John it is a surprisingly simple process probably made even easier with drop in sleeves. You would put the receiver groove in those on a lathe (as apposed to cutting it in a fixed deck block, where you use the bore to centre it). Knock out as many sleeves as you need, and if your machinist is any good they will be very close tolerances cylinder to cylinder. The tricky bit comes in transferring the measurements across to the heads. But a bit of bearing blue can be your buddy. Set the heads up in a mill and with a rotary cutter job done. But how simple something is to do, and how much it costs are two different things. Repairs are done using the same method... If you torch a head, weld, deck, and regroove.
