I learned more in this video than in like a year of looking at surgery sites and journals for that matter since this summarizes a lot of different techniques. Instead of hiding this from the patients.

this is a great explanation for this procedure, no BS, no marketing bs.....there is a doctor who trade marked the term "ponytail facelift" as if it was a special face surgery he invented....well as you can see in this great educational and honest video, that "ponytail" term crap is just marketing BS.

This helped tremendously as I will be traveling overseas for a face lift. Already found out one only does plication, that is how and why he is so inexpensive. That would be disastrous for my 65 year old face. And highly disappointing. Learned a lot with you. Thanks.

@@kstp7155 It went FANTASTIC. I tripped across the 'best' and cheaper than the rest. I found a group of flight attendants that had seen him. Plan to stay 10 days after surgery. I have seen long recoveries on USA youtubes. I was fine in 10 days. No idea why it would take longer. My friend had it all, and she also was fine in 10 days.

Question: If a rhytidectomy is meant to remove or reduce wrinkles, please explain why plastic surgeons are performing a facelift on patients that do not have wrinkles, such as 20 or 30-year-old patients? Based on the information. you have provided, would it be less invasive to just perform fat-transfer for patients that do not have wrinkles?

Luis, I suggest that you start with a search for board certified providers. Each country has a board and association that credentials surgeons (ABFPRS/AAFPRS or ABPS/ASPS in the US). From their website, you can search for providers in your location. Once you find a couple, go and meet with them, ask to see their results and listen to their recommendations. If you like them, have good rapport and communication with them, and their credentials suggest they’re qualified, then choose from your two best picks. Remember, when it comes to your face, you want to maximize the chance of getting it right the first time. Best of luck!

Correction: Not 100 years. Face-lift surgeries can be traced back to thousand plus years ago in ancient India. Charaka was the famous physician surgeon who recorded these operative procedure details, including tools used in such surgeries. Refer to Charaka Smrithi. These manuscripts found their way to Arab merchants who passed it along to ancient Greek and roman/ European practicioners. He is called the Father of Plastic surgery. Worth researching

This video demostrates why certain facelift procedures cost varies from one surgeon to the next,what I mean is you cannot have a decent facelift and then have coffee with your friends on the same day!!!In my view deep plane face lift can yield the best results

Angela Markopoyloy I agree with you on most of your points, but I would actually, myself, be a bit more nuanced and specific, and say that “facelifts employing a -deep plane of dissection- yield the best results” because, though there are several techniques that use the “deep plane of dissection” (e.g. deep plane, high-smas, composite, etc; I.e. extended SMAS techniques), I feel strongly that the High-SMAS technique is the most favorable technique due to its strong vertical vector. Even well-known advocates of the deep plane technique, like Dr. Jacono, favor a vertical vector, which makes it almost the same as the High-SMAS lift. Dr. Sam Hamra himself, who was the original creator of the deep plane facelift technique, abandoned it in favor of his “composite facelift” which, you probably guessed it, also uses a strong vertical vector of lift. Most surgeons like myself, who perform large numbers of facelifts day in and day out will feel this way. Thanks for your comment!

Nicely made video, but the literature doesn’t really support better or longer lasting result with more complicated SMAS or deep-plane techniques. It’s ironic that the masters of the procedure are frequently doing simpler procedures these days having recognized that more wasn’t better

Thank you; however, I don’t completely agree with your generalizing statement. Although SMAS techniques have been around for more than 50 years, there is a relative paucity of data adequately comparing the different techniques. The studies that do exist are inadequate at extrapolating differences in terms of longevity for a variety of reasons, including small sample size, inadequate follow-up, non-validated techniques that yet are used as being representative of the different surgical extents of surgery, to name a few. It’s a difficult area to build an intensive, quality study, especially when even, today, most surgeons can’t even agree on what to call a standard SMAS technique, extended technique, or even, comically, a mini lift. What most surgeons with expertise in facelifting do know, is that the cheek should be mobilized in patients with moderate midfacial aging (most 60+ patients). Although longevity differences between SMAS techniques haven’t been established, we do know that extended SMAS techniques have the ability to minimize issues such as the lateral sweep deformity, RSTL distortion, and incomplete jowl correction. This doesn’t mean that extended SMAS don’t also carry additional risks, mostly due to the more extensive dissection. It’s interesting you mention that most “masters” are doing simpler procedures; I’m not sure to which masters you’re referring. Dr. Hamra is a lifelong proponent of an extended dissection, and so is Dr. Barton as far as I’m aware. What you might be referring to is surgeons advertising “mini” techniques, which is entirely different. Even Dr. Jacono, an excellent deep plane facelift surgeon, has a “mini” technique, but it still employs a midface release and suspension that would certainly not be comparable to a simple MACS or S-lift. Going even further, the reality is exactly the opposite, almost all facelift surgeons with extensive experience and expertise in facelifting, throughout history, have eventually realized the importance of mobilizing the midface in a strong vertical vector. When you operate on a 65-75 year old face, you quickly realize that a “mini” technique becomes an absolute waste of time or a recipe for severe distortion.

@@AestheticMinutes Danny, when our specialty members are polled on this, most have drifted towards Dan Baker’s observations that more complex techniques with the SMAS add more clear risk for no clear benefit. There’s a strong preference for plication methods over complex smasectomy and deep plane dissections in 2020 when this is polled at meetings. VERY few people would still regularly do the Hamra or Barton style procedures anymore I’d suggest.

Great results but there is something very noticeable about the ears of someone who has had a facelift ... I don’t like the way the ears look pulled down ... maybe it is unavoidable or maybe it ‘loosens up’ over time but it’s a very distinctive indicator to me that a lift has been performed 🤷🏻♀️ just sayin’

Makeda’s Wrath I am part Israelite and Italian. I may lose my tan, but tan immediately. If pale as I am now it would be wise of me to stay out all day. As soon as I have a base it’s all good. Even those with very low melanin it’s still good to get a little light. We need it to make vitD3 so many have issues with their immune systems because of lack of D3. Sunlight is not all bad. It’s ones strength of their immune system that is the culprit of any cancer. Which is why making your own D3 is important.

That is the hard part about making educational content in a concise manner, you’re always bound to not mention some key figures, not because they weren’t influential, but because the need to properly recognize someone in this context rests solely upon a “first to publish ” basis. The reality is that the state of plastic surgery owes a great deal to many surgeons that are too numerous to mention, but I will try to do my best as I continue to create more content. On a personal note, Dr. Bruce Connell is one of my own idols of plastic surgery; his technical ability, results, and contributions were truly impactful. However, the content of this video focuses on a very summarized history of the facelift procedure, beginning with its inception, limited SMAS procedures, then extended SMAS. I could have mentioned Connell especially with regard to his platysmaplasty techniques and adaptations to facelifting, but in the end I chose to keep it short; you certainly can’t please everyone in this line of work.
