I just wanted to say that I really appreciate these videos. Historically there hasn't been a good way (that I knew of, at least) to follow developments in the diabetes tech world. Hearing about these, especially from a T1 in the medical world has been a great development, so thank you.

I think for a quick rundown you did a fantastic job of all four CGMs All of them have some good aspects to them I've used all them but the Medtronic and my choice hands down is the Eversense E3 it only requires now once a day and the next iteration in two or three months from now will be once a week and last awhile year and every CGM whether they say they need calibrations or not all diabetics still use a finger stick if not daily at least minimum several times a week just to make sure the CGM is right

1:57 Just a minor correction, but the guardian sensors (including the 4th) are not guaranteed to last up to 7 days. They're supposed to last up to 6-to-7 days according to their technical support when I had phoned in. However, from my own experience, they only lasted up to 4-to-5 days and always gave major inaccurate readings (sometimes *hundreds* of points in difference), sometimes refusing calibrations and not allowing me to give myself insulin at times due to said attempts at calibration (first attempt it has you wait 15 minutes before attempting another calibration, however if it doesn't like that reading neither it makes you wait 2-to-3 *hours* before you can attempt *again*, wasting test strips). The final straw was the terrible experience with technical support not understanding that the machine nearly overdosed me twice with the pump due to the sensor's constant inaccuracies. Their "6-day algorithm" is massively flawed. Months ago, I had spoken with my endo and I'm much happier on the DexCom G7. Never going back to Medtronic's technologies.

I’ve had a guardian for years and as time went on and more scar tissue came it became more and more uncomfortable in my belly. Plus the tape and injection was a bit more difficult on my arm. Trying the Libre 3 now and so far I’m loving it.

Might be useful to see how these cgms work with pumps as most likely a lot of us would consider the combination if they are able to work together.

Good point! So as far as pump compatibility, the Dexcom G7 is going to be compatible with the Omnipod 5 and Tandem Tslim. The guardian 4 is only compatible with Medtronic pumps, the Freestyle Libre 3 as of today is not compatible with any pumps, although that will change in the near future, I know they already have plans to integrate with the Ypsomed pump, and finally the Eversense which as of today is not compatible with any pumps, but I’m sure that will change as well. I would have included this info in the video, but I didn’t want it to run too long. Thanks!

Medtronic Guardian has the worst accuracy Thankfully I have learned to not rely on the Medtronic sensor; after years of damage. Very frustrating when the sensor reading is 140, and the actual finger stick reading is 280. Or the very common reading of below 40, when the actual blood reading is 90. I should have died countless times from the inaccurate sensor readings from medtronic. Dexcom G4 from 12 years ago was much more accurate than the Medtronic sensor that is available today

As I recall, Dexcom G7 was to be released this past spring/early summer. I see it has been approved in Europe but haven't heard anything about the G7. Anyone hear anything on the G7 release date?

Unfortunately it got pushed back again. Some sources saying beginning of next year, some saying end of this year. I’m hoping sooner than later, I’ve been anxiously awaiting its release.

Pointless your accuracy unless you test it yourself no point in taking their word for it. I found CGM'S inaccurate out side middle range they can zoom up or down once they are triggered by a movement in levels. I monitored all mine with a finger blood tests so even allowing for delay or slight difference I took readings for long periods when they spiked and my levels where in normal range while CGM showed I had Pre-Diabetic levels. The only accurate way is with finger prick blood test and my consultant said they were not accurate as well, it would be a God send if they were.

Request from a person with migraines and borderline diabetic I know freestyle 3 has it where you only scan when 1st apply so don't have to have alerts loud other then to treat the blood sugar do the others also do the scan once thing or are they like freestyle 2 where you need to scan for it to record the info (gonna check other vids from him after posting just asking to see if someone knows)

I had tried the guardian 3 sensor and had an incredibly bad experience with it. I have phoned Medtronic for help on a weekly basis and they send me replacements several times. My HbA1C was a staggering 8.6 on the guardian, until I got a low glucose alarm of 2.8 and realized that my glucose did not feel like 2.8. I did a fingertip test and the fingertip test showed 13.8. So that was why my HbA1C was so high. Then I turned to the Libre 2 and surprisingly enough the Libre 2 shows a 0.4 difference to a fingertip test and my HbA1C is 6.4. I will definitely not even consider guardian ever again and as soon as the Libre 3 becomes available in Canada, that is where I would go.

My experience with the Libre 3 was fantastic when I started using it in Feb. but in May I started having lost signal and a very wide difference between fingerstick reading and the cgm reading. The cgm would bottom out and then flat line. 8 failures in a row. I don't know if my body chemistry has changed or possibly the fact that we are close to solar maximum (bluetooth is a radio frequency and solar storms can knock them out) or something else. Temperature extremes might also account for some of it. It sits in the black mailbox in the Texas heat until someone checks the mail. I am going to start using the Dexcom G7 to see how they compare. I figure if both have problems then it must be me.

Don't the non calibrating cgms you listed require a puncture or finger stick every time you replace them? For example, the G7 would stick you at least every 10 days? How do all the devices actually work?

Nope, the only CGM that requires a finger stick is the Eversense. The other cgms do not require a finger stick. If you mean puncturing the skin itself to place the sensor, then yes, all cgms require a very small needle the size of a lancet the apply the sensor on day one. With the Medtronic Guardian 4, there is still an initial calibration required before going into the Smart Guard closed loop mode. Definitely nothing like the Eversense CGM but still not quite as good as Dexcom or FreeStyle.

Currently the Guardian 4 has been amazing much better than the Guardian 3. Major difference is wearing the 4 on my arm! Makes me so happy! Thank you for this comparison!

@@72Marcie no I am on the 780g pump. Eversense isn’t compatible with a pump otherwise I would have stayed. Medtronic is awesome too though.

Great video. However, it did not include an important issue - cost. Given that the two apparent leaders (Dexcom G7 and Libre 3) are not yet readily available in the US, this may be a bit difficult to determine, but… A big issue is: what is the non-insurance cost? From my research, based on the prior versions of the Dexcom and Libre, it appears that the Dexcom is something like three times the cost of the Libre. Is this correct? Any good sources? Thanks, Dan.

Thanks! Cost is definitely a big factor. Historically you are correct, Dexcom has been significantly more expensive than the Libre. The G7 has been rumored to be a bit more affordable though now that it does not require a separate transmitter. We'll hopefully find out more soon.

I hate the guardian 4 - after the 2 hour warmup it then goes to updating! for me putting it on my arm. Quite often after 6 hours updating I know it’s not going to work. It does do calibrations as I often get calibration not accepted notifications. Also I find it’s not the greatest accuracy. The other night it told me my BG was 5.4 but I was feeling yuk, on doing a finger prick it was actually 2.0 sadly I have another year before I can change insulin pump with the cgm.

I have the Libre 2 prescribed in the UK. It’s the only one the NHS prescribes as far as I know, don’t mean though, it’s free.

You should also talk about the links with pumps. Because the guardian 4 looks bad here but it has the « close loop» with the Medtronic 780 and it’s amazing.. (Sorry if it’s not clear, I’m French)

Just got the guardian 4 yesterday got my 780g last week. I didn’t wear Cgms because of the guardian 3 I always had to test and kept getting knocked out of smart guard. I wasn’t sure if I even wanted the 780g and the cgm but it’s absolutely incredible I haven’t been knocked out once I don’t have to test all the time doesn’t need calibration every hour to stay in smart guard. I haven’t even had to test in 24hrs and when I do test it’s very close. I don’t even realize it’s on my arm either I forget all the time😭 personally I love my Guardian 4 so far

Are you certain about the Libre MARD? I read in the user guide that bit is the exact same algorithm as the Libre 2. They didn't do anything to change it and didn't run any new studies. The Libre 2 does not have that low of a MARD so just double checking. Thx

While you are correct, the algorithm is unchanged, I believe the size of the sensor is what lends to its improved accuracy. A large part of what led to the Libre 2’s inaccuracy were due to compression lows at night. I believe the larger circumference of the Libre 2 was part of the problem, the smaller Libre 3 has no issues in this area and I’ve tried it out and found a big improvement in this area. Once the Dexcom G7 is out here in the US we'll get a better idea. Freestyle Libre 3 is also still slowly rolling out so I don't have the full details yet.

I put a sensor on 25/30 mins before I pair it and have no warm up time... once it pairs I just remove the old sensor

It's nice to see the Libre 3 came out on top. Has it been FDA approved already? I heard it's coming this fall, but Walgreens keep supplying me with the Libre 2 sensors instead. So I am not sure if it has something to do with the FDA approval process and insurance.

Yep it’s been FDA approved. I’ve been prescribing it for patients already, they’re doing a soft launch right now and it can only be ordered through DME shipping companies, can’t be picked up at the physical pharmacy. If you ask your insurance which DME company they work with, you can ask your doctor to send an order.

I just want to say I had medtronic. Switched to tandem... the dexcom never worked right so I switched back to medtronic... yes the pump is annoying with all the noises but the cgm was a very big deal.

I'm curing my diabetes. I used to use over a unit every hour on my pump and I now use about .4 every hour. It's a very long process and I still have awhile to go but I know I'll get there if I keep going.
