Finally hearing why I have prostate pain. Since my 40's (now 71) I have had prostate pain that periodically can get so intense it takes my breath away with cold sweat. No one ever mentioned prostatitis, only BPH. I recently told the doctor urinating in the mornings was becoming increasingly harder and was advised to increase the proscar from 2.5 mg to 5 mg. Over the years I have known men (including my brother) that had prostate cancer, and I always asked if they had pain, and everyone said no. I was puzzled, but now I know. Dear Dr. Brewer, I am a 74-year-old retired engineer. For several years I have had the problems and the urge to use the bathroom and it was making me crazy. Thanks to Cialis, all my problems are gone now. I can not agree enough with what you are saying. ONE IMPORTANT POINT in my case is that 5 mg of Cialis was not enough. Thanks to my Urologist. I am taking 10 mg cialis, zinc, and saw palmetto daily. At 70 my urinary comfort is much better than the 10-15 years before I started that regimen. I had been prescribed flomax to help reduce my tendency to have kidney stones by keeping the liquids flowing. But flowmax caused discomfort in sex (backflow) and also urinary urgency consistently. I have stopped the flowmax and the cialis, zinc, and saw palmetto combination have provided essentially "pre-old man" urinary comfort, while I am still moving a couple of liters of urine a day. I knew the role role of cialis in sexual health, but did not know the role it was playing in improving my urinary health.

As a retired country doctor I have prescribed these drugs you mention over many years. In dealing with my own hypertension issue in recent years I have settled on a Calcium Channel blocker daily and Tadalafil10 mg twice weekly. Tadalafil has a longer half life than the blue pill. My BP, which was labile, has become acceptably stable. The Tadalafil is taken in the AM on Sunday and again at Noon on Wednesday. This means dosing is on an 84 hour cycle, evening out the problem with an odd number of days in the week. Dr. O

60 yrs with mild BPH and diabetes. It gives me terrible lower back pain for 2-3 days. Plus, the effects down below aren't strong enough like sildenfil (which gives me stuffy nose for a few hours ... but I can tolerate that). Have tried it about 5-6 times over the years with consistent symptoms of back pain ... eventually gave up on Cialis.

5 mg Cialis has been extremely helpful with my BPH/LUTS/Chronic Prostatitis/Pelvic Pain. Initially tried Tamsulosin, but didn't like it (can't remember why, sorry). I also tried Proscar, but like 30-50% of men, stopped after a month with sore breasts and total loss of libido (yikes!). Also tried a Eurolift which stopped helping within a year. The reality seems to be that if you have an enlarged Prostate, maybe one should just bite the bullet up front and have a procedure done that generally relieves the issues long term. A TURP is currently the gold standard of care/treatment, but I think Aquablation may be even better and will eventually overtake/replace TURP. Problem I have is that now I have a Gleason 7 (3/4) and it's recommended that I treat that as well. A lot of abuse for one prostate. It's hell getting old. LOL

It isn't men that ignore men's health issues. Female Dr.'s and their PA's do as well. It takes 3 months to get an appointment (out her in the rural area where I reside)and yet questions or exams regarding such issues are not even asked. My experience...5mg daily dose had annoying side effects; mainly muscle pain in lower back. But going to 5mg twice a week proved to be beneficial. Far fewer night calls and much easier to get flow started. Efficacy of 5mg Cialis dose seems to be about 3 days. Added bonus, firmer erections.

Such DRY humour. We're up for that! Thanks, Doc. Cialis (Tamsulosin) + Flomax (Tadalafil) = trouble Dr. Brewer notes this is a bad combination. Both drop blood pressure. Together, precipitously. A doctor in Emerg prescribed both, together, to treat Edema (swelling in my legs & feet). Result was severe blood pressure drops (mine already tends to run low), I passed out & hit the floor. I wish the Emerg MD had seen Dr. Brewer's vids - or read the label. I could stand fine with either one. Together, I couldn't keep it up. Couldn't keep ME up.

Tamulosin has been very helpful for me. It took away all the nighttime wakes to get to the restroom. It hasn’t helped with the slow flow and it is actually easier to empty my bladder by taking the “alternate” route of sitting instead of standing. It takes longer but it takes away a lot of “leaking” after voiding. Weird to be talking about this at my age-69.

I used to get up multiple times a night about 15 years ago. The doctor I had at the time couldn’t help so I googled my symptoms and found out about over active bladder syndrome. I held my pee for as long as I could before going for about 3 months to stretch my bladder. I sometimes sleep through the night now but usually get up once. My quality of life improved dramatically.

*DON'T GET UP IN THE NIGHT - Just roll over and pee in the bedside urinal. I've done it since the nurse gently pulled out the urinary catheter inserted before my primary TKR/TKA in Stockholm, Sweden.Unfortunately followed by a post op infection.Two stage revision here in Bangkok Thailand where I am a resident. Don't keep running to the bathroom at night!*

I was reluctant to comment, but I was diagnosed with prostate cancer 18 months ago. I changed a lot of things in my life, I work out 4 days a week, no red or processed meat, improved my sleep habits, drink less alcohol, eat healthier, lowered my cortisol levels and so on, but I also started taking 5mg on Cialis each day which lead to a lot more ejaculations. My 1 year follow up biopsy found no cancer. I asked the Dr if it's gone and he said no, but it's gotten smaller. Please do not take this comment as medical advice for prostate cancer and keep on top of your dr advice. But I feel like Cialis was part of what helped me.

As a prostate cancer survivor I strongly recommend annual physicals with PSA testing starting at age 50, something I wouldn’t have done if my wife wasn’t an MD who ‘strongly recommend’ it. In my case my PSA rose slowing to 3.2 then between ages 67 and 68 doubled which I discovered was the point insurance will pay to for the MRI, MRI assisted biopsy, PET scan to rule out bone cancer, etc. In my case the tumor was very aggressive with high Gleason score and I opted for robotic surgical removal three months after the elevated PSA test. Had I waited another year I might not be writing this. A year after surgery PSA rose to .2 which post surgery is a indicator for reoccurrence. Fortunately a new diagnostic PET scan tracer Plarify which an spot prostrate cancer down the cellular level was approved and the source of the elevated PSA was determined to be a lymph node that had not been removed during surgery. Getting rid of the cancer in it required two years of Lupron hormone therapy and 38 daily radiation treatments. Something learned on this journey is that Testosterone promotes prostrate cancer growth. The Lupon they put me on prior to and after the radiation drops the T level to zero which caused loss of muscle mass, energy level and desire to tackle projects in addition to eliminating any desire for sex. Fortunately three months after getting off the Lupron muscle mass is increasing and energy level is back to normal and so is 👍 with the help of Cialis. 😂

My Urologist has me on a combination of 5 mg Cialis, .4 mg Tamsulosin, and 15mg Meloxicam for BPH. He keeps telling me I have Prostatitis, the last DRE I had sent me over the table and I peed blood for a day. Sorry TMI. I also have low-grade Prostate cancer. That combo of meds has helped with nocturia but still getting up on average once per. It has improved from 3 times per. Been on this combo for more than 90 days. Not to mention other things I do to combat inflammation. No alcohol, eat as clean as possible, turmeric and cinnamon in coffee, no sugar, etc. Also, my symptoms improved when I stopped eating and drinking water around 6 or 7 PM and fasted until 10 or 11 the next morning. I am going to try cold plunging for the next 30 days. May still have to opt for the HIFU procedure.

Ask about HIFU to treat your prostate cancer. It will shrink your prostate improving urine flow and treat the cancer. Medicare recognizes this treatment since 2023. BTW Turmeric combined with black pepper can be hard on your liver and can amplify medications leading to overdosing of prescription meds.

Thanks Dr. Brewer! Very good summary of the benefits of cialis/tadalafil! I was a little concerned taking 5mg/day with 25mg/metoprolol for palpitations. Never had any issues with low blood pressure, and it seems, for most, the risk is very low. Also, it has helped a lot with LUTS, due to an enlarged prostate. Do you recommend a course of antibiotics when starting low dose cialis to help with potential chronic prostatitis?

Im 31 years old and have been diagnosed with an inflammed prostate. The gp said its extremely rare for someone my age to have this and the only medication he was able to treat me with would cause lose of libido and decrease erectile function? Have been put on a category 3 waiting list to be examined but was hopeing lifestyle and complete diet changes would have helped me with regular excercise but still have symptoms 😡

I’ve had the same experiences as Dr. Brewer. BPH symptoms started at 50. When I got to the right doctor we began the treatment process trying three different alpha blockers like Flomax. All three created hypotension. After my research and discussion with the doc we decided on Cialis 2.5mg and bumped it up to 5mg. No side effects but I do suspect a slight improvement in lowering my blood pressure to a range of ~110/65.

Alpha blockers do not create hypertension. In the contrary they will lower it, as they widen the blood vessels. I take alpha blocker for high BP, and it works great. Looks like you need to change your doctor.

Tamsulosin for BPH was a disaster for libido (at 80). Cialis 5mg daily has proven fantastic. 'Ever ready' but sort of settled at twice pw. and 20mg occasionally. for 'wedding anniversary' type events that require concentrated enthusiasm. 'Waking friend' returned. Mental acuity improved. Sport competitiveness improved. Often sleep through without having to get up. Occasional indigestion the only obvious downside. This stuff works, so pre-agreement with wife essential, I advise. Otherwise I'm sure a one-sided view of life could lead to discord.

I take 500 mg of L-Arginine once daily for lowering my blood pressure as it helps create more nitric oxide opening blood vessels. It also gives me better blood flow when weight training, skiing and backpacking at altitudes between 7,00 feet and 11,00 feet. BTW, I'm 81. (yep, eighty one)

It works for LUTS and high BP. Cost and availabity through insurance can be a problem. It is now legal in the US to order a 90 supply from Canada. A legit company will require a prescription from your Dr.

This has many benefits for me as a 27 year old man, I have kidney stones and this helps relax the ureter and increases urine flow making it easier for me to pass any stones, and helps completely emoter my bladder so that I don’t have a build up of urine in the bladder (which can also create kidney stones) Additionally I used to wake up once every night to go pee, since starting Cialis I don’t anymore. I also do PE (Penile Exercise) and this helps with blood flow and makes it easier to get and keep erections, truly think every man should start taking this at around 25 years old.

Now 74, in excellent physical condition for my age, I have tried Cialis on and off many times from 5 mg to 20 mg. Unfortunately the side effects of muscle and lower back aches, and heartburn from hell eventually show up. Although the benefits last longer, so do unwanted sides.

Although I noticed positive effects each day using it, Tadalafil 5 mg gave me headaches, severe enough that I had to discontinue. Cost wasn't so much the issue, as the L-Citrulline I use everyday goes from 10 to 20 gm a day and is equally expensive.

My Dad, newly widowed, aged 79, wanted to remarry. In hopes of becoming, er, more youthful, to please a new wife, he went to a doctor and got on blood pressure medication and cholesterol medication. He told me all about this. A few months later, he told me he had been to ANOTHER doctor and obtained more medication. A few months after this, he had been having bad reactions to his cholesterol med, and his doctor changed to a different one. Not long after that, his girl friend (about his own age) called me and told me that he had had a terrible allergic reaction to his meds, his airway swelled up and he was having trouble breathing. She took him to the local clinic/hospital, where they performed an emergency tracheotomy. They thought he was stable. His girlfriend left to feed the animals on his little farm, and he just slipped away. Later my brother and I found a bottle of Cialis with his other meds. That’s why he went to a second doctor, because the other doctor wouldn’t give it to him. Sometimes Cialis does not play nicely with other meds. Dad died at 80. His younger brother lived to 95. You have been warned!

80 years old is a great age to die. (My father died at 76, mother at 73). I'm sure that your father was informed about the risks of taking Cialis, but he decided to take it. So yes, your father died earlier, but probably much happier than his brother. May both rest in peace.

I took your advice, because I suffer from "Nocturnal-Poly-Urea". Or would it be premature to say I used to suffer this common malady that aflict a lotta men my age (75), or so I am told. I did not discuss this fact with my friends. It is often accompanied by an enlarged prostate and the only remedy that medical science has offered is surgery. I still take supplements, without which it may have become worse. Still, I used to wake up a half dozen times per night and was never able to completely empty the bladder until the 5th or 6th attempt. I am happy to say that I can now use the past tense thanks to Ta..da... lafil that is! In just 2 months, I am down to one trip to the toilet and the rest of the time I am able to sleep soundly. Eating some pomegranate also helped. Both of them lower blood pressure as well, in ways no other BP med could. And, both help with Nitric Oxide production. Thank you Dr. Brewer for a very detailed and thorough presentation. I have forwarded a link to this to my friends, perhaps at some risk to my reputation, eh? 😬

I was involved in a clinical trial for BPH, using Cialis at least 10 years ago. It was a successful trial, and I have been taking 5mg since the trial concluded. No side effects.
