It ripped his face off making him; mute, deaf and blind. The rest of his body is so heavily damaged that he loses both arms and both legs. He is quite literally locked into himself unable to communicate or interact with the outside world and unable to move at all. The ultimate isolation.

Did you notice anything about the drumming in this song? At some point, you hear Lars fire off 3 short, three long, and then three short salvos in rapid succession, spelling out "S.O.S." in Morse code. The soldier in this song is basically signaling that he wants to be put out of his misery, because he has nothing more to live for due to his horrific injuries and is only kept alive by life support. The official music video (which takes clips from the movie Johnny's Got His Gun) shows the man tapping his head against his bed post, because it's the only way he can still communicate.

Edit: On the aside, we would of never heard of Metallica had they not been so aggressive in their promotion. They literally started by word of mouth. They sold their album (ride the lightening) mostly at their concerts. One was on their 3rd album and I do believe their first 'video' because MTV, though very popular at the time, refused to play their music sighting them as being too controversial aka too metal. Only when their popularity with the public could no longer be denied did MTV conceded and start playing their stuff and even then it was regulated to after midnight rotation. To see them at the Grammy's was ground breaking for my generation.

The song itself is nearly a direct musical representation of the 1971 Anti War film Johnny Got His Gun. The music video and the studio track even takes snippets of the film's dialogue. The film is about a soldier getting injured so severely, as he lay in bed in the hospital, the nurses think they are in a coma but he's conscious the entire time but can't move, talk, or communicate in any way. It's a very dark movie, I'd even classify it as a horror movie.

The movie scenes and voiceovers (not of the band) from the official video are from the movie Johnny Got His Gun (1971). All he had left are his thoughts and memories from his childhood and the war and all he wants to do is end his life because he literally had nothing left. He couldn't walk, couldn't talk, couldn't even signal to the doctors around him that he was stuck inside his head.

The book and movie "Johnny Got His Gun" is absolutely the type of tale that nightmares become reality. This soldier jumps into his foxhole in WWI and a star shell explodes near him. All that he is is a living brain with the remains of a body (no limbs, face nothing). That's it. Yet he can still feel, and all he wants to do is die. I've read the book and watched the movie. Metal is a form of lyrical art that is best appreciated when you analyze and understand the meaning of each song. IMO it’s best to start with studio versions, as some live versions are messed with by the bands and if you aren’t familiar with the song or it’s meaning, it can loose some of its ability to convey its true meaning/definition of the song when it’s done more casually in a live setting. Watch the Official video, it was their first one even after putting out 3 albums ("One" being on the 4th alblum) and it was "One" that seemed to get them the mainstream access any band would want. From what I remember Metallica was part of the big 4 including Slayer, Megadeth and Anthrax, these bands were popular enough to lead or break open the way for other bands like them. This era of Metal was called Thrash Metal and incorporated long instrumentals many at the beginning of the song with Heavy guitars and drums and many switch ups that made it seem like a different song at times, topics were Death, War, Suicide, Politics, Religion, the Supernatural and basically any heavy dark or controversial topic was found here. These bands didn't get much respect by the mainstream or with music snobs, even though their fan base was pretty dam large. So Metallica getting to do venues like in this video were huge and it's why Metallica gained so much respect, they were able to break into completely different markets that shunned them just years before. It's why MTV"s Head Bangers Ball was such a hit back in the day and it also helped to get these types of bands more known. The heavy (thrash like) music you hear now days stems back to these bands and other lesser known ones and musicians in Metal today owe them at least a little reverence for all the hard work, pain & suffering just as The Big 4 bands owe everything to the bands before them like Black Sabbath, Iron Maiden, Rainbow and others who were all in pieces/parts influencing a new sound that was coming called Thrash Metal and of course now there are so may categories of Metal that its hard to distinguish but they all have their specific elements. Just a little FYI from my memory, it's not Gospel but it gives you something to reference if you get curious.

You miss so much on this version. Check out the video version incorporating scenes from the 1971 movie Johnny Got His Gun with Timothy Bottoms. The song if from the POV of a young man injured in the World War when a bomb dropped on him, taking his arms, legs, and face. He has no way to communicate with anyone at the hospital, left with no voice and only his memories, thoughts, hallucinations, losing the ability to tell one from the other. It's a heart wrenching story, both the video and the movie it took the visuals from.
