I distinctly remember the whole story too... Disturbingly similar to the 1949 Superga disaster, in which the whole Torino FC squad perished, and which has practically blown out the team from the highest echelons of the sport (in that case too the other teams agreed on giving them the Title, Torino FC heading the Italian Championship with a large gap), those old enough to remember that other tragedy, and those knowing it too, immediately started drawing the comparison. A friendship between the fanbases of the two teams have also been born, and on the first anniversary of the Chapecoense tragedy Torino FC players went on field dressing a one-off green shirt reminescent of Chapecoense's official social colours.

The airline was already going through financial trouble. The captain was a part owner of the airline... Stopping to refuel would cost extra so he basically chose not too. Also, declaring an emergency would call attention to the fact that they knowingly disregarded the fuel requirements to destination and alternate, so they chose to go on. Even as the engines started flaming out, in spanish, they are say to the control: "a fuel problem has arisen" and when all go out, they say "an electrical problem came up".... meaning they still hoped to land with no engines and sweep they problem under the rug. Another flight had declared an emergency earlier which is why they were put in a holding pattern. With almost no fuel, thy chose to hold nevertheless rather than admit they had a problem - and admit they had basically planned the flight with that fuel range.

@olivergrumitt80334 жыл бұрын
The same thing happened on the Challenger flight that exploded shortly after launch. There was a fault with the O rings that engineers were aware of and warned managers but managers decided that although O ring erosion had been observed on previous flight, no disaster had occurred and so it was all right to continue flights. Essentially, managers were playing Russian roulette and sooner or later the bullet would be in the firing chamber. The 25th Shuttle flight, the 10th of Challenger was the flight which had the bullet in the chamber. This complacent approach to a potentially catastrophic problem became to be known as the Normalization of Deviance, which occurred again with the second fatal Shuttle flight, of Columbia, in 2003. Aviation and spaceflight are not tolerant of carelessness and such complacent attitudes must never occur again.

A few extra details: 1. The pilot was also one of LaMia’s co-owners. He was well aware of the airline’s poor financial condition, that’s probably why he adopted this conducts about flight planning and management. 2. He knew he would be fined if he landed anywhere flamed-out. That factor probably explains why the emergency declaration was made so late. 3. How bolivian authorities approved this flight plan is beyond me. That aircraft should never be allowed to take off with such little fuel to spare. 4. About the survivors: 🇧🇴crew technician Erwin Tumiri is studying to become a commercial pilot 🇧🇴 stewardess Ximena Suárez became an air traffic agent 🇧🇷 journalist Rafael Henzel resumed his career until dying in march 26, 2019, by a heart attack while playing football with friends 🇧🇷 backup goal-keeper Jakson Ragnar Follmann lost a leg in the crash, effectively ending his footballing career. He is pursuing a career as a singer, winning a well known Brazilian singing reality show recently. 🇧🇷 defender Neto retired from football wihout being able to resume his footballing career due to chronic pain from his injuries. He belongs now to Chapecoense’s staff. 🇧🇷 Wing back Alan Ruschel resumed his footballing career. He returned to Chapecoense after a loan to Goiás (a Brazilian first division, mid-tier club). About Chapecoense’s fate: Chape is a very small club that was very well run by the board of directors that perished in the crash. They never got back to the playing level they had before the accident. In the aftermath of the crash, several clubs suggested a three year period while Chapecoense would be free of being relegated, even if finishing as one of the four last in the league. Chapecoense, maybe under pressure by the Brazilian Football Confederation, refused. Last year, which would be the last year of immunity, Chape finished second to last and got relegated to Brazil’s second division. It will be really hard to them, as a tiny club, to get back to relevance after the disaster and the personal and financial fallout from the crash.

I’m calling bullshit, that’s absolutely backward. That sort of culture would encourage people not to divert for fuel and to press on regardless, thus crashing. Nothing wrong at all with diverting for fuel, most people have done it a least once if not more times in their career. Increased holding, headwinds or go arounds can all make aircraft shorter of fuel. To sack someone for making a safe and pragmatic decision to divert because they’re low on fuel is so ridiculous it’s clearly not the case.

Some major incompetence going on here. Flight logs revealed that the pilots basically allowed this to happen more than once. Majority of their flights were under fueled, and I believe a flight that took place shortly before this one, ran out of fuel as well, but the pilots were able to glide it down and land safely. Multiple situations and the pilots never thought “hmmm, maybe something is wrong with our fuel calculations?” Imagine being okay with this much stress for every single flight.

1) 10 minutes before landing (and with 20 minutes of gas in the tank) everything looked fine for this pilot coming from a South direction. I believe he was third in sequence for landing (LATAM and AV ahead of him). No delays, no particular weather on his route. 2) Out of the blue, VivaColombia (flying from BOG to ADZ) declares an emergency and request landing at MDE. 3) ATC quickly clears Viva for landing and puts everyone else in a hold. 4) The Lamia pilot has two choices: a) declare an emergency himself or b) hope the VivaColombia lands quickly. Even though the guy sounds quite distressed he goes for b) mainly because MDE will fine the airline (I think it is 25K) for a preventable fuel emergency landing. This new charter airline and the pilot might further get their licenses revoked to operate in Colombia if they are found to be operating recklessly -which is very likely-. 5) Viva faces some weather landing (he is coming from the North) and its landing takes an extra 8 minutes to complete. That emergency was due to a faulty fuel indicator. 6) By the time ATC goes back to the aircraft waiting on hold the Lamia plane is doomed. He declares emergency way too late. We know how that ended. 7) MORE IMPORTANTLY: both Avianca and LATAM planes are then diverted to an alternate airport while ATC tries to sort out what happened to Lamia. Those two planes, sequence 1 and 2 for landing with the MDE airport literally in sight AND good weather, spent over an extra hour in the air while the Viva and Lamia emergencies played out (and to take them to the alternate). Lesson: you do need extra fuel for both 1) delays not related to weather and 2) fuel to alternate.... even when everything looks perfect 3 minutes from landing with the airport in sight. avherald.com/h?article=4a16583c/0017&opt=0 (Comment by ColPilot on Thursday, Dec 1st 2016 16:34Z)

I only have about 100 hrs real flying time but 10's of thousands of hours on flight sim games and practiced trying to make a runway many times. More simulator time would have given the pilots numerous emergency situations, basically to train pilots to remain calm and fly the plane. They didn't have much time to think about raising the gear and flaps. After the first flame out. First thing after flame out, trim for gliding.

The captain said right before the take off at one soccer team players stories "They are flying with the best" And the voice at CVR is the Captain, and they where on hold due to one Airbus A320 in Mayday due to toillet problems, if the Lamia flight declared emergency... They had the priority... never happened. Sad.

Among the passengers we have a lot of press members, including the local narrator Rafael Henzel. Henzel did survive the crash, and came back to a Chapecoense game in 2018. Sadly he passed out in late 2019 because of a heart disease.

Wow. This was gross incompetence by the captain and first officer that bordered upon being criminal. That incompetence killed nearly everyone on board.
@stanislavkostarnov21574 жыл бұрын
I am sure if those Pilots had survived & at all intact they WOULD be facing criminal charges... as it is, I would like there instructors to answer a lot of questions... probably they did/are... I mean, if these pilots were not morons in the direct meaning of the word (and that means we are saying that all the involved pilots have well bellow average intellect), the only thing that can explain a moderately sensible, sane pilot doing this is very poor training or a hellish amount of pressure to get there (Like in the case of President Kaczyński's plane crash). whichever it is, someone outside that plane (or at least that cockpit) is responsible.

There were several factors discovered later on: - The totally unprofessional crew, leaving with a flight plan that hadn't enough fuel. That is no miscalculation, THEY KNEW IT. - Once in flight, they choose not to divert to refuel, even knowing there was a big chance of not making it. - They didn't notified ATC about the fuel warning until it was too late. - Didn't took corrective action to make it to the runway and slowed too soon. - Bolivian aviation authorities knew about regular LaMia's short on fuel flight plans, but we're corrupt and shut up for money. - CONMEBOL, the south american football association that organizes the tournament, urged teams to contract LaMia charter flights. Several teams used, even when it meaned larger flights like this one, at no better price that a direct flight contracted with any other company. Corruption once again.

There is no getting around the fact that this tragedy happened because of the pilots careless disregard for the lives and safety of their passengers and apparently did so on more than one occasion and this time it ended in tragically. They knew they had a fuel problem yet did not divert and get fuel when they could and should have yet. Well done video Allec.

These have to be the two most incompetent unprofessional pilots I have ever heard off. The people behind that cockpit door sitting in those rows of seats place their lives in the hands of the flight crew. This accident makes me so mad, their arrogance and unprofessionalism took the lives of those poor passengers, the young rookie pilot in the jumpseat and all the cabin crew because of sheer incompetence. You can hear the stress entering the voice of the controller as she began to realize the seriousness of it all.

What if they shut 2 engines when they saw the master alarm indicating that they have only 20 min of fuel remaining??

First they risk running out of fuel and then they act like nothing is happening even as the engines start flaming out. It's truly sickening how so many lives were lost due to stupidity and negligence.

Such a shame, and a waste. Also, one of my favorite aircraft for its uniqueness. I got to fly on one while it was in service for an American airline as a regional (IIRC) aircraft, and I just thought it the coolest little plane with its four high-mounted engines. I figured that it may have been originally designed for military use, but it seems that it was designed for short take offs.

"The flight late departure is due to a Chapecoense team member's request to have a videogame retrieved from his luggage in the cargo hold prior to departure". A capricious child (or star) mindset. "I want my Nintendo Switch. Right now. Otherwise I'm going on a starvation strike"... At the end, we had a starvation strike from the plane
@bjr45673 жыл бұрын
Still, considering all the errors made by the crew, both before and then during the flight, it's a wholly inconsequential issue. As it were, I'm not sure ANY flight crew could screw things up more thoroughly and impressively than this one managed to do.

SOOOOOOOOOO many errors made by the pilots and ATC. Neither took the situation serious. Amazing how humans can be that negligent when dealing with many lives. ATC clearly doesn't understand the definition of EMERGENCY.

@IanTaylor_14 жыл бұрын
Mizzy Roro true. However, when an air traffic controller hears a pilot say they have a fuel problem then they need to take it seriously. I was an air traffic controller for the US Air Force and I would’ve asked the pilot if they’re declaring an emergency. Very simple. Gear down and flaps configuration was enough to kill them all. Your next pilot could be that stupid.

It's not mention here, but one of the investigators theorized the captain had pulled the CVR's circuit breaker on purpose (as pulling it practically makes no noise) in order to prevent further conversations about the fuel situation being recorded.
