We also havent seen what spawned the extermination. Lets jot forget the time frame. Its never been established how long the exterminations have been happening, however, we do know that not 1 sinner in hell still alive remembers that angels can be killed. That means all of them, including Zestial do not remember a time before the extermination. And Sera says they started the extermination because Hell was in revolt. We dont know in what context, but we have to assume that the revolt in hell only happened because those in hell knew they were a threat to the angels. After all, Zestial doesnt even know of the revolt. And hes easily hundreds of years old, 17th century at the absolute latest, 16th century being much more likely. So the extermination has been happening for centuries, and yet, we dont exactly know what Heavan was afraid of. Their soldiers are teaming and numberless compared to the sinners, and theyre leagues more powerful, with even overlords in hell not daring to fight an exorcist unless absolute desperation kicks in

That's what I thought as well. I think in another frame (while St. Peter was singing), they also showed a commercial sign for "Science" or something like that. Considering how much work it is to create those frames, I think that are hints for something. Edit: Also, the buildings in the background do look relatively futuristic. Don't know what's the big idea, but it seems heaven is not entirely what we think it is.

Sera's biggest mistake was not making sure that Adam and the Exorcist didn't leave angelic weapons in Hell and that they were actually targeting Overlords rather than just simple sinners that they found on the street. Morality about the Extermination and Charlie's plan with the hotel aside, trouble was going to come to their doorstep had the secret about angels being able to be killed by their own weapons in the hands of sinners got out in a different matter than how it originally did; if Carmilla had decided to unite with the Overlords, regardless of their own affiliations and beliefs, it would have been a lot worse than it would be in episode 8.

A theory just occurred to me, I don't know if you remember A Bug's Life (1998) where the bad guy Hopper says that the reason they go to the island of ants is not for the food, but to instill fear in them and They don't want to reveal themselves, I think the same applies in this case, the reason why Sera allows the exterminations is not because heaven is really in danger, it is because she wants to feel like keeping hell under control, she wants to maintain the illusion of control.
