I am a senior now recovery from hip replacement. Before surgery, I had interviews with medical staff. I received infor but I appreciate the professional, personal input from the Dr's. Often a Dr will ask , do you have any other questions "? We don't know what we don't know. Spots are usually short . I found this video had reminders that I could take my time to hear. Brittany Hudson replaced two hips; June ‘15, Dec ‘17; post pain was MUCH less than the pain before...really was only three weeks of discomfort each time. Very much worth the effort. I've had both hips replaced left 2013, right 2018. My left hip felt loose about 3-4 years ago. Revision on left hip December 2020, left hip infection revision October 2021. I guess I beat the odds. Also knee replacement 2018 and same knee a revision in September 2020. Able to walk the golf course in 2 hours like I used. Stay in shape and you can through all these surgeries- but you have to do the work.

This is a helpful video. I have avascular necrosis and had my right hip replaced 2 days ago. I was sent home the next day. The pain comes in waves, but I am using a walker and have been up and down the stairs twice. I am hoping in the next few days my pain will dissipate considerably. I'll be having the left hip done in about 6 weeks.

Bursa injection mid-December, hip injection 30 December. Physical therapy month of January. No improvement. MRI scheduled for 12 Feb. Worse case scenario is hip replacement but if it will get back on the Picklebal court - I'll do it! thank you for your honest insite

Well thank you for that information,i didnt get so lucky my first 2 hips ,why? Wrong size ,these 2: lasted me about 2:years each back to back,i switched dr's he said he knew what my problem was ,he said you need to have a customized hip ,i had no idea that was a thing so i did have another hip done by this dr,this time my hip was great , lasted 14 years on that one ,i started to have problems so i went back to my surgeon,and I was starting to have pain and limping ,so the same surgeon did another hip ,not sure how this happened but i started having problems before 2 years on that hip ,i moved to another state ,while working as a cashier,i had pain and i was limping ,then my hip began to lock up ,after a 8 hour shift i clocked out ,i was exhausted for a number if days i would come home get in my recliner, wasn't sure how i was going to cook ,let my dig out ,after an hour i got up fixed something easy to cook,i was so miserable,i couldnt take the pain and my exhaustion was getting bto me ,so i went to emergency room ,they took x rays,,said there is something going on referred me to orthopedic and spine dr mire x rays ,they told me i was too much for them ,referred me to orthopedic surgeon,he said i needed surgery,so this is my 5 th surgery on my hip ,he said when he opened me up he was like ,it looked like a bomb went iff in my hip ,said i had enough metalosis to form a baseball,he scraped all the metallosis out ,i had so much bone and tissue damage ,surgeon put a new liner and antibiotic spacers ,i came out of that surgery to find out what a circus it was ,surgeon said i would have come back for another hip surgery to put what i say the missing parts in so i am at his mercy ,its been 8 months ,i am getting around with a cane and pain pills .i see my surgeon in about a week to find out what is next ,these surgeries are all on the right side,i am sorry this is so lengthy but i am getting my story out there so no one else has to go thru all these surgeries,oh and i have a recalled part or parts .that is the ball and ?? I am so tired of all if it and want my life back ❤️ soon .i believe this surgeon is going about my hip the way he feels is right and i believe he is .prayers please i am 61 and would like to get back to work get my life back so i can enjoy life

When my doctor told me InwS scheduled for left hip teplscment in Frb for May 20 I was totally floored. I onew nothing about what to expect except the little I understod from her exolanation she was doing it Anterior. My daughter and granddaughter both talked over me at that appointment so I lost track of most of what was discussed. I read and read thenotes of the visit and the After visit Summary we were given to tell us what to expect. In this case the Summary was 40 pageswith almost 10 repeating food choices. I didn't understandmist if what was written in both papers until I found your podcasts and heard your expkanations if Total Hip Replacment and then when reading both papers again recently I understood almost everything. Your Podcasts explain joing teplacment surgery so anyine who really listens can understand. I have had some possible setbacks at 4 weeks before surgery I found sime cavitys in my teeth so had to wt them fixed before surgery but by 3 weeks befire lije the dicyor wanted.when the dentistvscheduled the fixing the cavitys my surgeon mived my surgery forward 3 days to May 23 to have it exactly 3 weeks after the dental work but fortnatwly thevdentist coukd get ne in on April 31 fir the dental work. Then the Pre-surgery physucal found I had a urine infection and I was put on Antibiotics 4 times a day for 1 week which aksi covered the dental work. Tiday I am doind a final test to know if the infection is gone. Then I will be cleared for the surgery. I guess nothing worth while is easy. Have no more appointments until my surgery date. I did have a conflict about the walker I can use after the surgery. A young nurse tried to convince me that I would fall and not be able to use the 4 wheel rollator style I have only used for the past 9 years instead of the small 2 wheel walker with skids on the back legs. My surgeon gave me permission to use my 4 wheel rollator instead after I messaged her that I have only used the rollator style for almost 9 years and have never fallen because of it but would use my 2 wheel walker if she insisted. Listening to your podcasts and the other podcasts on U-Tube has given me so much information that I am not so fearful of the surgery as I was in Feb. I am still nervous about it but also a bit excited to be able to walk without leaning on my arms on my walker because I cannot put weight on my left leg. I am also hoping the surgery will make my left leg as long as my right. Now it is about 3/8s shorter and I have fitted a heel lift in my left shoevso I do not stand on my left toes to make my legs even and that makes my balance off.

Thank you for these wonderful informative videos. I'm considering hip replacement after years of pain and limited movility and have found your information both helpful and comforting. My issues involve both hips as well as plantar fasciitis. While I realize my feet are no where near my hips, I believe one condition exasperates the other, and perhaps could even have been the springboard for multiple issues.. my knees have recently become painful too. At my first ortho appt. (to evaluate the hip issue) it was made clear that only one hip would be 'worked up' at a time, and I was asked to choose. Non-operative options have been exhausted as well as an arthroscopic procedure, now 3 months past and my pain is worse than before surgery. Hip replacement seems to be the next step. I'm perplexed to think that the focus has been completely centered around my right hip almost to the point of being myopic. Treating the right hip alone has put more stress on the left hip, so much that I elected to expand PT to include both sides in addition to some back strengthening exercises in an effort to find some sort of balance... Shouldn't the workup include a broader view?

OH GAWD !! PT Has made me worse. I have heard some others say that PT involves constantly moving an injured or damaged area and this should not be done. I have friends and friends of friends who say PT made them worse. I should have known enough to reject it when it was suggested to me. After trying it, I told one doctor it did not work and caused more pain and stiffness and crepitus . The doctor just said I should try it for a longer amount of time because PT will strengthen muscle around the damaged or injured area . It sounded not applicable in my case at least, and at worst complete illogical nonsense. You will end up injuring the already damaged or injured area before the muscle around it builds up. At best PT did absolutely nothing to help, at worst it increased and moved the pain elsewhere where I did not have pain before. I have come across doctors who try to emphasize and put me through PT but I have to out rightly resist. If it does not work or the pain and condition gets worse, the doctor would just shrug and say "Ok so it did not work, so what?" but I have to live with the worsened condition and I cannot just keep doing as certain doctors tell me to do regarding PT and not be improving and not getting better and only postponing certain treatments that may work better. I was lucky to get some cortisone injections and they work excellently for the pain. I wish the medical establishment would wake up about PT. It should not be a first resort to "avoid more serious procedures." It can eventually make it necessary to do more serious procedures. A lot of people have told me that cortisone is so miraculous and they wondered why no doctor I went to immediately suggested it. It took me years to find a doctor who would do it. It has been the best treatment overall even though I probably need more than one injection.

Well guys my hips are bone on bone I have worked and played hard and im paying the piper now. Just saw my surgeon last week he asked if I wanted both done since I don't have a good one. He advises he will do the anterior procedure and im happy to hear that. He did advise I could play hockey but Im a goalie says that wont happen (ive read NHL goalies have it done and play at that level- purebreds vs donkeys Im talking about me being the donkey) He advised it will be a lot of work at the front end of recovery but worth it. The pain right now is off the charts now and seems to get worse by the day, I have a very little range of movement and I was into karate and could do the splits I would be happy to be able to walk without pain at this point. May 2022 I had my l femoral head reshaped. December 2022 , 7 months later, my mri showed osteonecrosis which I think was due to the surgery. I’m headed for a THR now. Thanks for your series.

Hi Joyce. Labral tears is an important topic that we will try to address in the future. Typically as hip arthritis advances, all patients will get labral tears - they go together. The unique case is when someone has an isolated labral tear and minimal arthritis. Those are the patients who would typically benefit from arthroscopy however that patient is rare. You two gentlemen have been a great source of information for me as I was getting ready for my TKR and THR on my right side. My TKR was done late October 2021 and my THR was done late January 2022. My arthritis has also destroyed my left hip and I will be having it totally replaced in a couple months. I am concerned about my mobility after this surgery. I was very fit until my back failed me in my early 40s. I have a T10 -S1 fusion and a C5-C7 fusion to go along with my new knee and hip. Due to arachnoiditis, I’ve been using a walker since 2014. I am 61 years old and want to continue to walk, even if I need a walker most of the time. Should I be concerned about having my other hip replaced?

Doctors, great series of videos. Apologies if I’ve missed it but have you discussed a double hip replacement? I’m 58 with severe arthritis in both hips. At a previous consultation a surgeon proposed to replace both hips at the same time. Is this a procedure you perform? If so any additional risks and impact to recovery? Kind Regards, Dave UK

Hi Dave. So we have not addressed this but have talked about it a little in some of the videos. So we do not perform simultaneous bilateral joint replacements. The main reasons are increased pain, slightly increased risk of complications like blood loss or pain but more for us that we want to ensure patients like the first one before we give them a second one. Many surgeons do bilateral surgery successfully and if you are young and healthy, this is the group that gets it. It does avoid prolonged recovery and multiple episodes away from work so we understand the desire. Hope that helps. Thanks so much for watching from across the pond! Stay safe

I'm wondering if you could talk about specific things around the hip that before replacement are chronic like a tight IT band, how that and other things like a very sore knee, revert to normal after a replacement. Or do they?

They may. Some knee pain is referred from the hip. If you have a bad knee though it will likely still hurt. IT bad issues can sometimes persist. Physio is a big part of the recovery.

Hi El. Yes sometimes there can be associated knee pain with hip arthritis. Some of it will reduce after total hip replacement depending on how much knee arthritis you have. Thanks for your question.

So it depends on your pain. Sometimes both are done at same time but often stages 3 months apart good luck

Hi, I'm scheduled for a THR 4/24. Currently, I skate once a week, limiting myself to 3-4 jumps and a couple of spins, but it hurts. I've skated 30 years as an adult hobbyist and hardly ever fall, but the bone on bone is causing less mobility and a lot of pain. Understanding this is potentially a high impact activity if I were to fall, do you think following recovery I will be able to continue skating and jumping off of the replaced hip's leg? I land on the good one. What about skiing on gentle slopes (greens and slight blues) where I also rarely fall (once a season maybe).

Hi Glen. Surfing may be okay but it is the falling that will be the issue. You could dislocate your hip or break your femur. You will have to decide how important it is to your life. Thanks for watching and be careful.

Hello --this question is a follow up on a question last night regarding loosening of the tibial plate. As you pointed out, the tibial plate is the weak link in this operation for a TKR. I am curious as to how often do you see a patient that presents with a loose tibial plate? Is this a common occurrence in your practice, or, is it a relatively rare event? Is there, or, are there common events that cause the tibial plate to loosen? Thanks again

Hi Tom. So the loosening is typically called aseptic loosening which is actually a result of microscopic breakdown of the plastic liner that leads to loosening of the tibia. It is much less common than it used to be due to better plastic liners. The longer you have the knee and the more wear of the liner the higher the chance for loosening. Hope that helps. Excellent question. Perthes disease can present unique challenges in the setting of a hip replacement with respect to both the femur and the acetabulum. It is a good idea to look for a surgeon that has a particular interest in hip replacement surgery and also someone that does hip replacement surgery regularly. Having said that, there are many surgeons capable of dealing with this issue.

Not sure if this is the video where you mention it, but I am concerned about the potential for inflammatory reactions from wear particles. If they happen, are they usually localized to the hip? I am guessing it wouldn't be recognized as from coming from the artificial joint if it were systemic inflammation. In the same vein I wonder if there are concerns for patients who have had hormone receptor positive breast cancer (I had DCIS, breast cancer "lite", sometimes called "stage 0") since part of the component is plastic? Doesn't seem likely it could make it's way to interact with breast cells but I sure don't have a clue. Any problems for folks with autoimmune diseases? Just curious if there is a body of research about any of this that you are aware of? Thanks.

With respect to stem cells, yes, this is relatively new technology that is still in developmental stages throughout orthopaedic surgery. My surgeon has suggested he may do a partial knee replacement vs a complete replacement. He suggested I may have better movement afterwards. I ride my bike approx 5,000 km/ year. I’m 67 & I hope to continue to do this for 10 yrs+/-. 1. After recovery, will 1 vs the other be more desirable? 2. Will 1 vs the other last longer than the other? 3. I hope to have my surgery in November, recover across the winter & be back riding with my cycling club in the spring. Realistic?

Hi Harry so a partial is easiest to recover from. Biking in the spring is realistic. The partial may not last as long as often 8-10 years later they have to be converted to a total knee replacement. One thing you do not mention is recovery time and if physio will help. I had my left hip replaced on 11 December 2023 due to arthritis. I did not lose much blood during the surgery but had an internal hemorrhage shortly after the operation and my hemoglobin dropped from 15 to 7. They put two bags of blood in me and I had to take iron and folic acid treatments for a couple of months post-op. I ended up spending a week in hospital. The surgeon told me that the recovery period would be 6-8 weeks and that I would not need physio but it has now been more than 4 months since the operation and I still have stiffness in the joint and walk with a limp. Is this normal for someone of my age (70)?

Hi Murat. We are not big advocates of resurfacing as it primarily preserves the bone of the femur which is less important and often requires a larger socket which removes more bone from the pelvis which we feel is more valuable. I was walking without a Walker week 2. Returned to work week 3. Was mountain bike riding the same hills I used to do before within a month. Its 4 months for me now. Hey Docs how about Brazilian jiujitsu? I've read online some people are doing it after getting HIps done. One woman competes and won gold after one year post hip surgery. I'm still going to try it anyway but it's never mentioned in activities to avoid.

@@TalkingWithDocs Just letting You know DRs, I had the dual mobility implant. He also used a camera. I've been doing jiujitsu for over a month now a couple times a week. I'm also doing full body squats, one hundred of them every night, as well as my two hour a night mountain bike riding. I have zero pain in the surgical hip but now my other knee and hip are giving out slowly but surely!! I'm not afraid at all to get it done now though. Dual mobility is the way to go if you want to remain active. I'd one thousand percent get the other hip done because I literally have zero pain on my surgical one. I have my life back.

Hi Robert. We can't think of a reason that you cannot take testosterone because of a total hip replacement. Hi debra. We use special drills that slowly go up in size until you can't go up anymore. The damage would have to be way below the neck to be an issue but if it is, it just means more metal to replace it. There are good solutions for this problem. Hi El. Yes there have been some recalls of specific implants however there are some surgeons that are still performing metal on metal bearing surfaces. The other bearing surface options would be ceramic on ceramic, ceramic on polyethylene, or metal on polyethylene. Hope that clears it up.

I have an inherent leg length discrepancy that I have had my entire life. Both hips need to be replaced. If I get the longer leg replaced first, at what point will xrays be accurate to determine the final length of the surgical leg? In other words, when the implants are placed and the skin is closed, will that be the final length of the leg or will there possibly be changes with healing?

I'm needing bilateral THR. First the right. X-rays show cyst's from none on bone for a while. Will that complicate things? My dr says I have to lose 40 POUNDS before he will do my surgery. I'm only ten pounds down in two months. Is there a faster way you can suggest? My pain is crazy, walking with a cane and can't sit or sleep good if any????

Hi Coral. Sorry to hear that. We do not put specific weight restrictions on patients unless their BMI is above 50. Best advice is whole food plant based diet. Sensible portions. I am on day 11 after hip Pouteria surgery. I woke up from surgery like I had no pain med. After given a oxycodone and over half hour given another one. Finally got pain under control and tolerable. So I started feeling a lot better after five days. Then for whatever reason seem to be in a lot more pain. I was told that happens and is normal being that outer area is healing faster than the deep inside area of the surgery. Is that true?

How is a THR going to affect a previous but recent Lumbar Fusion (L4-L5). The fusion is needed first bc I can only walk or stand for about 2 minutes before I have to sit or find something sturdy to lean over on. Knowing THR can be a somewhat violent (sorry, I hate using that word) procedure according to which approach is taken, the dislocation of the joint, the sawing, the pounding concerns me. That’s a lot of pressure possibly being put on the lumbar region I would think. Of course I’ll be asking my neurosurgeon and orthopedic surgeon but thought asking you guys could give me an idea about it bf I do ask my surgeons. Thanks so much!

Thank you for all you provide. I am experiencing left hip pain, an x-ray showed bone on bone on both sides. He told me the right side is actually worse than the left side. I have no pain at all on the right side. This surgeon uses the anterior approach to replacement and told me he would make the left leg a little longer and then when I do the right side he would match up the leg length to the left side. I asked him what grade the x-ray was 1-4 he told me it was a 5 if there were such a thing. I am 66 and he explained I have the hips of an 85 year old person. Low dose ibuprofen 200-600 mg provides significant relief. Your opinion especially on the difference in leg length approach. Thank you!

Hi Elizabeth. CRPS after total hip replacement would be very rare. Patients can however have residual pain after surgery. It would depend on the character and location of the pain. Hope that helps.

Thanks for the question. Femoral neck fractures in young people can be a complicated problem and often will take a long time to heal. Sometimes when the fracture doesn't heal a second surgery can be done which loads the fracture site in a different way increasing its chances of healing. I am not familiar with Super Path. With respect to total hip replacements in young people, they are certainly good at relieving pain but the main issues are related to their finite life span and activity limitation when joint replacements are done in young people

Talking With Docs would a bone stimulator be a good option after 10 weeks. I feel we are just prolonging the replacement surgery. he currently has a plate and 3 pins. no pain just still no weight bearing and on crutches still. just need quality of life back. what are your thoughts?

A bone stimulator is certainly worth a try - the risk is primarily time and at a young age it would be better if you can avoid another operation acutely. Bone stimulators have some evidence to improve healing of fractures.

Talking With Docs I just hope this can still help the healing after 11 weeks post op. They will say that is a difficult place for the bone stimulator to be able to place it on just the right area. Is there anything diet wise or vitamin that would help? He is currently taking vitamin D and drinking more milk.

Identifying the location can be tricky due to the depth of the fracture however the beam of the ultrasound is typically triangular and has depth penetration to 10 inches in some circumstances. A balanced diet including calcium is a good idea. Vitamin D often requires supplementation (see video on Vitamin D). Weight bearing exercise is also a good idea if he has been cleared to weight bear. Hi Kat59. So when someone has a hip replacement the femoral head can be donated to the bone bank at some centers to be used in surgery that requires allograft bone. Hi Roman. Very good question. The question of which bearing surface is superior is a difficult one to answer. Historically ceramic on ceramic bearing surfaces had better wear properties than polyethylene however with time plastics have become much harder and their properties have improved. The debate is ongoing and probably is best answered by your treating surgeon. Both are good choices and have pros and cons. Hope that helps.

My hip replacement bread a total disaster. I had near a out a year ago. He x-rayed it and said it was fine. My whole top of my leg broke out in red bumps. I amstill have them. I had the lateral. I still can't walk. I limp and my hip gives out on me but my hip hurts. My or try ho surgeon is really done with me. The hip replacement was a bad surgery. I had my hip done one morning and went home the next day. I wasn't really ready to go home. I would never have another hip replacement again!!!

Hi there! Im 39 years old and I had my hip replacement about a year ago. I had a juvenille reumatoid arthritis when I was 11 years old.. after 7 years of physio and meds I was ok when I was about 18. Anyway, after a very active life, I was cycling, dancing, swimming a lot I almost forgot that I have arthritis. But he remembered me so he destroyed my hip. After this long story I would like to ask you guys. Im cycling and swimming again but Im worried about dancing. I dance again but Im very carefull and im scare to do moves. Like to sit on the floor when Im standing, or the deep squats..what do you reccomend? How easy to dislocate? P.S I had the robotic surgery and it was a success. No discrepancy, no pain only muscle pain sometimes especially at the beggining.. Thank you for your videos! X
