I read it was the FA that led the PAX through the rear baggage door. Good on her getting the job done.

CRJ pilot here, our thrust levers are the same as the Challengers, there have been previous incidents of the idle cutoff locks being accidently lifted allowing the thrust levers to be retarded to cutoff unintentionally. Captain reaching around for flap lever is one scenario where it could happen.

Thanks for confirming this. I had read this elsewhere and it sounds like a very plausible explanation as to how things could suddenly go so wrong. Given where this jet was on a visual approach, if the FO was flying, the Captain could well have been deploying flaps. I'm not a fan of having the thrust levers also being the cutoff levers. Reminds me a lot of the Thunderbirds F-16 accident at Colorado Springs a few years back in which something similar allegedly happened to TB6.

I have 9,000+ hours in the CRJ. Never once did I accidentally cut off the TL in the plane or even in the sim. Never heard of it happening... not once. As you know, the TL lockouts are independent; are you telling me they accidentally snagged BOTH of them and at the exact time he was moving the TL to idle? I don't think so.

Juan Brown suspects too long at altitude without the 'antifreeze' in the fuel and it was gelled in the lower fuel tank. Fuel starved.

​@mrsaskriders incorrect, the airplane has a fuel oil heat exchanger and doesn't require anti-icing additives in the fuel. I'm willing to bet that the engine shutoff tabs on the back of the trust levers were bumped and allowed the levers to go all the way to shut off when the pilot flying actually intended to go to idle. Bad design and this sort of scenario has happened on several CRJs (same setup) in the past, but all made it to the runway or were high enough to delight the engines.

I was flying a medevac mission in Iraq in 2003 when another medevac Blackhawk crashed upside down in the Tigris river in Samarra. While I didn't see the crash happen I overflew it only about 5 minutes afterwards and could see the red cross on the bottom of the aircraft sticking up out of the water. For the rest of our mission that event was definitely on our minds. We knew it was not a survivable event. Even though we had no choice but to continue flying I can definitely tell you the crew was not focused 100% on the task at hand. We had been and continued to listen to all the radio traffic, which was fairly intense as the ground forces and wingman reacted to the crash. I realize that's not the same as watching the actual event, but certain things stay with you and unconsciously they have an impact on your ability to fly and be as effective as you probably should be.

I drove past the Challenger crash site today. And yesterday while on approach into APF to the same runway, 23, we had a flap fail. Executed a missed approach, ran checklists, and checked landing distances to confirm we could still land at APF then proceeded to land uneventually. It struck me how we did everything right and took our time to make sound and safe decisions. The challenger crew, from all appearances so far did not have the luxury of time. Setting aside whatever events transpired to put them in that situation their final actions were of the highest caliber. They did what they had to do to bring their passengers to the ground alive.

3 crew, 2 pilots 1 F/A with 2 pax. You have 3 egress points in the challenger Main cabin door, Emergency exit over the wing and the aft baggage door. It’s looks like they egressed out of the aft baggage door. Yes. APF has a lot of corporate traffic. Actually I’m heading there tomorrow in the Falcon. HUGE Kudos to the Flight Attendant! HUGE credit also to the pilots!! They did an amazing job putting that challenger down with the cards they were dealt! Being that low, configured, you’re committed fly it to the ground in a fight for life. Super critical airfoil, didn’t stall it, kept it up right! So sad they tagged the acoustical wall. Praying for their families!!

I disagree. The two deceased pilots are lucky no one driving on the interstate were killed because they wanted to land there. How could you explain saving yourself and passengers but killing some poor family in a car minding their own business.

@HAWKSTA24 As opposed to what? They had no power. That is a dense residential/commercial area. They were too low and too slow for any options when they lost power. It was a shit sandwich, and they had to take a bite. They didn't have the luxury of sitting in their comfy homes coming up with fantasy outcomes. They did the best they could with what they had to work with in the moment. Show some respect, or at the least keep your mouth shut.

@@HAWKSTA24 Unfortunately this is a rotten opinion. The chance of injuring or killing someone in a vehicle during a highway landing is high. The chance of killing the entire plane by not highway landing is certain. Fly the plane all the way to the crash sight, that's what they did.

​@@HAWKSTA24 We don't know yet, but it's very possible that a contributing factor to crashing was the pilots avoiding the traffic. Until you've been there and walked away, statements like yours are unnecessary.

Naples is one of the busiest GA airport, particularly for bizjets in Florida and it is the Wild West. Around thanksgiving I had a near miss with a helicopter that cross my runway right as I was rotating and I flew about 100 feet over them. I was surprised they released FlexJet right in the thick of an emergency in progress.

The Fire Dept would likely have command authority at the crash site. The Hwy Patrol would be responsible for the freeway, closing it to traffic, trying to determine alternate routes. Shadow 5 would be important in providing information about the crash scene itself, directing fire assets into the crash site, etc. (9 yrs LAPD Air Support Division Sgt, 1992-2001).

Greg Feith (a legendary NTSB investigator) mentioned on his podcast 'Flight Safety Detectives' that when he was investigating crashes that documentation from the fire crew was so critical in the investigation. They're on the scene quickly and seeing/documenting everything...by the time the NTSB shows up the aircraft could be completely reduced to ashes. Props to the fire crew who had to witness the aftermath of this horrible event.

Those aviators went out heros and the quick thinking lead to the rescue of the crew. They will be forever remembered for that. There will be some PTSD and healing needed for the survivors. They will have one hell of story to tell their grandkids. My Grandfather is retired AF and SAC-NAV, served at IWO then went on to do ATC for 40+ yrs. As a kid I would bug the hell out of him to tell me some stories. No matter how much I asked or how I asked, he would never tell me or the Family any of the stuff he saw in service or as an ATC. I wanted to follow in his footsteps and go into the AF, but to be a fighter pilot. Sadly I could not join because I have mental deficiencies and am Autistic lvl 1. It was soul crushing and I still have pain from not being able to make him proud by going into the academy. Thank you Mover and Gonky for your service and bringing these and many other stories from your time and experiences in service. Godspeed -

You can kinda hear in the voices of the Cessna crew and the tower operator that cleared FlexJet----they know what happened. Points to them though---they kept their cool and stayed in the game long enough to facilitate clearing the area.

he hit the back of a pickup and that is what put him into the wall - the job he did was amazing - there is one vid out there from a semi that was behind him - I heard somewhere that his destination was FLL which is two miles from my house, Naples is the local exec airport where the small jets use to stay out of Ft Myers - I was in there a couple times about 15 years ago when I was hamburger hopping with two pilots - I have around 5 hours above MSL and 1800 below - I was a helmsman on two nuc subs -

As you likely saw, they came in at an angle to the hwy, the right wing hit the wall and that swung the cockpit right into the wall, likely killed them. See the double impact marks on the wall. Since the aircraft slide backwards, it allowed the passengers to avoid the initial fire and get out.

I live about an hour north of there, and we have tons of birds, especially lately. I'm wondering if it was a bird strike. Hitting a flock of white pelicans would be a nightmare.

There's speculation that the jet fuel froze and they flamed out because of fuel starvation. When they bought fuel at the previous stop, communication audio was captured of the crew ordering jet-A with no prist. We'll see what the investigators find.

@@PistolPackingPilot Interesting. My brother flies 747 out of Miami, so he might see more of this kind of thing. Where I'm at in Utah - I rarely see large flocks of birds. Maybe our altitude density isn't ideal for them... :)

They managed to turn back to the left and mostly align with the right edge of the highway, but the right wingtip touched the wall and turned the nose into the wall. A trucker's dashcam recorded the plane as it turned and descended onto the highway.

Naples is a municipal airport and Shadow 5 is a LCSO air unit who was on routine patrol when this all went down.

Go/No Go after witnessing such a sad event is a really tough call. Can't second guess that decision either way. Thanks for not speculating on the cause, way too many people jump to conclusions.

I did 20 yrs with NetJets. In&out of APF dozens of times. Given that they didn’t “witness” the mishap, I likely would have departed if my F/O was okay with that so as to complete the mission with pax on board. If I was just ferrying, I might call it quits for the day.

I fly out of KAPF. There is really not a lot of part 121 traffic. It's mostly part 135 and private jets serving the very affluent Naples area. There are also a couple of flight schools. I saw the video of the attempted landing. Looks like his left wing struck the highway first and pushed him to the right towards the wall, which is where his momentum was taking him anyway. I think if this had been in the off-season when there is a lot less traffic on I75, they very well could have had a successful landing on the highway. But trying to place the aircraft in between cars and avoid making their emergency someone else's emergency too, they didn't have a lot of room to maneuver. Made it very tricky, and they can be considered heroes for saving their passengers.

If you watch the video where the plane actually touches down, it looks like an excellent side slip to land on the road... however it catches a sign and maybe a truck that swings it into the retaining wall

Biz jet pilot here, currently on the 737 BBJ but have about 200hrs in the CL60 type and luckily you can indeed access the rear cargo and open the baggage door from the inside. Not here to Monday morning quarter back or second guess their decisions (Hop-a-jet is a respected operator and those guys were by all accounts great pilots), but I wonder why they didn’t elect to put that thing down in the grassy median instead. What are your thoughts on that?

@@jchang3327 they are there, with the median, you would likely have a wingtip in traffic and then once hit get spun into even more traffic. I would say they made the right call. In the instance I was in, if we had chosen the median, I would not be typing this..

Do you think it would help to put emergency landing areas on the side of some roads? Similar to the idea of emergency brake points for runaway trucks in the mountains. I know it could be impractical but it would be nice to have in an emergency. No cars or houses in the way.

you commened on cactus 1549 as the only other dual engine flameout, did you consider TACA flight 110 which, as I understand it, was in a lot of respects similar to th Cactus 1549 flight?

You can access the rear cargo compartment through the lav. Not sure if you can open the cargo door from inside but there is definitely a way to get out from the back. Ie emergency exit.

Thanks again for all of the cool videos, and saying what you think. Wondering if they were trying to avoid the road and land on the grass. The median always has concrete drains/holes/etc...and at 140kts. they would have wiped out a bunch of cars. Sad that such pros are lost to us...hope their families are ok and can get through this somehow.

Fuel icing. My best guess so far. There have been a few dual engine failures in business jets and that 777 in England that landed short.

This is probably true according to the recording their last fuel stop they ordered the fuel without the additive to prevent jelling as reported oonthe Blancolario channel.

Challenger ! 😢 I thought this was a dual engine failure on takeoff and they were trying to circle back to the field… couldn’t make it and tried to land on l-75. Also that a wing tip caught the sound barrier wall and spun the plane around, hitting the same barrier, nose first… did I miss something?

it appears that landing was going well until the wing hits the white vehicle ,the plane kind of swerves to the right then appears the right wing contacts the wall and causes the 180 and contact with the wall ?

Yes, one of the 3 survivors was the 23 y/o FA who definitely saved the 2 pax. No way they would’ve known the rear baggage could be opened from inside.

@@jchang3327 Even if they were briefed by the F/A before takeoff, that there were 3 exits available ... most F/As instinctually act - most Pax instinctively panic. Had the F/A been rendered unconscious, they likely would have all perished. ^v^

God rest the crew. Praying for the families. I trained in Naples over 10 years ago. Very sad. So far I’ve not heard as to why they say they were coming back. Had they departed and then found an issue they were trouble shooting. We will find out with the report. Prayers to the families.

Could it be that they didn't have the anti-freeze gel in the jet-fuel as some have suggested? And another question for you, since you worked Naples, as Lemoine briefly mentioned or was curious , is Naples a place where there are a lot of rental biz jets?

Saw the dash cam video from the semi on the highway. The pilots almost pulled off a successful emergency landing. Amazing bit of flying. They did a fantastic job getting the passengers down safely. God bless them and their families.

My feelings on that is that most drivers have very little, if any, situational awareness. I don't think they would have noticed him in time to get out of the way.

This unfortunate crash reminded me of Sully's flameouts from the bird strikes. I'll never forget him stating: "we're not gonna make it, we're going into the Hudson".

Non pilot here (30 minutes in a Cessna), but if I were piloting an airplane waiting to take off, after an airplane that took off from that airport crashed nearby, I’m going to taxi back, shut down, and triple-check my fuel load, just on the off-chance there was fuel contamination or ground crew that mixed them up etc.

Aviate, navigate, communicate. I thought in an emergency it’s actually Maintain aircraft control, analyze the situation, take appropriate actions. Communication may not be in the top3 especially when you are already cleared to land. Seems awful quick to perform those 3 actions between the time “cleared to land” and “we have dual engine failure” (unless a lot of time editing in the audio). I’m sure the Challenger has a few steps to perform in the dual engine checklist.

It is still aviate, navigate, communicate. When you say "Maintain aircraft control, analyze the situation, take appropriate actions", that comes under aviate.

Another YT channel claims they ordered fuel without the additive keep it from icing. When they throttled back to descend the heat of the engines was keeping it flying before because it was under higher power setting. Once they throttled back to descend and land heat was no longer there the fuel freezing cold gelled up. It's speculation I suppose it's plausible how often do have duel jet engine failure unless run out of fuel.

False. The CL604 does not require fuel additive and it makes plenty of heat via oil to cool the fuel, even at idle.

They had a few dual engine failures on Beechjets. Beechjets don't have a fuel-oil heat exchanger though. Challengers do have one, so they don't require the anti-ice additive. In those Beechjet cases, it happened at altitude when the aircraft was still cold soaked. I'd be very, very surprised if that was the cause of this crash. I'm leaning in the direction of the accidental shutdown via the power levers scenario.

@@JonnyJetPilot That was also mentioned on that other channel accidental shut down. I'm sure the NTSB will quickly figure it out if either of those things happened. I'm assuming it had some sort of flight recorder hopefully.

Lots of business jets, people going to Naples, Marco Island, etc. Sure glad I wasnt on that stretch of 75 that day. Plane I think had flown down from Ohio. Pilot was cool as a cucumber. He saved lives that day. Bless their souls.

They didn’t take Prist in their refuel. Maybe fuel tuned to jello at high and cold altitude. They saved their passengers. Well done gentlemen.

I came up on the crash minutes after it happened. I couldn't believe anyone walked away from it. After seeing the truckers dash cam I realize these pilots did everything they could without endangering lives on the ground. I saw some things that will live rent free in my head for the rest of my life. God rest their souls.

2 pax and a Cabin Server onboard. The Cabin Server was emergency trained and got the pax out. Typically Cabin Servers are not trained in this like Flight Attendants are. So we are lucky she knew what to do.

I 75 was bumper to bumper on Friday like always, they would have crashed into loads of cars. Naples airport was a stone’s throw away! I believe the two (great and courageous) pilots were trying to make it OVER the wall onto Wyndemere Golfcourse on the other side, visible to both pilots, who had to make this decision in seconds. What makes both engines go out ???

Numerous Bombardier pilots have chimed in on this and other KZbin channels indicating that there are scenarios in which pilots could inadvertently shut the engines down on these airplanes, because of the design of the thrust levers, which are also used to cut off the fuel at shutdown. Based on their accounts, that is one potential explanation as to how things could go so horribly wrong for this crew so quickly. I have seen another video of the landing, and it appeared as if they were going to successfully put it on the highway, until the right wing grabbed that acoustic barrier and turned the airplane's nose into the barrier......just like some of the accidents you see in NASCAR. Like you say, these dudes were cool as ice to the very end. Hey, they saved all of their passengers and in that respect, they did well.

They did not hit the noise wall head on, the left wing clipped the wall and rotated the cockpit counterclockwise into the wall.

what's wrong with speculation ? everyone does it, chances are they ran it out of gas right ? all we have talked about here, is how cool we all need to sound after you run a plane out of gas, and how cool everyone else here sounded on the radio, Oh and he was NOT configured, at most a notch of flaps, grainy but no flaps, because the engines run the hydraulics that deploy the flaps.

It’s the same as MAY DAY, MAY DAY, MAY DAY. The plane was already aligned with the runway to make an Emergency Landing. But the controller instructed the plane go around the pattern and land in the other direction. I’m very familiar with the Naples Airport as I live nearby. Are you a pilot? I am. Bad directions!

It's not the same in the ICAO world. That's the point. We've covered many mishaps where people criticized not using "Mayday mayday mayday." Do I think declaring an emergency gets the same point across? Yes. But that wasn't the point. Did you watch the same video? The controller didn't instruct them to do anything once they said they had lost both engines. What do you fly?

It's not a Cherokee. The scavenge ejectors would keep the collector tanks full unless the mains were bone dry. Unless there were a previous large imbalance (over 200lbs) from something like extended single engine operation, the auto crossfeed would have automatically balanced fuel at a 200lb difference. You don't normally ever "switch" tanks in a Challenger.

They have nothing to speculate with. Double flame outs are not uncommon for more reasons that zero fuel.
