Brian was in a horrific motorbike crash in which he suffered a smashed femur, broken ankle, detached kneecap and to top it off he also has arthritis which prevents his arm from fully extending.

If only you were here.. seen so many bike fitters as a person who has two strokes and be blessed to ride is my own happiness Learn a lot from you, I am self learning my own fit with my bike mechanic. It's surely not 100% but at least am more aware of issues that can be for me.. Thanks James

shortening the stem by 30-40 mil is quite huge

The stem is not shortened. The saddle has changed 35mil, not the stem.

I wonder if a majority of fits result in a shortening of the cranks? If so, would it be wise for the bike industry to move toward shorter cranks in general?

Classic case of someone not lowering their saddle height as they age, our height only decreases over the decades, our inseam only decreases as we age mainly due to joints wearing down.

Is it possible to have different size cranks to overcome extension issues with one leg rather than 'pandering' to the lowest denominator?

Usually not recommended as it messes with symmetry in the sagittal plane.

Shims between shoe and cleat, possibly cleat fore/aft adjustment if leg length diff is mostly in the femur, or custom shoes with different sole thickness is they way to deal with leg length differences. Changing crank length addresses other issues such as range of motion limitations, positional compromises due to other issues or discipline specific performance oriented changes, eg, for TT or Triathlon.

What saddle did you fit. I have narrow sit bones and can’t find anything narrow enough

How wide are your sit bones if you've measured them? The PRO Turnix and Falcon are available in 132mm and the Selle Italia SLR in 130mm and Flight in 135mm. My sit bones are just under 11cm and have used the PRO Turnix 132mm for several years. It supports me well and no thigh rub or hitting the wings with the back of my legs. 1'm 184 cm tall and about 90kgs.

Selle Italia SLR Boost was my choice - 130mm wide and is the comfiest thing I've ever ridden. 68kg, 181cm tall, narrow framed but herniated 4 discs, compressed one and damaged left side nerve system. As a result, I'm like old mate in the video (literally, same positional problems) but 31yo. Saw a local fitter last Nov in Aus, he did almost the same adjustments, tried the SLR Boost (got the 3d printed one) and I'm pain free.

Bike industry has been maknig a huge disservice for below average height people with the crank length availability, it's slowly getting better but it's shameful you are resorted to go for lesser known more expensive niche brands if you need anything shorter than 160mm and those only became available with the latest generation. They are still hard to find despite being on catalogue, shame on Shimano/Sram/Campagnolo for causing fit problems for their shorter clients, heck, both Japan and Italy aren't known for their tall populations, what were they thinking?!

Yup, I had to buy from Croder to get 160 mm because it wasn't available from Shimano. I didn't want an aluminium spindle that would have been eaten by the steel bearings of the bottom bracket, so I bought the steel spindle. I ended up paying Ultegra price for a Tiagra-heavy and rather bad looking crankset 😅

On MTB’s many of us moved to shorter cranks years ago, it was so we could run lower BB’s - but once we did we realised that they worked fine, even for us with taller folk. I run 165’s and a near metre inseam.
