My Dad got a similar type of injury when he fell and broke his leg. Everyone freaked everyone out that his leg got shorter and was so crooked. I learn a lot from your videos, keep up the great work 👍

Ooooph, that's gonna leave a mark. I sure hope they got everyone to the ED real quick and in a hurry. Reminds me, I was first on a single vehicle a couple weeks ago on I-20 to find a compound mid -humerus and I played back in head what little training I have, but in your voice. Had a good laugh about that later.

It’s my belief that every department should have a dedicated off-road vehicle for extractions from hard-to-access areas. Some places aren’t good options for heli (dense woods, for example).

Nobody ever talks about triage. Was on an MCI years ago before the cavalry arrived. There was a DOA, an entrapment, and 3 other various injuries. I made some hard decisions that day. I think about them time to time, but i stand by them. Follow your training and integrity.

i have saw A lot of bad things happen over the years riding quads all over the midwest!! for the most part people freak the F**** OUT or the just freeze & stare!!! i have litterally had to shake a by stander to get them to be able to call for help!!! I saw a girl in a buggy similar to this video not make it to the helo ride, (didnt know her) I watched the buggy flip down the hill to the river bottom, the driver was shaken up but walked away. the girl was impaled by a tree limb that came in the cage!! thanks for sharing this great info with everyone!!

That was such a short clip. Any idea what happened to these people? I hope they all survived. Thanks for the valuable info. If I encountered a scene like this, I’m sure my instinct would be to focus on the guy with broken legs first, but you’re so right-often the most seriously injured person is not the one with the most horrific-looking injury or the one who’s screaming. This is why training is so important.

A limb with internal bleeding with become swollen, but any injury will become swollen so it's hard to determine exactly how much bleeding is going on in there.

I wouldnt apply tourniquets, I'd prioritise straightening the legs and putting traction on. Will reduce pain and reduce bleeding too.

Same here ... it was like the first thing I saw ... I was like, how the hell are we going to get him out? I didn't see any roll bar or cage near him ... and that is a big plus. The girl in the back might be a bigger problem ... there is quite a bit of tubing back there. I wish there was more video on this one.

After he's stabilized in place, he's getting covered with a tarp/moving blanket and then you bring in the spreader jaws on the Jaws of Life and a hydraulic ram. The dash needs to be moved up at least 8-16 inches to completely clear his legs. Cut the "B" pillar to his right, recline/cut the seat backing, slide a backboard under him, secure and slide him out. Similar for the girl on the back. Instead of taking her out sideways, clear enough of the wreck from behind her (back window, seats, possible truck aread), slide a backboard under her and pull her straight back. This is referred to as "tunneling" into a vehicle. The only good thing is that the vehicle is a lose the second the wreck happens which allows you to destroy whatever needs to go in order to properly extricate someone in any direction which maintaining the best possible spinal/neck positioning while moving them. Ideally, make it into a door less convertible and remove anyone that cannot move under their own power

Crush injuries are so dangerous...Could kill him easily especially when you move him out of the environment quickly. Can cause muscle cells to release deadly toxins into the bloodstream which can kill on the way to the hospital

I've had a broken femur. He needs to get out of that thing before the pain sets in. Right now there's just a strong burning for a few min. I have never in my life felt pain like that. I got caught between the load on a forklift and the tailgate of a pickup. I still have knee trouble from that. Luckily this is America and we have great healthcare. Ambulance was there in 10 minutes and had drugs in me to ease the pain. And within 30 min f from it happening I was at the hospital. I could only imagine being in a desert.... that was a Thursday evening and they had me up and putting a little weight on it with crutches By Monday and out of the hospital. I can only imagine what this guy is going to be feeling

Maybe. It looks like the dashboard and floorpan intruded/collapsed and it's possible his feet are pinned down low. The floorpan and dash intrusion is likely why he has that injury. Or he wasn't wearing his seatbelt harness, so in the crash he slid forward out of the seat and became thousands and thousands of pounds "in weight" and slid forward pressing against the floor with his feet and putting all that pseudo bodyweight on his leg(s) which unsurprisingly shattered immediately. Even if he was wearing the seatbelt, it's totally possible that the floorpan getting shoved up violently in the crash shattered his leg(s) in the same manner especially if they were resting flat on the floor fully extended. :(

@@abccde3832 If you're just a bystander running up on him? Make sure the vehicle isn't at risk of bursting into flames. Keep his head and spine still and stop him or others from moving them around. Keep the leg stable. Make sure he can breathe and can protect his own airway. If there are bleeding wounds, pack or tourniquet them. If there are sucking chest wounds, seal them. Otherwise, leave him there until EMS arrives with C collars, a back board, limb braces, and the manpower to extract him from the vehicle steadily and with as little movement of that spine and leg as possible. If the vehicle is leaking gasoline or burning already, you gotta do what you gotta do to get him out of there with as little harm to his spine and broken bones as possible. What good does an intact thoracic spinal cord do if he burns alive in the fire before EMS gets there to extract him properly, you know?

@@abccde3832 You're welcome! Another good thing bystanders can do is examine the person for traumatic amputations. If this guy was missing his left hand for example, search for it and when you find it just wrap it in a clean cloth to protect it and put it on ice in a cooler if you have that available to use. Cooling the hand would increase viability when it's time to reattach it at the hospital. If no ice or cooler are available, just wrap it up and keep it clean, safe from further blows or burning or chemicals, and out of the heat of direct sun. Make sure the EMS team takes the body part with the patient and knows what's been located if some stuff is still missing.
