Incredibly helpful video! Had a pt not restarted on this med post surgery and hospital discharge ( had been hypotensive with a mild hyperkalemia from AKI). This helps me to understand the mechanism of this med. Thank you!

Careful with valsartan (Diovan). Hopefully you have bi-annual bloodwork. I had heart failure in 2010. I did/do not have high blood pressure. However my cardiologist prescribed valsartan, he said it will aid the heart in recovery. The day I picked this up, the pharmacist told me this is a wonderful drug but in 10 or so years your kidneys will begin to fail. Forward to 2024, I am in stage 3a kidney failure and recently have a reduced heart ef. I am stopping the valsartan on my own to give my kidneys a break. I check my bp daily, and again have never had high bp. I get quarterly bloodwork. I have had covid twice, am a type 2 diabetic that came about after initially starting the heart failure meds. @@knuftobor I am sorry just saw this. I got a ultra sound which claimed my EF was at 45. Cardiologist insisted I try to figure a way to pay for Entrestco and get off Valsartan. I did that. 4 weeks after my Ultrasound he had me do a nuclear PET scan. That said I as at 68 EF! I feel good. Baby aspirin makes my skin get blotchy and least little nick I bleed like crazy. I feel as good as I can at almost 74. Wish you the best. Last my cardiologist would not bad mouth Valsartan but did say Entresco was the preferred treatment. I take it twice a day.

I have been on ENTRESTO since August 2019 and this is the only useful area that I have seen. It has increased my EF fron 35% to 50%. I wished it did not cost so much. Thanks for the idea of cutting the pill in half. I will ask my doctor to write the prescription to double so I can cut in half. Very helpful.

Novartis did offer a one time free 1 month supply to any patients having Insurance trouble when getting an RX filled & a Pharmacist should know about the procedure. My question is: Can anything be done to treat the failure & improve the Ejection Fraction Rate without the use of Pharma Drugs ?

Entresto is produced by Novartis in Malaysia, it costs approx. $500 per month. If you can buy prescriptions overseas, check out Brazil. The cost of the same dosage of Entresto, also made in Malaysia, is about $12 per month. I forgot to mention this, the medication from Brazil is packaged in blisters in Italy, making it more expensive than the one in USA that is sold unpackaged. I am sure it is sold in Mexico also for a fraction of the USA price, not sure about dosage availability. Be safe!

Taken into emergency due to breathing problems. Was given too much oxygen that almost killed me, scan and MRI done diagnosed with Heart Failure, had to discharge myself out from the hospital as they kept increasing my dose of Furosemide due to pulmonary odema. It made me so ill I could not stand. My kidney pain was unbearable. Came home in an uber! One of the last meds they gave was Entresto it seemed to help slow my heart rate and was able to walk enough to get out! Was confused if I was still supposed to have bisoprolol and this video has clarified it. No beta blocker. Trouble is if you check yourself out, you get treated like shit and don't get meds etc so am on my own now, looking for private care as opposed to NHS

Diagnosed in the UK in late November 2023 with heart failure and reduced EF between 25% and 35%. Main symptom was breathlessness, but angina-like pain was what prompted me to call an ambulance. Spent five days on acute cardiac ward, am now taking a large cocktail of medications, including Entresto 49/51. I feel considerably better. Minimal breathlessness, if any. I can walk up and down the high street in my local town with no problems, visit the shops, and do some shopping. I am male, 77 years old and had a 3X CABG IN 2013.

It’s 2024 now and I just started on this medication. Is there any update since four years have passed in terms of as people take it longer? has the price gone down? I live in Portugal as a legal resident so my cost are really minimal including the fact that I am going to get a “upgrade “on my pacemaker to include a defibrillator that’s paid 100% by the country. Not so bad.

Thanks for the video! What were the death rates in the original study for patients on placebo (not just the ACE inhibitor), or did they not do that, and if so, why not? Also, you didn't include examination of the ejection fraction as part of your follow up. What sort of increase in ejection fraction, if any, do you see? Will you keep a patient on the medication if they do NOT have an increase in ejection fraction? Thanks very much for your help!

Hi Jeffrey, I will try to answer your questions: For the big Entresto study there was no placebo group, because medicines like enalapril had already been proven to be superior to placebo for this type of patient. I think what you are getting at is what is the likelihood of benefit of entresto over nothing. To get to that number, we have to use the old studies of enalapril (or similar medication) vs placebo which date back 20+ years. The average mortality benefit of enalapril or similar over nothing was 1.3% per 1 year. The entresto study had 2.8% mortality benefit over 2 years. So you are looking at about 2.7% decrease in mortality per year with entresto over nothing. There are a bunch of assumptions baked into that, but there will never be a study of entresto vs placebo. Really we can't multiply that 2.7% per year of treatment, that simply isn't known. So for example if someone takes entresto for 10 years, i don't think we can assume the mortality benefit is 27% absolute reduction. As far as EF goes, that was studied in PROVE HF trial, where Entresto was associated with increase in EF of about 9% in absolute terms (compared to Ace inhibitor or ARB) over one year of treatment. I consider that study kind of a garbage trial though, it was not blinded, all kinds of risk for bias, and other issues. In my experience, EF does not improve much with entresto. I think it works in a different way. It causes urination of fluid in a way that is less damaging to kidneys than usual diuretic therapy. But that is just a guess. Many patients do not have increase in EF with entresto, however, they still seem to benefit by feeling better, and staying out of hospital. So I keep patients on entresto, typically regardless of what happens with their EF. I have a love-hate relationship with the medication. It helps some, but it is no cure. Some patients don’t seem any better on it compared to the old standby lisinopril. And it is so expensive, it is hard on the patients when they are paying big bucks and there is no guarantee that it will work. There are definitely some patients who do great with it and have an impressive response. Thanks for your questions, and best wishes!

why are doctors so sure about this stuff when they've been fined hudreds of millions in the past for illegally marketing to doctors, prominent researchers and doctors say the trial was rigged and....recent clinical trials where they compare it to generic valsartan show it's no better, maybe even worse. what makes them so sure?

Doctor I can't thank you enough . My wife is on Entresto 24/26. By asking her cardiologist to change to 49-51 and cutting tablet in half saved us 1/2 the cost on our Part D plan copay AND prevents her from entering the donut hole of Medicare Part D. God Bless you!!!

a combo agent at max dose vs. single ACE (enalapril) at half its max dose + plus run in period to weed out anyone who could not tolerate max dose of combo agent... Dr. Vinay Prasad does a good job of scrutinizing the data.....

25% to 52%. I am taking Entresto & Carvedelol. The EF was 9 months after starting Entresto. I was told my Kidneys were functioning properly however, I have been having sharp pains which I think is my kidneys and I have itching on my hands a swelling ankles. I have low blood pressure. No other noticeable symptoms but I am concerned about Kidney failure.

Took entresto. I had an EF of 40% Within 6 months, i went to 50% In a year, it was at 60% I was mostly trying to eat keto/paleo and intermittent fasting(8 hour window) to boost autophagy (self-healing). I also regularly get chiropractic adjustmentandnd take coq10 and magnesium supplements. I'm 35 years old and not overweight.I declined an ICD. As you can see above, I used holistic health methods alongside the medication.

Thanks for this reply, it has given me some hope. I too, have a RF of 40% and had no other symptoms other than a heart murmur discovered by my PCP several months ago. I'm scheduled for an angiogram next week so I'll know more then.

I have an EF of 25%. Was diagnosed around 2 years ago after difficulty breathing. My cardiologist had me on Carvedelol, Spironolactone, Lasix, and Lisinipril. I took them for 1 year and they did nothing. I got off all of them because I started to develop diabetes for some strange reason. I fasted for 3 days straight only drinking water...which got my diabetes glucose under 100 by just fasting. This was AGAINST my cardiologist's recommendation. Started taking Cq10, Magnesium, Berberine and Hawthorne for my heart. Took Cinnimon, garlic, Maqui Berry for my blood sugar. I also refused an ICD. Refused Metformin for my diabetes. Never had a heart attack since. Blood sugar at 150, so it's a little high, but not nearly as bad. Never felt better. That said, my EF is still at 25%. So...I wonder how long I have to live. If you're at 40% and have no symtoms, I think you have many years left. But that's just my opinion and I am not a doctor.

I forgot to add that I intermittant fast every week for 40 hours. I believe this helps, but my cardiologist says he doesn't think it will help me at all. I dunno, but I will take my chances.

@@BillyO8828 it’s obvious from your post you don’t understand heart failure and what the drugs do - you’d be advised to ask your cardiologist how beta blockers and diuretics work to help your heart in cases of low ef before discarding the therapy. Secondly your t2d is out of control if your bs’s are consistently 150 - you need to get that number down asap with diet and more exercise - how’s your A1c? On the positive fasting will help, a largely plant based diet will help and taking the right drugs for your heart failure will help your long term health and survival - you may feel great not taking the drugs but your life expectancy will be limited as your thyroid pumps stressor hormones into your system. check out what dr vinay prasad yt video on paradigm trial. parallax and life trials show it's no better than valsartan, maybe worse. no fart is has been fined hundreds of millions in the past over valsartan. there are 9 generic arb available. how can any doctor be so sure about it??

he's a salesman for novartis, entresto is a scam, they just invalidated the patent, it doesn't do anything the generic valsartan can do and it's actually worse than valsartan in some regards, the evidence is right there in the LIFE clinical trial

It could be cut in half to make it easy to swallow. Manufacturer do not guarantee even dose distribution in both halfs. How much dose change and could it affect your health, i leave it to your decision.

Hi i had heart attack last month after stent my FR is 35 my dr put me on entresto 50 one aday to see my reaction to it along with Brilinta 90 and acetylsLicylic acide 100 Telling me this might increase the FE i hope so 2 question pls I forgot that i have to wait 36 hours I took it after 28 hours is this goning to make damage to my The second question my daily life routine i have work not very hard but carrying loads something like 15 to 20 kg and walk very fast But i feel good I don't feel tired or something. Should i stop that

Hi my girlfriend is 22 years old and she had a heart failure last year. Her EF was at 10%. I believe it was caused by untreated PCOS. She is currently using a paraguard copper IUD. My question is, do you think that the progestins in hormonal iuds (like mirena) will have a negative interaction with Entresto?

Great information. I'm currently suffering from the heart failure you described as a result of a bad case of covid delta pneumonia. There is NO WAY I can afford this drug so I'll have to see what the company can do and talk to my cardiologist. I sure didn't like the fact that the drug study was sponsored by the maker and that there is a 17% risk of dying!! Scary things to think about. I still think of myself as being young but I'm 63 so I guess I'm middle-aged! Thanks for sharing the information.

I dont think tablet can be cut, its a complex and not simply a combination, if u cut it then equal amount cant be achieved leading to destroy the cyrstalline structure

Been taking this for almost a year and I feel great about it. I tried stopping it occasionally but conditions gets back so i think i need to accept the fact that Entresto will now be part of my budget forever. I had this cardiomyopathy after i had covid

I did the same as well I stopped using it to see what would happen I was ok for about a week. Then I felt I had to go back on it. I don't like the sides and found I tolerate it better with food.

Hi Dr. I was diagnosed with a weak heart due to my left ventricle was not pumping properly. I was hospitalized and at the hospital I was in code blue...I was gone from this earth. They shocked me and CPR then I was resucitated. Had an angiogram done and nothing was found in clogged arteries whatsoever. Due to arythmia in my heart an ICD was implanted. I got shocked once already not a nice feeling. I felt like someone was beating my heart with a bat....ouch... The recommendation due to the arythmia was that for a heart transplant. I am taking Entresto 49-51, Spironolactone 25 mg, Carvedilol 12.5 mg, Furosemide (Lasix) 40 mg,... I need your advice and guidance in this situation. I dont want a heart transplant, but doctors said that if I keep getting shock from the ICD what kind of living would that be?? Honestly the unknown terrifies my thoughts....please advice. I was using it for last 2 month it gives me diaherria and dizziness, cannot continue daily tasks when I take it. I was taking 50mg then asked the doctor switch for something else I wish I could take it. If anyone had similar effects like to here how to cope with it.

the answer to that can be easily found by going to klinical trials doooot gov and searching for it's reg# LCZ696, then search for the two trials where it's compared to valsaarrtan. in short......generic valsartaaan is actually better than pateneted entresstoo. (cmnts r delted by AI, thus mispelt)

@@itsjustvalsartan7844 I was taking Lisinopril (10mg once a day) for years before being switched to Entresto. Daily I wonder if I felt better with the Lisinopril or now with the Entresto. ((or maybe I should "switch to just Valsartan and try that")). I currently have a supply of all 3 of these drugs and I know about the 36 hour wash-out period if switching. **But I don't know equivalence of how much Valsartan to take to match the current lowest dose of 24/26 that I'm taking now with Entresto** Does anyone know what dose of Valsartan would be appropriate to start for someone currently taking Entresto 24/26?

@@joshpembers do you live in a country where they give such meds without a prescription? You should have a doctor set your meds, not good to try and do it yourself. My only goal is to inform people about Big Pharma. But....to answer your question, the amount of valsartan in your entresto is clearly indicated on the label. in this case it's 26 mg. it looks like the lowest dose of valsartan is 40mg, you could cut one in half maybe. Let your doctor set your meds, just tell them you have doubts about entresto, there are 9 different generic ARB available, actually losartan is more common I think.

@@itsjustvalsartan7844 My "Primary" and "Cardiologist" both advise me that I can go back to taking Lisinopril if I feel that made me feel better and have fewer side effects than now while taking Entresto. ((They advised me that Entresto-24/26 is equivalent to Lisinopril-10.)) Now I understand your point and I agree with you that I should work with an MD to advise before going to Valsartan, especially since I have no history or experience having taken that drug in the past. Thanks for your response. ((note: I live in USA, however I also have access to most BP meds without a prescription...but I also do not like dealing with Cardiologists.)).

@@joshpembers I've got a hunch that the ARB just works better for people who also have COPD, and CHF, which is a lot. The enzyme ACE is found in the lungs, maybe stands to reason that people with lung problems would benefit from ARB over ACEi in this case. I dunno. My ejection fraction has went from 25% back up to 55-60% using Valsartan 320 and diltiazem 240 once a day and colonadine 0.2 3 times a day and carvedilol 3.125 twice a day. Im 28 years old

Hi. How long were you at the lower level of ejection fraction ? I think it’s because your so your that’s why your recovered so well? Do you mind me asking you what way you felt when it was at 25% ? I’m at 35% my energy level is up and down some days some not so good

@@robarbour3982 my ef was down for more than a year. My energy level was horrible when it was low, however when it was lower my main symptoms was shortness of breath. My energy levels are still up and down. Some days I just don't feel good at all even tho my ej is back up to normal.

When I found out it was that low I gave up and was just going to let it run its course because I was tired of doctors and hospitals and all that. I know it sounds bad. I ended up going to the one hospital that I hadn't already been to and they are the ones who got my bp down and thats when my ej started going back up

Hi @CardioGauge. I (27 Female) had my daughter six weeks ago and just days after labor I developed PPCM with an EF of 45%. My PROBNP level was 4,913 with elevated D’Dimer. My chest filled with fluid. I have been taking Entresto 24/26 twice daily, Carvedilol 3.125 mg twice daily, and Spironolactone 12.5 mg once daily since it happened (in addition to my maintenance meds, like Levothyroxine 50 mcg and Valtrex 1000 mg). Up until developing PPCM, I had a history of angina, HSV2, hypothyroidism, gestational diabetes, HR non-16/18 HPV, and diet controlled high blood pressure. My first few weeks taking these medicines I had new onset memory problems, new angina, new headaches, new onset heart palpitations, new sleep apnea, low BP, and bouts of extreme* confusion that got progressively harder to deal with in following weeks. I visited a different cardiologist that told me to cease Entresto (so I did) and much of the confusion and fogginess has abated, as well as most of the heart palpitations and feeling like my heart would stop. There is still a bit of dizziness and low BP. I am still on Spironolactone and Carvedilol, but I want to know what I need to look out for. I am getting BP readings (still) like 100/56 and fogginess. I was wondering, 1.) what BP do you, yourself, consider too low in your patients? 2.) How long do you typically keep your PPCM patients on HF medicines? 3.) Have you seen adverse reactions like these in Entresto patients? 4.) What has been your experience with PPCM patients and their recovery? I understand you can’t give out medical advice and that only my doctor can direct me in what I should be doing. I apologize this comment is so long. This has just been a really stressful time and I’m trying to be more informed about my condition.

My mom has heart failure and she's on entresto. Her pump EF is 25% on the last test before she was on Daflon and vitamin D3. She takes Daflon which contains Diosmin and Hesperidin and she's able to get out of bed in the morning instead of the midday again. She also feels much more energetic and the edema in her face has disappeared whenever she's taking Daflon daily. She also has had high blood pressure for years. She also takes 6000 IU vitamin D3 daily and that made her feel great too. Because she has a dark skin tone (her family is partially Indonesian) she is more at risk for vitamin D deficiency and vitamin D was shown to repair damaged hearts and improve EF in heart failure. She lives in Holland and so do I. Hope this helps.

It's the percentage of blood pumped out of the left ventricle with each beat of the heart. Not all blood is pumped out. 55 to 75% is normal. Below 50% might be considered heart failure in some people.The lower the ejection fraction, the worse the heart failure.

@@BigSamHH I had to change my eating habits. I drank Super Beets in AM. Fish oil pills. …1280mg. Meditated in AM. Salmon 2x a week at least. Sardines are really good also. Find the right vitamins for you. K2 is great for the heart. Vitamin D3 is essential. I threw out many vitamins because they were Toxic to me for some reason. So I try to eat salads at least 2x a week. Excercise…..I only did walking and light weights…as I also have Osteo. Monitor your BP. I’m on a low dose of Lorsartan. Had Thyroid surgery….getting the med dose correct took over 2 years. Thyroid messes with every part of your body and most especially your heart. So if you have heart palpitations have them check your thyroid also.. just had Echo done….heart has improved …a little… I do limit my carbs…..it’s called Garbage Keto as I love Veggies. Try and limit sweets. There is no quick fix. It took me years since my body rejected so many meds including Statins. Good Luck. I wish the Best in finding what right for you. Remember your Cardiologist is your friend not your enemy. Find the right Dr. For you…it took me years also. There are homeopathic Drs out there.

Wow medication is expensive in your country. I was getting a months supply for $6.80. The Australian government has recently changed this to 2 months supply of medication for $6.80 if on pension and up to the maximum $30 for people working full time. Depends on medication. My son works and pays $7 for his meds. Helps that we have universal healthcare
