It was planned since the first game. In the mission "Diamonds are a girls best friend" after Packie and Niko escape Bulgarins last ambush after trading Gracie for the diamonds and on the way back to drop Packie off, Niko says he wishes to put a bullet in Bulgarins head but Packie responded with "never say what you wish for outloud or else it might not come true". This is why the three episodes perfectly tie into each other because each protagonist is getting rid of the other protagonists' threat/enemy. Luis killed Bulgarin, Niko killed Ray (after Ray had Niko kill Jim, Johnny Klebitz's best friend) and Johnny.. well. He didn't really kill anyone except for his own enemy.

Yes, in the Price is Right mission, Dimitri betrayed Bulgarin by ambushing the latter's men and selling the heroin to someone else. Jimmy P, with a crappy mindset and desperate to put his crumbling criminal family at par with the Commission, didn't realize that.

I remember after this mission I was so shocked that Dimitri would turn on Niko, and Niko simply just didn't listen to Faustin. When it was time for me to choose between Revenge and Deal, I decided "screw Dimitri" and just go with "Revenge". Even to this day, I STILL choose the Revenge ending.
