Hi, I am worried about taking long term drugs for managing pain associated with cervical spinal stenosis, foraminal with nerve impingement and central, herniated disc with spinal cord impingement, DDD, kyphosis and anterolisthesis C3 in C4... if pain is masked and I therefore do more, can I irritate the condition and make it worse? I have found when taking pain meds like paracetamol with ibuprofen and working, once the meds wear off I am worse. Isn't pain also an indicator by the body to stop or modify activity? I dont mind taking pain meds if I can rest, but taking to be active worries me. Thanks.

The symptoms you describe (ie some of the 5 D's) that indicate a "positive test", make me think VBI...and if my pt presented with these I would suspect a much more sinister cause than cerviogenic dizziness which I thought was more so related to a disruption of the normal afferent signals from the upper cervical proprioceptors etc. I thought cervicogenic dizziness was a diagnosis of exclusion once the neuro screen, vestibular screen, VBI etc are ruled out, thus if my pt had any dysphagia, dysarthria etc during the test you described, I would be concerned and would not just treat as cervicogenic dizziness ... Is my thought process incorrect? Any help appreciated, thanks for this series!

Curious about this too. I was also taught that cervicogenic dizziness is a diagnosis of exclusion once all other tests are complete. I think you may be correct tho.

Yes, some of the signs & symptoms can be indicative of VBI and we have listed them as they were listed in the study by Treleaven et al. (2019). At the same time we would not expect symptoms of VBI to occur if the head remains in rotation position for such a short time. Symptoms are described to worsen with prolonged rotation (see the extension-rotation test or the cervical torsion test). Cervicogenic dizziness is still a diagnosis of exclusion, so a proper screening procedure would be the first step, but we cannot include the whole process in each video. Like so many other diagnoses specific tests can only do so much as to make a diagnosis more or less likely, so they can add weight to this diagnosis of exclusion.
