how did you nail 6 inches on the top part of OSB? There is not support under it.

5 nails on the top of the sheet were nailed through the sheet and into thin air! lol You can't space your intermittent nails every 12 inches if your rafters are 24" apart.

Unless the sheeting is 10" long it spans one to many rafters also!

why are no H clips installed on the edge of the sheathing (the 4 foot edges)?

They aren't needed and would hump the roof up in that area. The purpose is to provide support as well as a gap.
