Thank you. You are much more than a PT; a great instructor and serious researcher. I attribute most of my leg, hip, and knee problems to bad advice I received a decade ago from a VA physician. His instructions were a perfect match to what you describe as pain inducing.

In 2014 I jumped off the back of my work truck, a height of about 6ft. I'd done this many times b4, but this time my Knees buckled & I laid grimacing. To cut a long story short, my Orthopaedic Surgeon,highly recommended I have a double hip & double knee replacement, but I declined, opting to have my left meniscus removed instead! Eight years later, having adjusted my diet which includes daily intakes of Krill & vitamin K2 (M7) pills, exercising regularly, especially strengthening those Glutes, this 70yr old is still climbing ladders, doing all our own home maintenance work, pain free. I think I dodged a bullet! Thanks Dr David...I include many of your strategies👍

John Norms, good for you for having the courage to not have the surgery!! 👏👏👍. Surgery is not the answer to everything! I tore my Meniscus around 45 years ago and refused surgery and healed myself with vitamins, Physical Therapy, and massage. The Meniscus has not bothered me since then. I do have arthritis and pain only if I eat foods that’ll irritate it, otherwise I can walk around pain free like normal!!

WOW! Talk about “looks being deceiving,” I just started bicycling outdoors again, in an effort to strengthen my legs. & I was so happy to notice some nice definition forming in my quads. I took it as a sign that I’m getting stronger, doing something right. I had no idea it was a result of overworking my knees. Bummer. Happily, I discovered you & your awesome content. So, I’ll just have to start squeezing my butt when I ride, cuz I love riding my bike too much to give it up. I’m a senior & just getting back to riding on a regular basis after about a year & 1/2. So people are already looking at me sideways as I wabble down the sidewalk, getting my bearings. Wait til I start squeezing my glutes as I ride. That’s gonna be a sight. 🤣 Thanks very much for this crucial information, Dr. David.❤️🙏🏽😁

I ride to alleviate my knees, 10-12 hours a week. Making sure I have my seat high takes pressure off the quad also think about your ankle rotation using the calf muscle more, this tends to use the back of my legs and glutes. Low seat is 100% quads, horrible. This works magic for me, if I go more than 2-3 days with no biking, my knees are painful. Good luck! Find what works for you.

I'm currently recovering from miniscus surgery. Aside from huge amounts of physio and strength exercises, I walk. When I use my soft knee brace, my leg/knee feels alot more stable AND more importantly, isolates my Glutes. The difference between my quads and knees being sore after a walk, and my Glutes screaming in pain (the good/workout pain).

Walking is so so painful down the front of my shins, everything you have said makes so much sense now. I really struggle with os I’m only 50 and none of the doctors or consultants tell you how to help yourself. I have been given exercises mentioned in this presentation which are a NoNo. I followed your advice on tightening more of my glutes. I am so ecstatic something works. I’m 79 y/o & retired RN. After listening to your presentation I was in pain on both knees. Did your glute exercises for a week now & has helped to reduce the stiffness when walking. Please continue these presentations. I’ll share these with other retired RNs.

@Joy Herb He said in another video not to strengthen the hamstrings either as it decreases resting muscle length of the hamstrings, which has the effect of over pressurising the knee joint.

100% agree about the power of the glutes... Not only for knee but even more so for lower back. Always shocks me how little emphasis there is to how important the glutes are. The only thing that concerns me whether or not I'm allowing my quads to weaken too much by emphasizing glutes so much...

I did so many things that you mentioned in the video, not knowing that they can be harmful for my knees health. And so at this moment I have a horizontal medial and lateral meniscus tear in my right knee. I just got a steroid injection yesterday and start physical therapy today. These advices are very helpful to me. I will focus now on my glute strengthening. Thank you so much Dr David❤

Dr. David thank you for this! I’m 61 and this year I suddenly started having knee pain. I’ve always been active loved dancing, doing yoga. With this sudden pain I went to see my GP she has sent me to a orthopedic specialist and he told me I have a bit of arthritis in one knee more than the other. So PT it is he called it Core to bottom. Watching this video has completely acknowledged what I thought. I had worked out with a trainer when I was 36-40 and was in the best shape and continued with other exercises until this March of 2023 when I went to do yoga and my knees were in so much pain.

What a great physio Dr. David is. These videos are pure gold. My PT has never given me these detailed recommendations. I already have one full replacement and one partial. Six years later the knee with the partial is killing me. (I am a very athletic 68 year old woman who races single handed sailboats, plays Pickleball, and loves to hike and bike.) Makes me sad that maybe I could have avoided them, but I am assuming that I can still strengthen myself now that I have found Dr. David. If you are listening Dr. David--do you have advice for folks who have already had replacements and want to get rid of and/or prevent pain? The surgeon is now recommending a hip replacement too but I am going to avoid it (hopefully_ using your suggestions. But does everything you say also apply to my knees with replacements? Please answer! And thank you for your service to all of us!

Excellent review!! Great explanations. Thank you for emphasizing this critical issue. I've have talked to I'm blue in the face about one needs to be aware of muscle imbalances. One of the main reasons folks have poor posture with walking, sitting and increased joint issues in their hips, knees and ankles is poor activation & proper use of their gluteal muscles, weak abdominal &/or weak back muscles and inflexibility of their ankles. You can make your brain more aware of a muscle group you want to activate and use more by "gently" slapping that muscle - gluteal mm while you are about to exercise that gluteal mm. You see athletes doing this all the time. Your brain is powerful. IF you don't activate and properly use a muscle, then your brain will just sort of not think about activating and using that muscle. Also, one gluteal muscle = one butt cheek can be working, firing & in use more than the other butt cheek. One must work on waking up that butt cheek with more brain awareness; especially of a mild neuropathy or nerve disease or damage is causing one side of your leg & or butt cheek to be weaker than the other. Butt muscle use and strengthening is mentioned so often because it's one of the most important muscles in one's body BUT it's also one the most ignored muscle (both mentally & physically), in the body.

Wow! I just found your channel. So many of the exercises you have mentioned do cause my knees distress. I have had knee pain biking and thought it was just me- everyone says it is best exercise for knee arthritis. I have, just on my own , have been doing glute exercises…now I will search your videos and find the info on proper walking using glutes!

Hey Brenda! Using a bike will be helpful if you can get your glutes to work during the exercise. All this makes absolute sense to me I had my right knee done tkr end of June but the more I do it seems like it's making my left knee sore and I'm doing some exercises that you indicate not doing I'd like to see more of what a person like me needs to do so I'm not making my left knee so bad that Ive got to replace it too. They offered to do both. Cuz knee replacement is just not that fun. Thanks

Very informative. So I was at “low”stage 4 with my arthritic knees, looking at bilateral replacements, & with did PT for 2 months, with attention to the Glute med/min. Pain, and mobility improved to the point where my PT told me to hold off on the surgery and continue with the exercise plan. Although my knees fill better, I pay the price if I walk too much. Would like to know, how to activate these glute muscles during cycling and walking.

I have knee arthritis and I am gym cycling, yoga and swimming. I had to give up walking as I had a lot of food and knee problems as a result. Please tell us what we can do instead as we do need cardio to keep us fit. This video has depressed me tremendously as I now do not know what I should be doing. I am in my early 70s and was fit till I discovered I had knee arthritis.

Hi Sumibak, We are sorry to hear that this information caused you distress. Aside from the yoga, you can continue to cycle and swim. We would never have anyone stop doing the activities they love but we are suggesting making some adjustments. When you cycle and swim the exercise must be driven from the glutes. Your glutes must feel tired and sore after completing the exercises otherwise there is a possibility of making your knee arthritis worse. Yoga is ok too as long as you have the proper instruction as there is a tendency for many people to over-stretch and cause an injury to the knee. Muscles that help relieve knee arthritis, at this stage, need to be strengthened over being stretched too much. We have a knee arthritis help playlist you might find helpful. Thank you so much, Dr. David, this video has been so helpful. As I listended, I see where I was doing all the wrong things. I went tp a PT who strengthened my quads to steel strenght but I still had keen pain on steps and after walking a good distance. I did machines in the gym which set my knees on fire with pain worse. Of course when I didn't feel better, wasn't getting the results, I blamed myself. I understand clearly now what I need to avoid (all 11 of these activities.) and get my glutes stronger. Thank you again. Ps. the weight loss, looks good on you! I am sure you can feel the difference. I watched your video where you talked about diet and foods that can help the healing/health process. All the best!!👍

Thank you so much for your information. I once having a bad quadriceps tendinitis caused by lifting my knees for few times. I couldn’t walk for 9 days.

All these exercises you mentioned, are done in my exercise class. I will not do any of them as my knees have told me no over the years not to. We do glute exercises, when I walk I try and remember to use them. Good video as always, take care 😊

I love to swim and have water on the right knee. I thought it was a good exercise but noticed that my knee got progressively worse the more I swam, so I gave it up. You have confirmed what I thought. Thanks.

My experience is that doctors and therapists don’t listen to me. As if I don’t know about what’s going on in my body. Do you have glute exercises that are specific for seniors? Left to my own shaky devices, I’m starting your glute exercises at 1/2 the “recommended dose” of 10 reps, doing 5 instead. Thank you for your help and for sharing your valuable knowledge and awareness!!!

Am a new subscriber and have done about 3 videos along with you! I had a knee injury almost 20 years ago gardening. I put my full weight on my kneecaps and developed chondomalacia of my left kneecap according to my doctor. I still get pain in it when I kneel on my knees or even walk too much. Since concentrating on the glute muscles activation, my left knee feels less painful and more flexible. Thanks so much.

I will definitely watch your video about chondromalacia patella and really pleased you have one on that part of the knee, the kneecap! My daughter lives in Texas after moving there a couple years ago to No. Richland Hills. She loves the weather and says Texas has only 2 seasons--spring and summer! lol! Last month may have been an exception, though. Today I was able to do a bit of vacuuming and dusting and this was after I’d seen and done 3 of your videos! They make me feel like I can be successful and not hurt myself when doing the exercises! You are very good at demonstrating them and saying what exercises not to do. I have an exercise bike but noticed recently I had more left knee pain after using it so I stopped riding it. For exercise I had only been walking but that’s something not good for the knee either as you said in that video and my knee pain did worsen after my walk. Will now watch the video link you gave me. My life means I must drive minimally 2 hours daily - having had partial meniscus removal and now arthritis develop in the knee and the ankle means driving with the left leg and foot is painful - do you have any tips Dr. Middaugh for alleviating the pain associated with driving? I'm overweight. My quad muscles are weak. Most of my weight is in my upper body. I'm on anti inflammatories . My knees don't hurt at the moment. I don't exercise. What exercises can I do to help strengthen myself or my quads so I don't inflame my knees?

Thank you so much for your advice. I've been doing some of the exercises in physical therapy that you mentioned and progressively feeling worse. I told them yesterday that I wasn't going to make any more appointments until I could see if I saw an improvement after the third hyaluronic acid injection.
