIR marks schemes for your assessments.

This is so important yet often overlooked sometimes I feel like, in their late 20’s a former classmate of mine with no risk factors/family history was diagnosed with a fast moving breast cancer that had begun spreading to their lymph nodes and this kind of exam helped to identify it and get them the treatment they needed in time to go into remission and be declared cancer free. It’s amazing the power these exams can have!

As someone who has had thyroid cancer spread to my lymphnods, I can really appreciate this video. I had a lot of attention placed on my neck's lymphnods, and i can tell you from first-hand experience that it was worth it. Great video!

My best guess is he interviews them before the exam and if they do have something he probably recommends them to their own doctor since already have their prior medical history

Yes, we didn’t expect that! But normally the students have no issues, as we need to train on “normal” so that any problem is much more noticeable to us clinically

Have you considered going more in depth? For example when doing a lymph node examination explaining why you are pressing on certain parts or what the more in-depth reasoning behind certain areas of the body?

If I my add my 2 cents: I understand this is done for teaching purposes, but addressing the public would spoil the ASMR effect, because the focus is moved from the patient to the audience POW. I think that people who follow mr Gill for asmr, just like to "peek in" how an exam is done. But this is just my opinion and my feeling, so feel free to discard it.

I've been trying to find these nodes on myself but I have no clue what they are supposed to feel like, and I haven't been able find anything. What should they feel like, and what does it mean if you find them/not find them?

@@MisterTipp it’s good that you can’t feel them! Most lymph nodes are not palpable unless they’re swollen, and swollen lymph nodes are often a sign of infection or (sometimes) a cancer.

He knows but he never directly acknowledges the ASMR. The videos are primarily for the medical preparation for students but in his heart he knows he’s reaching out to us ASMR lot!

This is one of the most relaxing videos I've seen from Dr. Gill -- also extra educational -- I've learned about lymph nodes I never knew I had. Do all the nodes serve the same purpose, or are some more specialized?

Great video man, that was relaxing, though this makes be a bit anxious because lots of my lymph nodes are making lumps, we've checked a lot of them wirh my doctor a few times ago but I have an appointment scheduled cause I feel like more and more are appearing .. Hope this goes well and it's as relaxing as this one lol

Thank you, as too often a doctor will be pressing all over and I'm not always sure what they are looking at or for....plus the dental assistant does this sometimes. Finally I asked one, what was she looking for and she explained about lymph nodes. I thought that was pretty neat, that they do more than just clean teeth.

As I’m sure you are aware people find these relaxing to watch for non medical or educational reasons. The term ASMR is often used. I had thought (until these videos became ubiquitous) that I was the only one that enjoyed medical and eye exams, haircuts etc. What do you think is the reason so many people enjoy these? Is the human race not getting enough physical interactions where we are touched?

I get a soothing asmr as I take a quick break between classes, and now my abdominal pain is greatly reduced and I feel calmer, but also I'm observing some clinical skills, oho I'm getting two birds with one video.

@@DrJamesGill I’m in nursing school and we were doing the head and neck and cardiac assessments clinical skills last week, which included lots of videos you already made. It helps me a lot to see how it’s done visually rather than reading off a paper and glancing back at the professor intermittently. Your flow is very nice.

Dr Gill would it be possible for you to make a video on Degenerative Disc Disease for the t11 and t12 I was diagnosed with it last year along with Kyphosis, scoliosis and DDD in my left hip and pelvis I'm only 22 it all started at 18 and I still don't understand it

Sometimes when I have a migrane there are exactly two points on the back of my lower skull. And it eases the pain when I or somebody else massages these two points. Now I see that these two points are in the exact same spot as two of the lymph nodes you have shown. Is there a medical explanation that lymph nodes can be a trigger or symptom for a migrane?

It’s more likely that you are pressing on the nuchal line on the back of skull, or the external occipital protuberance This is where a number a muscles insert, and it may be than pressing over that area improves a tension in the muscle that was trigger the headache But certainly chat your doctor. A physio might be able to assist

question, what may be the reason for the increase in all groups of lymph nodes (submandibular, cervical, occipital, axillary, inguinal, supraclavicular). they have been enlarged for a very long time, about 3-4 years, they are very hard and painless. I am 18 y.o, Iive in small city in Ukraine and dont have the opportunity to get to the right doctor, I will be very grateful if I get an answer. thanks 4 videos

Swollen lymph nodes are not hot, red, or possibly fluctuatant as you may see with an abscess.

Uhhh I have a lump that’s been on the side of my neck for months, if not years. It doesn’t hurt or anything though, I just figured it was a knot that hasn’t gone away (my body is always very tense). Is it something I should worry about?

They are important, but tend to be focused on for their respective examinations - such as an abdo, or genital examination To reiterate, I always check them when someone has abdo issues

Just one talking about the regions on a plastic body, and this HOWEVER, lymph nodes checks are a requirement for most other clinical examinations. It’s good that you are seeing how important it is, and how often it comes up in different scenarios 😊

I have a slightly raised outgrowth of skin at the heel bone part of my right foot. It is not distinctively noticeable but visible if looked from 4-5 feet of distance. Is this simply a skin tag or something more serious? It has been here since 1-2 years now, has no pigmentation and hasn't spread.

Shirt removal: 3.6/10 The guy from the shoulder exam really needs to coach these guys up. Just bad form, yet again.

Agreed. Previous contestants performed a fairly better shirt removal. But, in honesty, we didn't see the full athletic gesture in its entirety, so this could be an editing issue. I hope he will perform better in the 2nd trial.
