The main differences to real life are generally limited to personalities being different, some actions being attributed to the wrong person, some supernatural stuff thrown in, etc. But you can learn the history of the Three Kingdoms period from a birds eye view from the novel with few issues.

Fact: As someone who have read 3 Kingdoms, I can say it's impossible to remember all the characters' names because there are over a thousand characters in the novel, even for the Chinese people where it originated from. The only good way to remember some of them is to acknowledge who they serves according to their colour (red, green, blue). Even Sun Shangxiang or Lady Sun, whose sole purpose in the whole story was just having a forced marriage to form an alliance between Shu (green) and Wu (Red), was disneyfied and given a hero role in the game.
