Thanks again Dr Gupta I'm recovering from Stroke (6 months) with Left side Paralysis.I was in hospital for 3 months.During investigations it was found that I have a heart failure..Left Ventricular Systolic Dysfunction I'm also Type2 diabetic. With all the meds for heart failure, I could not get out of bed. I slept almost 24/7. Now I eat free range chicken eggs every day, real butter, grass fed and finished beef, wild caught salmon, etc. I'm walking 30 min a day, cutting my grass and growing my own vegetables. I could use more energy but feel alot , 80 percent better. Praise our Heavenly Father!!

Dr Gupta, thank you so, so much for this video. I have an almost lifelong case of Afib, which seems to have gotten worse after every ablation surgery I had (four of them). After two of those surgeries, which obviously never worked, I actually got worse, which eventually resulted in heart failure. For several years I had doctors arguing about whether or not I even had heart failure, even though my cardiologist was treating me for heart failure. Finally, someone decided there was an actual thing called HF-Pef......Finally, finally acknowledging that HF-Pef was not only real heart failure but actually represents more of heart failure than the traditional type. But there was still no medication to help HF-Pef patients. Now there is! I am so hopeful now that this might actually help my symptoms, and maybe make life a little easier! Thank you, thank you, thank you.

I am HF-pef too. It can make it hard to get the proper diagnosis and treatment. Dr. Gupta seems to be the only Dr who even addresses it!

I just want to say, for the last 2 weeks, for some reason I’ve had PVCs every 3-5th beat. It has been the worst 2 weeks of my life. I’ve never been more anxious or miserable. Thankfully they’ve subsided today and I’m really hoping they never come back, or at least as bad as this. Dr. Gupta, your videos have helped me get through the last few weeks. Thank you so much for giving us some peace of mind.

And the more anxious it makes you the more they stick around. I know because I'm the same way. I have panick disorder and anxiety disorder and have weeks and even months with ectopic beats every couple beats but once I stop worrying about it I noticed they get less frequent. I also have rbbb that I developed at 27 years old

WOW! What timing to come across this video! Feb 10 guy goes into cardiac arrest, shocked 3x, brought back eventually 3x. (Told died 10 mins, 5 mins and a few minutes) VT (V-Tach) for 7 hours, plus awake, alert, responding by eyes open or a hand squeeze or slight head gesture, during the entire ordeal. Both ICU doctors verified what they were seeing was real ie monitor alarms/readings vs patient status. They'd never seen such. At hour 4 the hospital ICU doctors reached outside to a cardiac electrophysiologist who specializes in treating rhythm disorders of the heart, and by hour 7 he was out of VT, because those doctors were told to reverse SOP for heart attack patients. They did a take down and it worked. In a couple days the breathing tube (in this non-aggitated calm 61 year old patient) came out, and in which a balloon pump was put in through groin by cardiac cath lab. They've done double testing to see if he is a candidate for open heart or stent placement / (pacemaker). He is a diabetic (hyperglycemia) with pretty normal Blood Pressure, except for new USA guidelines along with added white coat syndrome which occurs with every doctor except his Primary Care Doctor. We are still awaiting careful Thoracic Cardiac Doctor decision on what the end result will be to rectify his issues, as 6 days later it is still undecided, but hoping for something concrete in the next couple days. After all that, I have done a lot of reading on the studies associated with Farxiga / Dapa-hf and Entresto, of which I've never heard of until your more than informative videos! Thank you for that! But, wondering if anything "new" could be beneficial in respect to the prior mentioned person, that we could talk to his cardio or endocrinologist about, in future?... (Heck at this point they are considering stent, but I think why wait till something happens again and you are perhaps 20 years older and in worse shape, so why not be done with it and have the open heart since the main artery was 100% occluded and the other 2 nearly as much again. I think the number 55 was for the main and 40 was the other two which actually created Collateral Circulation (amazing I must say)! / Arteriogenesis / Fluid Sheer Stress. Patient guessed could have been 1.5 yrs since was heart cleared for last surgery. Fascinating! I'm trying to recall the number count by those whose care he was under at both facilites and stated they had no idea why or how he could even be alive, let alone functioning normally without current impairment. Astonishing journey this week with this person. Hoping the future outcome proves impressive. There were several not so great things but I think the worst was being in VT so incredibly long which I am quite sure had to have had a significant further impact on heart quality which was already obviously somewhat diminished. Almost shocking really!

Though I am not in any medical field, out of desperation to get my spiked BP down (meds were not working)...I gave up all sugar sources aside from occasional watermelon or blueberries. Within one week, I lost 10 lbs...BP remained high. After 2 weeks, I lost 5 lbs...bp came down slightly. After 2 1/2 months, I have lost 28 lbs and my bp is normal. My heart rate is in the sixties again. The weird thing is that I had gone vegan about 3 years ago and that was when I gained most of my weight and when my bp started climbing. I still cannot eat meat but eat seafoods occasionally. It just goes to show that veganism isn’t for everyone.

After doing some research some docs say it causes a release of toxins and you have to use a binder with it or the toxins can float around in the bloodstream and cause adverse effects and/or the toxins get reabsorbed. Have you ever heard of that?

@It's Highway Time Yea, doc put me on a product called Blackbox by QuickSilver and I took it for 30 days. I told him I wanted to try the sauna for 30 days as well and then retest because honestly I don't feel too much a difference after the Blackbox. Still lightheaded, dizzy, and waking up 3-4 times per night. Been going on for the last 6 years. I've also just recently increased my caloric intake and good fats to see if that will help with sleep and being lightheaded. I think my caloric intake was too low because every time I would exercise it would get worse. Just started the increase a couple of days ago. I'm in the health and fitness business for the last 27 years and know a ton about the human body but this has been throwing me for a loop for the last 6 years along with several other doctors I've seen because other than the heavy metal toxicity all other blood markers are perfect.

@It's Highway Time Yes, doc wanted to do IV chelation which if I am not mistaken contained EDTA. I was a little nervous about it so wanted to try the blackbox instead. Now I will do sauna and then if blood tests still show high metals I will more than likely do the IV chelation. Dr Garry Gordon has a vitamin C formula that contains EDTA, but my doc said it would take years to get it out of my body compared to IV chelation because it takes so long to get the lead out of the bones. Frustrating for sure.... Appreciate your input.

I note that Dapagliflozin is contraindicated when on a low salt diet yet a low salt diet is recommended as part of my heart failure management plan. Something to consider when assessing this new drug.

Are you referring to dehydration and sodium excretion as a byproduct of taking an sglt2 inhibitor? Because that’s a help for heart failure patients many of whom also develop type2 diabetes. Dapagliflozin - farxiga, jardiance etc are not contraindicated when on a low salt diet

An amazing video doctor. You give hope to the hopeless. I have a cardiologist who is very funny. Not really worried bout what happens to patients. I am on entresto. Boy, the side effects of that medication on me were challenging. Drops blood pressure very much. Abdominal pains. Just pains in your back arms neck dizziness etc. I told her and she said everyone who has heart failure like yours feels like you do coz the heart is bigger than normal. Asking bout the side effects of entresto there was no answer. I hope most wont be like me. I cant even take the recommended 1g vit C, vit D 50000iu and zinc 50mg to help my immunity because I did and the blood pressure further very very low and am very dizzy.

Am on Entresto as well the side effects are a challenge to deal with. Correct!! It causes low blood pressure, dizziness, diahrrea and some more effects. Doctors swear by it that this medication is the top notch for heart condition. Only time will tell with the side effects. Much blessings 🙏 and be well.

Dr Sanjay: would you do a video on how to improve ejection factor with diet and exercise. I am not fond of meds as almost all have intolerable side effects, for me. I've been on my own heart improvement regimen with exercise diet meditation and prayer and deep breathing and can now walk for an hour at a brisk pace most days, after being exhausted after a 20 minute walk. The vaccines greatly worsened my heart health. I listen to your channel every day and wish you were here in the USA. God bless you and thank you!!!

This video would indicate that the root cause is Heart Failure patients that benefit from this use of this game changing drug are consuming too much glucose and therefore, rather that taking a drug they should go on a low carb diet and do moderate exercise., like going for walk after each meal to burn up some of the glucose consumed during the meal. Cardiologist like most medical specialists got to where they are because they are great "rote learning machines", not because they are good at problem solving.

Sadly my father developed CHF a few weeks after getting his second booster. He was walking 4 miles daily and now can barely walk 1/2 a mile. His cardiologist surprisingly hasn’t mentioned SGLT inhibitors. We see him in a week and I will definitely bring it up since dad is only decling on metoprolol, lisnolpril and amlodipine. I’m praying the SGLT works.

That's because there's no money in not consuming sugar (and carbs in general). Actually, the complete solution (AS IN CURE) is FASTING and FASTING MIMICKING diets (like KETO) since Heart Disease is part of Metabolic Syndrome whose primary symptom's are Obesity and Diabetes. But again, YOU CAN'T SELL FASTING. LDL isn't bad: SUGAR COATED LDL is bad, because the body doesn't recognize the LDL anymore (lost it), and can't use it so the body makes more LDL: Where do you think all that lost LDL winds up? York Cardiology is only in it for themselves. They should move to the US and stop destroying the UK health system. Diabetes type 2 is linked with heart attacks, insulin resistance, inflammation, heart disease, plaque build up in the arteries. Is it possible that people suffering from heart failure may be suffering also from direct side effects of Diabetes type 2. It is now very well proven that a low carbohydrate diet, high good fat, no sugar, no vegetable oils diet will reverse diabetes and the high levels of glucose and insulin that cause so many other health problems! Thank you for discussing a pharmaceutical drug that shows incredible promise for patients that will not eat a healthy diet or those that have done so much to weaken their heart that they may have a chance at some semblance of decent heart health. On my last visit to the doctor's office, I was asked (when he was headed out the door) if I've ever thought about stopping this medication because my latest test shows my thyroid is normal. Question: Under the circumstances, could this medication be the cause of my heart palpitations at night? 💕💕💕

I was diagnosed in June 2022 with left side heart failure probably due to the medication I was being given to treat the after effects of covid depleting my steer of vitamin B12.My EF was 21% ie severe heart failure. I was put on dapiflagloxin and other medications along with eod B12 injections. My EF is now 41% and the chambers in the heart have returned to normal size. The downside of Entresto is that it raises blood sugar so every time I have a blood test I have to explain to the GP that ‘ No I don’t have diabetes . It’s the medication I take’

My uncle is 77, had a heart attack, diabetic, and once he started on empaglifozin, which is another sglt2 inhibitor like dapa, he is doing significantly better. 4 months now and still alive.

Thank you so much for this ray of hope. My husband takes ramipril, bisoprolol, epleronone and amlodipine but he suffers from breathlessness and fatigue. His heart failure symptoms worsened significantly six months ago after a fall which resulted in a subdural haematoma. I wonder if you have ever come across a case of this happening.

Thank you AP. D3k2 sounds especially appropriate as he also has a severely calcified artery (LAD) and is waiting for a stent, which he can’t have just yet as he would need blood thinners first, which his neurosurgeon won’t allow yet because of the risk of another bleed on the brain. Thanks again.

I have CHF I’m 67 years young but I have never had a heart attack or stroke but my blood 🩸 test indicated that I was low on vitamin B12 and my doctor never told me anything I found out on my own and a look up on google indicates that lock of B12 can cause a week heart ❤️ my doctor had no clue 🕵️♀️ therefore if you have CHF check your blood test thoroughly because they don’t know anything but to prescribe, prescribe and prescribe! They even told me that I had asthma in the beginning. Just keep the hope alive that’s the dream 😮

Dapagliflozin was first approved by FDA in 2014. There are risk factors and side effects and it may not be the best option for some heart patients. Ask your doctor about Entresto if he hasn't already prescribed it instead of Dapagliflozin.

Doc there are reported side effects of the gliflazines to pancreas, kidneys etc not to mention severe thrush, can these drugs be taken as a preventive measure for patients with none heart failure ?

My dad has 50/55% left ventrical. He is on Furesimide, 3 tablets, spirolactone, and a few others you mentioned. But he can loose too many salts, his are 1.32. His legs are big and his feet, can't get shoes on. He had to go to hospital in April to have water drained off him as it went to his groin area and up to his chest. He isn't too bad now, but the swelling is still there on his feet/legs. If he takes too many diuretics then it mucks his kidneys up

My husband had chf, EF 40%, was on lisinopril cardiologist switch to farxiga, he’s not diabetic but had side effect of burning sensation on urination, so he stopped it then was switch to entresto the lowest dose he was ok for 2 weeks but on the 3rd week, he has a lot of muscle pain, 8/10 scale n severe bronchi spasm coughing so he called the cardio n stopped the meds for 3 days now after 3 days switch back to his usual lisinopril

So to access this drug, would it just be clinician awareness when getting to use the drug for heart failure? As it’s already approved for diabetes, I think it’s called “off label” prescribing in the US when you use an approved drug for a condition or age group other than what it was intended. The doctor has to state it is medically necessary.

I have heart failure and diabetes type 2 and take both Entresto and Forxiga (dapagliflozin) and Metoprolol, Digoxin and Spironolakton. Afternoon adding E Entresto there was a difference, don't k ow about Forxiga because have had that longer. Heart has become a little better, doctor thinks I have right medicines. I live in Stockholm, Sweden.

Would you please spell out the medication you talking about.. Also this week had echo- normal sinus rhythm with biaterial enlargement and ST andT wave abnormalities. He says one of bottom chambers is doing extra beats. Did prescp of Diltiazem HCL ER Coasted BE 120 CP24 I think its for high blood pressure. Would this clear up the extra beats, Thanks

Leona Owen diltiazem is a calcium channel blocker which does lower blood pressure. But it also slows conduction through the AV node and bundle of HIS, so it will slow your lower chamber contractions. It can definitely stop or slow the extra ventricular contractions.

So many people take steroids so I have a question from my recent experience. I had 6 days iv steroids ( 500mg I believe) for autoimmune disease and now over a week later I have developed tachycardia.never had it before. Can iv methylprednisolone and other steroids cause (sinus tachycardia) after you stop taking it?....and will it progress to Afib? Sorry to be cheeky with a question Doc? But its freaking me out!

My cardiologist gave me a 2 week supply of Jardiance yesterday to try out, I hope it's better than Entresto because Entresto did absolutely NOTHING for me. I will check back as soon as I know something, wish me luck I could use some. My question is, how does Jardiance improve life, what should I expect if it indeed is working for me? TheReaper!

Do I need this drug? My chest X ray says: Aortic Atherosclerosis is 170.0, so how bad is that? Is there a chart that shows what that means? I have eleminated any supplements with calcium phosphate. Is it too late?

Hello Doctor, Please update the Dapagliflozin information with explanations of WHY it works for CHF patients who do not also have diabetes. If the method of action is by washing out glucose from the bloodstream then why is is this beneficial for heart patients ? If blood glucose levels are not an issue for us would Dapagliflozin reduce blood sugar to a dangerous level ?

Than you --heart attack on oct 2nd 2022, using bisoprolol right now---now i know not to resent my increased urination levels though i wonder wether they factored in my anaemia as my carers noticed a total absence of red blood vessels tho my blood tests have proved ok.
