😢 Here is one to add to your list. On October 12, 1984, while on the set of the film, "Golden Opportunity", a young actor named Jon-Erik Hexum was bored with all the delays so he picked up a .44 Magnum prop gun filled with blanks and jokingly shot himself in the head. Despite being rushed into emergency surgery, they could not save him, but he did save the lives of many others because he was an organ donor. He was only 26 and his death serves to remind us, as does Brandon Lee's, that prop guns, can be just as dangerous as regular firearms and should respected as such. RIP Erik and Brandon ❤

Jon-Erik Hexum was working on a TV series called “Cover Up” , not a film. I will never forget that day. I had just started working in the industry. Our agency had an actor working on the episode they were shooting. Erie, hearing the story from someone actually on set that tragic day 😢

Two other tragic accidents on set that I didn’t see mentioned. First was in 1965, veteran stunt pilot Paul Mantz lost his life in a crash during the filming of Flight of the Phoenix, starring Jimmy Stewart. Also while filming the Vin Diesel film, XxX (2002?), veteran stunt man and former Navy Seal, Harry O’Conner, died performing a paragliding stunt for the movie. He collided with the pillar of a bridge and broke his neck.

Don't forget Art Scholl, a famous aerobatic pilot, who died in his aircraft filming a scene for Top Gun in 1985. From Wikipedia, "Scholl died during the filming of Top Gun when his Pitts S-2 camera plane failed to recover from a spin and plunged into the Pacific Ocean. He had entered the spin intentionally in order to capture it on film using on-board cameras. Observers watched the plane continue to spin as it descended past the planned recovery altitude. Scholl's last words over the radio were "I have a problem‚ I have a real problem,” after which the plane impacted the ocean about five miles off the coast, near Carlsbad, California. The exact cause of the crash was never determined. Neither the aircraft nor Scholl's body were ever recovered."

You forgot the head on collision on the set of The Cannonball Run in 1980 that left stuntwoman Heidi Von Beltz a quadriplegic. The Aston Martin DB5 she was riding in collided with a van while filming the scene where the car is filled with smoke.

Gone In 60 Seconds Part 2 from Wikipedia. On June 9, 1989, Toby and Denice Halicki began to shoot the sequel Gone in 60 Seconds 2, which would star them both. The sequel would not have the same storyline of 1974 film; Toby Halicki wanted a bigger story about a professional international thief. He bought over 400 cars to destroy for the film. On August 20, while filming in Dunkirk and Buffalo, New York, Halicki was preparing for a dramatic stunt sequence in the film, during which a 160 ft. (49m) tall water tower was supposed to topple to the ground. When a cable attached to the tower snapped unexpectedly, it sheared off a telephone pole, which fell on Halicki, killing him instantly.

His life was spared because of a Kevlar vest worn underneath his shirt, kudos to the smart stunt coordinator who wasn't taking any chances, and realized only full well the kind of accidents that might occur on the set.... And kudos to the star who decided to wear it because not all stars bother to listen to the stunt coordinators and think they can do everything the way they want to.

You mentioned the Alec Baldwin in another video, but never mentioned Kevin Smith of Xena WP life was tragically cut short shortly after Xena had ended. He was on a high rise and fell off during filming or that Lucy Lawless had a freak accident while on the Leno show with a horse and broke her pelvis among other injuries. So it must be the shows you know that get mentioned. There isn't a mention of that actor who was killed by his car rolling down the driveway and crushing him against a wall or gate.

When Alec Baldwin got charged after the incident I thought it was just an accident but the more I think about it, you never fire a gun at someone even if you think there's a blank in the breech. Culpable homicide.

First thing, the US doesn’t have Culpable homicide. We have manslaughter, either intentional or unintentional. As for never firing a gun at someone, that isn’t true when it comes to the entertainment business. Actors often fire guns at others or in their direction while making films/TV shows, it isn’t uncommon at all. All actors are under the impression that live ammo isn’t allowed on set and that the firearm they are being handed is either unloaded or has blanks in it. For a person to be guilty of unintentional manslaughter, they must have committed an act that a reasonable person would know could be fatal. And again, a reasonable person wouldn’t believe that live rounds would be in a gun used in a movie, or even be on the set. As much as I dislike Baldwin, he did nothing that would be considered unusual or that he believed could be fatal. Therefore he isn’t guilty of manslaughter in any degree. The lady I charged of the firearms was found guilty because her job is to make sure that all firearms are safe to use and not loaded with live rounds. Baldwin was one of the directors on the film and does have some responsibility for the safety of everyone on the set. He could be, and most likely will be, found guilty in a civil suit for wrongful death. But nothing he did that day was criminal. There is no law that says an actor has to check a firearm before using it on set, although movie studios should have that as standard practice. And there isn’t any law that says live ammo cannot be on set, although that should be a law. There should have been an investigation into who brought the live ammo onto the set. It was reported that some of the cast and crew would shot some of the guns in their spare time and that is most likely where the live ammo came from. Whoever brought that ammo could also be found guilty in a civil suit, but not in a criminal court. So again, Baldwin didn’t break any laws but as the top production personnel on site he was “responsible” for everything on set. But since what he did wouldn’t have seemed dangerous by any other actor or person in the film industry, he isn’t guilty of involuntary manslaughter.

These are interesting and sad.So sorry for all involved. I think one that shocked me was the first. The fact that the famous Cecil B Demil’s name isn’t muddied and soiled by the fact that HE LITERALLY DID MAKE the mistake of telling someone to use live rounds in a gun on set. And even though they switched to blanks, a live round was still in the area and got in the gun. The point, De Mill was the responsible one but the world didn’t try to destroy his life and career. (I know time were different, but the point is the same). OBVIOUSLY I’m referring to the way Alec Baldwin was treated after the ACCIDENTAL shooting on the Set of the movie “Rust”.
