I have been told I have bone on bone arthritis in my left knee, so six months ago I decided to get serious about taking care of myself. I've lost 50 pounds, exercise (gently) in the pool and I have watched many of your videos and have incorporated your suggestions into my daily routine. Strengthening the glutes has made the biggest difference. Thank you so much for your practical advice. My knee still acts up at times (when I've overdone it the day before) but otherwise I barely feel any pain.

I so love that you offer your time to explain WHY you recommend what you recommend. Your videos continue to help me and my friends who are suffering with joint pain. As an RN, I don’t know one of my coworkers who haven’t injured themselves in one, more often, several joints - knees, necks, backs, hips. Thank you.

I'm sending this video to my friend who has tried everything for her knee except pain killers! She is on the verge of injections. If I was her I would be eternally grateful for this broad coverage of safe treatment for her osteoarthritic knee. Right now she walks until her knee hurts and keeps walking a little farther.

I tried several of your techniques like massaging my tight thigh muscles and trying to strengthen my glute muscle by doing only a few seconds I already feel like I have no more knee pain. Hooray no need to go under the knife. Will work out more and do some biking, drinks more water and lost some weight 😁👍🏽. Thank you Doctor!! I was told I had osteopenia but not what the heck I could do to improve it. LATER they told me I had osteo arthritis but NOTHING about improving or at least how to TREAT MY CONDITION. Then I was told I have arthritis. Same story ...nothing about what to do. LATER I was told I have ARTHROSIS. Now I'm almost 78 yrs old horrified and petrified about SSURRRGEEERRRYYY....😢

I am 37, detected with Stage IV bone on bone osteoarthritis, left knee is fused and right has 90 degree motion. Doctors say I have to make peace with whatever is left, practically. Cannot have any surgery because I have grafted skin on my legs. Your videos, and the excercises you've recommended are like Krishna's blessing... Thank you again for teaching practical exercise. Tomorrow a good friend Dr is going to inject hyaluronic acid into my knee. I told him I WILL NOT HAVE a knee replacement. I have been watching many of your knee videos and they have been so helpful and I have found concentrating on strengthening the glutes has been amazing. Great info very helpful, before my holiday my knees were starting to hurt could not figure out why, then on holidays in hot tropical Thailand my knees were in really bad pain, possibly more inflamed as I was on my feet all day in 35 degree heat. Tried to some stretches and even ibuprofen which helped but now back home in cooler weather doing these exercises and others has really helped and I like the bit you mentioned to don't stretch too hard but more often, that makes good sense and I am seeing good results, thanks so much for this valuable info and keep up the great work.

@@epmanualphysicaltherapy I’ve been told I have bone on bone, but what’s not bring figured out is why I can be walking, just simply walking, and my knee will all of a sudden feel like the top of my leg has gone over the bottom and It’s an excruciating pain with swelling. I then can’t put my foot to the floor for a couple days, then I can put the ball of my foot down but not the heel, then about day 3-4 I can step again. It’s happening with more frequency- about 1x a week now😢. What do I need to push in asking to be looked at?

Thank you for your videos! I almost have my husband convinced we need to relocate to El Paso so you can be my therapist. I have a near complete root tear of my lateral meniscus, that sadly responded quite poorly to surgery. I've barely been able to walk for two years .I am so quad dominant it's like none of my other muscles exist. I was in PT for six months strengthening my leg muscles! I've started following your quad exercises, and hopefully I'll be able to balance out some.

I can't believe you sent this video today! 2 nights ago I went to stretch my left leg in bed and felt pain. I took it easy yesterday and feel much better today. Thank you for these tips that I needed to review again. 🙏 💪 ♥ Also, I believe the problem was due to me walking on lush, high grass Wed afternoon. I try not to walk on grass because it tends to be uneven. Can you address that sometime? Does it make a difference?

As you feel stronger, you can gradually increase both the range of motion and intensity. It’s best to do these exercises every day throughout the day. I have been a bike road racer, coach, and now avid cyclist for decades. All of this time I have been doing this on a totally inadequate/weak left knee and lower left leg due to a serious motorcycle accident at 17 years old that all but avulsed my leg. I have developed a form of riding that is incredibly efficient and low stress on the knee. My junior riders became vey good at time trials using my methods. It does focus on using the glutes, shorter crank length and keeping forces very smooth with much less stress during all but the highest intensity. I wish you were nearby so I could show you.

As I mentioned in the comment, shorter cranks could help by reducing the amount of travel your joints go through and also reducing angles. This will also help with reducing peak loads which, for a fragile knee, are important. You can search for this video by an engineer and cyclist how this works "Bicycle Crank Length Debunked: Position and motion analysis using 4-bar linkage" Certianly peak loads are sometimes difficult to manage (steep hills). I imagine your seat height is correct as I am sure you are really experienced. Always work on keeping your body relaxed as possible. When I ride I ask myself which are the muscles that should be working and which should be relaxed. That goes for the entire body. That includes keeping your back flat and stress off the shoulders. I also maintain the right gear. I never want to push too big of a gear or spin too fast (that really hurts) As I shift to a lower gear and find the same effort I go faster then I know that is a better gear. I had to adjust my cadence somewhat as I used to spin slower with larger cranks. This may seem like a lot of constant attention but for me it is something I enjoy. I love to get more efficient with better form. I can look at all kinds of "rules" to be a better cyclist, but it really gets down to constant attention. When I ride it is often like meditation.

Great video. Im a former dancer and runner and my right knee has deteriorated to the point where my lower leg veers to the right. Also when sitting and strengthing glutes, i noticed that when i squeeze my body rises a bit making it noticeable that im doing just glutes. So with lower leg angled out, any suggestions? Oh, i also get deep tissue massage every two weeks and am 73 years young.

You are highly intelligent and helpful for many people. My question is that from last four years my joints are giving sound whenever I stand up after sitting 2 or 3 hours and in the morning also now I feel fatigue and slightly pain in my knees. Doctors said vitamin d deficiency is there so take supplements I for 3 months but I couldn't recover that sound and still there is crunchy sound and sometimes pain what I should take please recommend someone supplements or food.

Trekking poles give better posture than leaning into a Walker or putting the force in on one side of the shoulder like a cane. They’re also better for the cardio vascular circulation aren’t they? 4 pts of balance …adjust shorter for incline, longer for decline. They look more fit and allow for loading the glutes more smoothly with a longer stride.

Your thought's on knee swings with added weight and shorter swings. Both my knees are bad and my hamstrings so tight they knot up instantly trying to do anything. I have been walking thimking I was doing the right thing, but after this I will take some time off and work on my lazy gluts. Thanks for the video!

I had a right knee replacement 6 weeks ago. My rom is 70 and flexion is not straight. Doc says a build up of scar tissue which doing MUA procedure next week. Anyway my left knee wasn’t as bad as the right but now after 6 weeks of my other knee taking the brunt of the weight for all those weeks, my knee is now cracking at times and painful. No way can I go through another knee replacement. I do have a brace from physical therapy which has the hinge and a good quality one from insurance. Oddly the brace was made for the wrong knee at the time 3 years ago so I never had to wear it since it wasn’t the problem knee but now it is. Would wearing this brace save me at this point? Also since my knee isn’t straight for the moment until this procedure, should I be continuing with the walker most of the time to take weight off this other knee? I’m seriously scared as now I’m hobbling around with two bad knees and I’m still not comfortable with the cane although I use it when out

Hi, I messaged you a few months ago and as to your advice have been doing some glute exercises. Its tough as I am so used to using the thigh muscles.. I catch myself doing that. One question I have is about the tailgate exercise. It seems that everytime I do that, I feel like the sides of the knees seem to tire very quickly. Whats the cause and how to avoid that?

I seen 10 sec and have myself patella mi... that stuf with a pcs. of cartilage halfway torn off kneecap, it once got stuck but somehow managed to get free. I kind of know where it is at at the patella. Anyway, all his vids reg. kneecap are somehow relevant so I allso see this one and will respond with answer to this comment how relevant it actually is for my situation.

@@epmanualphysicaltherapy Hi, my name is Carsten Nielsen, you once remembered my name from another comment. When a doctor recommends replacement vs. training, then that may be due to, that it is allmost impossible for the doc to know how much and how correct you actually do the training, so a kneereplacement is a more safe recommendment, cause what if the person train enough, wrongly or slight wrong, then that person may accuse the doc for bad advice. If the person have a not optimal physio, then that is yet another variable the doc have to evaulate the situation in light of. More variables make it more unsafe, espc. these are both dynamic or unknown (who is the physio?) It is impossible to standardise how much a certain person trains correct even with help from a physiopt. Studies for physio is highly with many diff. outcome or succes, I guess...?! The nonworking cases then need replacement. I above asked how much nonoperative kneecartilage problem can engage in martial arts training, not sparring even. That also is highly individual and so more a subjective evalution. I will ofcourse take care of not going too hard with the injuried knee, but when is it too hard....?! A highly subjective case this is. But if pain can be used as lead, then that may be a very good indicator. Pain may not come gradually though, so when it is too hard? When this, being too hard, is the case, then it may do damage and tear of more cartilage?! You see the problem in subjective approach? What is not subjective is when doctors can use.... legal support from a big pharma supporting their operative components into the knee and that the evidence and approval clears this and them from payouts for problems and failed operations.

Three orthopedic surgeons have said the same thing after doing their own X-rays of my left knee. That they recommend knee replacement surgery. Unfortunately, years ago I ruptured my ACL and went through that surgery repair so that I wouldn’t face a knee replacement, but that surgeon, perhaps didn’t realize my cartilage would wear down and I do have genetic osteoarthritis in most of my joints due to genetics. But I am going to look at your videos on strengthening my gluteal muscles. I’ve always had rather weak glutes.

Our body always heal by itself, everything I tried from this therapists never worked. Rest and train what you can without pain, and your body will heal itself. Listen to your body, he will tell you what exercises you can do and not do

I have 4 category KNEE OA since last several years. I am 80 yrs old and had one knee joint replaced. I walk without stick and after my surgery 4 yrs ago , my walking speed has decreased since I could not walk during Covid 19 for about a year as 5e Garden was closed during lockdown down. I used to walk around four rounds after surgery in about 30 mints ( approximately 3km ) Now since I walk for same half distance ( two rounds or 1.5 Km ) in 20 minutes Every time when I walk for initial 4. - 5 minutes I feel very mild pain in lower back and both knees which gets REDUCED later and I walk and complete two rounds after which I feel tired and hence I return home. I feel less pain after cold water on both knees and back and I do physical exercises suggested by my physiotherapist , which I understand are for strengthening leg muscles. I will try your Butt strengthening exercises

My knee swells every so often. Why? I don’t know but I have a feeling that I torqued the knee doing some kind 🎉Of movement. Orthopedic wanted to operate as I have bone on bone and says I have no meniscus, yet MRI says I have a frayed meniscus. Who’s right? So confused. Ortho never read the MRI in spite of me asking him to do it. He said what for? You have bone and bone and if you don’t want to operate, you are going to continue with pain. I bought your knee program and will start. I have been maintaining my knee through your glutes exercises and some physical therapy. I can’t find a dR or PT that has your mind set. Know anyone here in Fort Lauderdale area? Desperate.

Hi Doctor. I have OA in both knees as well as other joints like lower back, right shoulder, right elbow. Do you have any online program which can help me with avoiding surgery despite multiple affected joints? I am taking glucosamine+chondrotin, gelatin powder and Omega 3 capsules on a daily basis. I dont yet have an XRay or MRI done. Can i join your online program, or is Xray/MRI necessary before that? Please advise.

I was told i have patellofemoral syndrome that in the xray my knee looks good but i that i have misalignment in my knee caps. I have good range of motion just pain when bended and laying down for long periods. I used to experience most irritation when walking and bending, but now it's when i lie down. If i stand up it relives. Any thoughts on why that may be? I've been to specialists with no help.

Just had total hip replacement and the swelling has mostly been in my knee. I have a lot of atrophy on that leg ang it feels like my bones are doing all the work instead of the muscles. No loss of motion just impact pain. I doubt this gets read but any advice would be helpful.

Walking in a pool or the park can be good for your knees if you are using your glutes as the main form of muscular control in the motion. If you are using your quads too much, then you are putting a lot of muscular force through your knees.
