Just to add something: if someone is living in Portugal on a non-permanent basis, there is social security insurance (about 120€/month) that gives access to the services as if you were a Portuguese citizen, including receiving sick subvension (baixa).

I used to work as a financial representative at a major hospital here in the US years ago. I used to see people being rejected for life saving procedures like transplants due to lack of adequate or no health insurance at all. I also witnessed people literally blow through their entire life savings and file bankruptcy when a family member needed expensive, ongoing medical treatment. It was heartbreaking. It blows my mind that cancer care is free there.

Portugal was the first foreign country I visited outside of the USA when I was 22 years old back in 2008. On the flight over I contracted a bad case of bronchitis. Visiting a doctor (and subsequently a pharmacy) in Portugal for the first time was an experience that completely shifted my worldview. To be seen right away without waiting for 30 min - 1 hr in the waiting room of an American "urgent care" and then another 30 minutes sitting alone in am exam room before anyone comes and to actually meet with the doctor instead of just a nurse practitioner or assistant, to actually spend time explaining my symptoms as they listen and to carefully undergo examination instead of just being prescribed antibiotics in under 2 minutes flat and being dismissed, to leave and find out what I owed for the visit (I think 8 euro's maybe??) and then going to the pharmacy and getting 3-4 different medicines for literally a few euros I was completely blown away. I kept thinking the prices were wrong. This is one of the major reasons I believe in the power of travel and the importance of leaving the USA. People need to understand with their own eyes how it could be, how this tiny poor country figures out how to do it better than the richest country on earth. I've been back every year since and used both private and public healthcare and I'm never not impressed. I want to add another note for your list: If you do use private healthcare, there are a number of insurances available both in Portugal and even out of the UK. The cost is extremely low (like a thousand euros per year) and will cover almost everything in a private clinic without co-pays and deductibles (the kinds of scams we live with here in the states.) For context: I'm self-employed finally have health insurance now at 35 along with my wife (who is Portuguese) as of last year and it is costing me $955 per month plus my deductibles, plus co-pays, plus still paying out of pocket for medications, etc. Just the sacrifice you have to live with here if you don't want to end up bankrupt.

I see a lot of people excited about the European health card, but in addition to being only possible for those who live and work in Europe, it does not entitle you to free "a la carte" health care in another European country. You can't go to Germany or the UK for a health problem and expect it to be free. The card is only valid for stays, if you fall ill during a trip in Europe (tourism or work) you can receive the necessary care there, in the public health system of the country in question, (at local prices), without having to return home. It is a protection, not a (free) credit card to use in health.

Don't forget that 50 are an outright "no" regardless of whatever the legislation could be, simply because of who wrote it, and who will have the final signoff on it. You didn't see this for other widely popular Covid relief bills under the previous administration & Senate. What they are not telling you is that to make an appointment takes months and months. We were there on vacation (I am a Portuguese citizen) my son was involved in a serious car accident, in which 2 ppl died. He was in the hospital for one month before they ever did any type of scan.. when I asked them to do an MRI, they said it would take another two weeks. I said a prayer and with the help of friends was able to put my son on a plane back to US. Within 11 hours we had an MRI done, a bruised spine was diagnosed and we were home. It’s a lovely country, but unless you have access to private health care, don’t expect high quality.

I was born with chronic health issues (including kidney failure), and haven’t seriously considered being able to live out of the US because I grew up being taught that the US has the highest quality healthcare. That other countries may be cheaper, but they couldn’t take care of me as well. But taxes are higher in Portugal. Because you have to finance it. Be very careful, it is not as negative as you think. At the opposite. So yes, taxes are higher in Portugal, and gas it twice as expensive as in the US, for example. But now, pharmaceutical companies are not facing individuals, like in the US, they are facing a population, represented by a government. The direct consequence is that anything healthcare related is way cheaper in Portugal, and by a lot. My dad who spend half his time in Portugal and the USA, pay around $10 per month for his inhaler in both countries, when he goes to the pharmacy... There is catch. In Portugal, the transaction is complete at the pharmacy. In the US, the pharmacy send a $1000 bill, per inhaler, to his insurance company... Next time you vote for an US politician that promise you tax cut, make sure you understand how you are being scammed...

The agency helping us with the paperwork for the consulate appointment has told us that it’s not possible to obtain the European Health Insurance Card because we haven’t been contributing to the Portuguese Social Security system through work deductions, (well, that’s what I understood when I asked about the card). Obviously your experience was different. Would appreciate hearing from any other recent expats about this!!!

If you are going to pay your taxes in Portugal than you will enter the Social Security system and thus receive a Social Security number, then you can apply for the European Health Insurance Card

@@milomhoek yes, if you are under the NHR regime and do not paye taxes because you do not work in Portugal, you cannot have the European Health insurance card. In other word, if you do not work thus are retired or work for a US company, you cannot have a Portuguese social security number, as a result you do not contribute to paying in the Portuguese social security system and cannot get the European health insurance card. Same for nursing home priority is for Portuguese and for those who paid in the Portuguese social security system. For a retired US citizen you will be directed to private nursing home.

Great information, and thanks especially for looking at nursing homes, a longterm concern. US health care is a shambles with profit clearly (and insanely) the driving force. But, some states have set up policies and systems that are more humane than others. We're in Oregon, and grateful to be here at least for now.

The cost of a nursing home and the assistance that's given to cancer patients is what definitely caught my attention. I'm stage 4 and at some point assisted living or skilled care may be in my future. It's comforting to know what's available and what's possible even when experiencing a difficult situation. Thanks.

Great video! We're planning our exploratory trip for this December or January. Not sure if we'll be in Lisbon, Porto or in the Lairire district. How's the weather then? I see 50 and 60s which can be really cold especially for someone like me who is always cold. Will coats be be needed then? I really don't want to pivot our trip to Mexico and visit Portugal in the Spring. I just need it to be warm wherever we go.
