I've been out of the medical game for a bit, but when I was a Corpsman they were pushing TXA in an IV during massive bloodloss. I know they switched to combat blood transfusions which whole blood is the best treatment overall, but its possible to get TXA, and may be a good option to have for minuteman, prep, or as an adjunct depending on your scope of practice

@Vinnytwotime long term studies have shown receiving TXA has had a marginal increase in survivability and recovery when used before surgical intervention and post operation. It is less effective than blood products but hypervolumating blood cells to promote clotting while keep a higher BP and flow definitely does something on its own.

@@BlueJayWaters Not too sure for surgery, but I'm a medic and use to combat any coagulation compromise at the POI. Empirically giving it in trauma events does no real benefit for the pt. Learning ROLO, then giving it would be beneficial to your casualty

You mention not wanting to administer IV fluids in trauma. Current protocol in battlefield advanced trauma life support for catastrophic haemorrhage is permissive hypotension. Administer IV/IO fluids (colloid or crystaloid) in 250ml bolus to maintain a systolic BP >80 (use radial pulse as a guide - if they have one then it's over 80). This maintains adequate renal perfusion while not retarding clot formation/blowing formed clots.

ATLS protocol is colloid or crystalloid, not whole blood. Blood isn't carried by CMTs or most civilian prehospital providers. The point is to keep systolic BP >80. There isn't a significant dilution effect with non-blood products

@LFAnon Eventually, yes. In the prehospital care setting, though, you use other fluids to keep them alive long enough to get it. You do this by keeping the BP high enough to maintain renal and cardiac output to let what blood they have remaining perfuse effectively.
