They touch on fist swimming briefly. One drill I've done is one length fist, one length normal, one length with paddles, and repeat. The fist length after paddles really makes you feel your forearm having to pull.

On the breathing... Triathlon Taren describes it as a full exhale followed by a "sip" of air inhale. Not taking a big gasping inhale has actually helped.

I breathe on the left but when I'm fatigued my right arm wobbles when stroking.
