This was so helpful for me. Thank you! I added three things personal to myself. 1) Being OK with having this TMS condition, 2) Being OK that I get resentful that I have it (self compassion) and 3) Being OK that deep down sometimes I seem to take satisfaction in being a “victim” of this. Crazy, huh? But the way you put this helped me see some things clearer & be OK with whatever is going on with me in the moment, no matter how embarrassing it might be to admit. Just sharing in case others ever felt like this.

My back went out last week, I ve been spending it in pain with a very minimal amount of movements, although I read John Sarno years ago and was sure it was emotional, and did my best to ignore and focus on the real emotional pain, I was not ok in feeling the pain, felt dread instead and did not realise it was making it worst! Listening to this video was a real awakening! I live offgrid and needed to cut wood for the fire so I followed what you said and felt, accepted the pain and kept awareness on the physical pain and whatever was coming with it, soon enough I became emotional but i did not overreact, old painful memories popped up and I allowed them to come and pass. Well, I ve cut 2 wheelbarrows full of wood and the pain is totally gone! Thanks for those precious videos!🙏🙏🙏⚘

I understand all the concepts. I have been trying to incorporate them. I was put on opioids many years ago. They do help with the pain but I know they are not the answer. Do you think they are interfering with my progress?

I just found you two days ago and you have already broken things down for me better then the psychologist I've been talking to for the last 3 months. Every Western medicine doctor nose very little about mind body and their first line of defense is change of diet, maybe vitamins then medication. I want to go the more natural route and really get to the root cause and not just self-medicate. I really appreciate your videos as my anxiety has been happening for over 10 years and now causes random symptoms and sensations causing me to be extremely hyperventilate. I'm learning so much and my rational brain understands what I need to do to recover for the most part but is having a very hard time putting it into practice and not getting caught up in what I'm feeling

Hi Jim, I really enjoy your videos, you're so good at clearly explaining what's going on in our nervous systems and bodies. The somatic and emotional tracking exercises are particularly helpful, and I go back to them regularly. I have a request, would you be able to compile a playlist of all the videos that can be used as a practice (brain training/meditation/somatic tracking exercise), so we can incorporate them into our daily lives more easily? There are probably exercises that I'm overlooking because I can't see from the title if they're an exercise or an educational video (which are also great, by the way). Thanks for doing what you do!

Hi! Not sure I’m able to do that but can look into when I have some time. Meanwhile if it’s TMS you are OK you are just trying to let your brain know that and acknowledge underlying emotions and stressors that cause physical symptoms. You are right, I recovered from my concentration issues within 10 days by completely accepting them. But I still have other symptoms which aren't getting better despite what I think is acceptance. You really need to do it in a particular way for it to work, it needs to be a complete paradigm shift as you said, and that is definitely easier said than done. It seems there is a bunch of hope and enthusiasm driving that paradigm shift as well, the problem is that it is hard to keep those feelings going for more than a few weeks.

This word OK keeps coming up in my life… 1) first in re-parenting myself (“You’re ok”) 2) then in learning healthy boundaries (it’s a misnomer to think “I’m not OK if you’re not OK” - that’s co-dependency) 3) and now with unlearning my pain by telling my brain that it’s OK - there’s nothing wrong with my body

That’s a lot though. I think I can be ok with the hurt I feel about things that have happened in the past but I still feel the hurt. It’s not going away just yet. I feel that’s more acceptance than forgiveness. And stepping outside dépression is such a tough one, like believing you can breathe in air when you are drowning. Would you say to aim for these in order or all at the same time? I understand what you say but it seems almost impossible to be ok with all those things. I like the videos very much, I think I just want you to acknowledge that these things are really hard to do.

Yes I agree with you it’s not easy and there may be some of these things on the list that you are OK with and some other ones that you’re not. Here’s the thing it doesn’t change what you’re dealing with because you’re still going to be dealing with theses things regardless. It’s how you look at and perceive them that starts to elicit a shift.

Great advice. Although "not treating them" maybe has a caveat? I have CPPS and IBS (really i believe the latter is just a subset of the former) and while i accept this is TMS, and try to be willing to tolerate as i work to address the underlying issues, still...meanwhile a hot bath, or some qi gong or a meditation etc etc can certainly help ease the physical symptoms and help calm. I recognize its not a "fix"...but hopefully not unhelpful (??). I guess its a question of if can still be OK without these tools...which i would be, or would try to be. But as i say this im sipping a peppermint tea and the anxiety-induced bloating in my stomach has backed off, y'know? Its like a form of self compassion

Dear Pain PT, is it okay to place a heat pad under your butt/perineum to ease the severe pain of a flare (in CPPS) or will that too be misconstrued by the brain as an effort on the subject's part to 'fix' something down there? :) I am asking because often such a flare can last for hours and can test a person's endurance, especially if it comes on at the end of the day when one is struggling to stay awake. Btw, this is IMO one of your best presentations to date. It is a crisp yet comprehensive synopsis of the key issues/ bullet points of the Mindbody approach to Neuroplastic pain as pioneered by Dr. Sarno and others.

My opinion is its an act of self compassion. As long as we know it's not there to "fix" then why not? It's not much different than somatic tracking meditation or anything that can help with calming. I know my cpps/ibs is TMS. But I like a warm bath or a peppermint tea. These are real physical symptoms, and likewise have real things that may soothe (not "fix"). I like to imagine the inner parent saying something like "we both know this isn't the cure ..but if it can soothe a bit, luft the mood or whatever...then take in that in"

You have to get the prefrontal cortex working for you to change these patterns in the brain. It starts with understanding that your issue is not related to structural pathology. Then you’ve got to get moving and get going again to put your energy on other things outside of your body.

That is ok to do absolutely. BUT at a deeper level you have to come from the brain to generate the highest level of communication which is that you are not concerned by what’s happening from the limbic brain reactions

I appreciate the video! One of many symptoms I have is feeling numbness, tingling or itching. Sometimes I'll respond by touching the area and seeing how strong or weak the sensation is or I might scratch the itch. So according to #2 do I need to try to not let myself itch the itch or adjust my clothing? I do this a lot. Especially when I am at work and not wearing clothes as comfortable as I'd like. Thanks!

Yes see if you can be aware and pull back from doing that. that is a behavioral reaction that is just strengthening the symptoms. You will have to sit with the uncomfortability of some feelings coming up when you don’t do that and let those pass that will start to break the link

Yes try to be OK with that. If you read about panic attacks a lot of people will say tell your brain to give you more you’re trying to break fear as that is what drives anxiety symptoms

Ok here’s a question. What about taking meds consciously? So if I tell myself, I’m going to take this med for pain AFTER I realize this is an emotional issue, is this ok? Many people would suffer horribly if they went cold turkey without medication. Myself included. And if it’s ok, pointers on not having my brain shift back to “it’s physical”.

Take as needed with the goal is to try to wean off of them. we know long-term it’s not good to stay on pain meds it should be for short term use only. I think once you put the pieces together to see that your pain is more about stress and emotion then keep your mindset there and think of it that way. You have to take the extra step each time of converting the symptom into an emotion

Jim, this is probably more specific to some of the other videos you have made, but I'll go ahead and ask. In your experience with patients, have you ever notice a map of pain symptoms on the body that are mostly associated with certain emotions? For example, thigh related to anger, neck to fear, etc.? If so, I have been wondering what emotions pelvic floor pain and tightness is mostly associated with? I love your videos.

It’s a good question but I don’t think I have a good answer. One thing I can say is there is overlap on the body maps for emotions so it can be different emotions. I think it’s better to look at the scenario or situation in terms of how you’re feeling to link up the emotion versus the location. You could have multiple emotions which is quite common. Fear is one of the biggest ones which manifests as anxiety. Then it can be a version of anger or sadness categories as well. Even over excitement can cause symptoms. Anything where there’s too much emotional arousal can create physiological symptoms in the body

@@thepainpt Thank you! I think I also thought whichever symptom (area on the body) is the most pronounced at the time that that would mean that emotion needs the most attention at the time. I do know I have a lot of anger, fear and sadness, but I can see that knowing which area on my body is from which isn't going to help me recover quicker. They all need addressed/experienced.

depends on the stress, but stress will likely cause you symptoms because your window of tolerance is not big enough. The only way to increase it is to expose, stress the system. Thus, without overdoing it, exposing yourself to stress is the only way out and therefor it´s something positive

I can do this with all of my symptoms except SIBO symptoms. I know there is a medical condition there so how can I stop being hyper vigilant when it's not a TMS symptom like Fibro pain, migraines, etc.? Am I over thinking this?

The most important thing is the cause of the symptoms. If it’s chronic and not acute or subacute and any severe structural pathology has been ruled out, then yes you are working with your brain and nervous system and trying to teach you that you are OK and can do these things again

@@thepainpt I've got a slight bulge in L4 L5 I've had at least 8 Drs tell me it should not be causing any trouble I had successful surgery neck Aug 2020. I've simply gone down hill since I focus on every sensation ache and pain. A neurologist as told me it's my brain that's doing this to me. I'm struggling to sit down because of built up fear. I can walk bend stretch even do the splits (maybe not ) I know its me I focus the minute I open my eyes.

If you have ME/CFS you should NOT just go back to exercise as you did before. It is very dangerous. It is not just about being worried about the exercise itself. You have to gradually teach your body that it’s ok very very very slowly. Otherwise you could end up bedbound for months or longer. I can tell you from experience. So please don’t listen to point 5 as if it applies to you.

I wish it were so. But i was in a situation where a large amount of anger and hatred was continually generated. I accepted these feelings as being okay, which lead me to act on them, which further strengthened and worsened the symptoms.
