Loop diuretics, such as furosemide, cause an increase in the excretion of chloride (Cl-) ions in the urine, which leads to an increased loss of hydrogen (H+) ions. This results in a decrease in the hydrogen ion concentration in the body, causing the pH of the blood to increase, or the blood to become more alkaline, a condition known as metabolic alkalosis. Additionally, loop diuretics can also cause an increase in the excretion of bicarbonate (HCO3-), which can contribute to the development of metabolic alkalosis. The decrease in chloride ions and increase in bicarbonate ions can lead to a decrease in the activity of the enzyme carbonic anhydrase, which is responsible for the removal of carbon dioxide from the body, this can also lead to the development of metabolic alkalosis. It is important to note that loop diuretics can cause other electrolyte imbalances as well, such as hypokalemia and hypomagnesemia, which can further contribute to the development of metabolic alkalosis. It's important to monitor electrolytes levels of patient who are on loop diuretics.

Negative free water clearance: refers to the retention of water in excess of solute (electrolyte) retention. Loop diuretics increase the excretion of electrolytes, leading to a decrease in negative free water clearance, which can contribute to dehydration and other electrolyte imbalances. It is important to monitor fluid and electrolyte levels in patients on loop diuretics to ensure proper fluid balance and to prevent adverse effects such as dehydration and electrolyte imbalances.

Q1 We know what maximum reabsorption of electrolytes occur in PCT but why doctor give loop diuretics like furosemide. Q2 Due to give of diuretics to patients Hyperuricemia occur , how to decrease level of uricemia in blood.

Q. 2- i think as I know colchicine is a best drug to decrease the level of uric acid but here i am not sure because it is a drug of anti gout but it also inhibit uricacid formation so we can use that drug in this condition

Unfortunately drug that may act on PCT has not been designed yet that's why loop diuretics are the best option among all diuretics. Although Carbonic anhydrase inhibitors work on PCT & affect Na+ reabsorption in indirect manner, leads to mild diuresis.

Diuretic induced hypercemia can be managed by following healthy diet & if required allopurinol can be prescribed as urate lowering drug.

That's a reason because maximum amount of electrolytes are reabsorb from pct, if the drug which block the reabsorption of 65% of electrolytes from the pct can cause bradycardia and severe electrolytes imbalance. I think that's a basic reason
