My downstairs neighbours renovated three staircases (the lowest 3). And the workers extended the stairs to add an extra step to each case, so the lowermost step now extrudes out the railings. Then they didn't even smooth out the edges of the new stairs. Do now, we have watch that we don't cut ourselves at the sharp edges, especially the extruding step.

A lot of these are the result of badly planned remodeling. They change around rooms and hallways and forget the the stairs can't exactly be moved or adjusted because they're part of the buildings infrastructure. Halls and rooms are easy to move, but the stairs are part of the floor and expensive to rebuild, so they usually end up fudging it at the end of a project.
