Deaths in Fairy Tail aren't very explicit and usually happen offscreen or caused by the enemy themselves. But I remember Simon being killed by Jellal, Natsu melting into dust like 1k of soldiers from Alvarez Empire at beginning of arc and Laxus killing the cyborg guy from Alvarez (i'm pretty sure Erza killed some people too along the anime, but isn't explicit).
