I had a femoral neck fracture in August of 2016. Fainted, fell off step and hit cement and bluestone. Age at that time 54 1/2. Had fixation with three pins in Jersey hospital. One pin slipped out of place by 1st follow up. Walked with limp, did PT, etc, Fast forward, Spring 2017, a lot of pain. Decided to go to NYPH in June, top doc tells me pin scratching cartilage must come out. Within 8 days of learning that, had all pins out. Had to let holes heal a bit, then started PT in mid September. This has been the longest road of my life. I wish I could turn back time. Was dehydrated and hungry at the time of fall. I’m hoping eventually I will be healed, but I’m losing faith. Have Beta Thalassemia Major, so that complicates things, and osteoporosis. Mobility is priceless, just want to get back to where I was. Word to the wise: Always stay hydrated and well nourished. Thank you for posting this video🖖🏽

My 84 year old grandpa fell and hurt something and I’m guessing his hip or pelvis. Whatever it is it’s super severe that he’s in massive pain and can barely walk (he walked pretty fine before the fall). He’s in heavy pain killers every other hour and he’s still hurting. But anyway after all the research I gathered, I think that’s pretty much his life now from now on. Life is scary

Good information. At some point I would like to see information on how a hip fracture or any serious fracture affects the whole body even when surgery is within the 48 hour window. However, after reading the sad stories of some of the patients I want to present a hopeful story. I was/am a very active person with a passion for hiking and back packing. I back packed solo on the John Muir Trail when I was 78 and was told I was the oldest solo woman to do so. In the last month of my 79th year I suffered a traumatic fall and broke my hip. It was a clean break in the femoral neck and repaired with a partial hip replacement. Never any post op pain. Pain came in the stiff tendons, etc after the postural restrictions were lifted. The surgeon fixed a crooked pelvis and a discrepant leg length I had suffered since age 5 after polio. Recovery went along at a fairly good pace and mostly forging ahead. dispite predictions that I would not regain all my pre fall function at 9 months I was hiking again in steep mountain canyons and am up to 20 lbs in a back pack at 12 months. I am about to turn 81. I did PT 2x week for 21 sessions and worked dilligently with exercises at home. I still do some of the exercises just to keep limber. The body's energy output caused me lose 18 lbs . I am a small woman. Nutritionally I focus on protein for muscle, calcium including a supplement called "grow bone" , potassium and just nutritional density. I feel so very grateful to be in the 25% group of fully recovered seniors. I now wear a phone and whistle from a neck lanyard and always when outdoors have a hiking staff or cane in my hand to stop any fall. I am expected to live into my 100s. 3 of my close relatives including my father died within 6 months of fracturing their hips all were in their 80s and had co morbidities including dementia so folks were scared for me. I had an excellent surgeon and fantastic physical therapists who saw me through recovery including a few set backs. Full recovery is possible.My surgeon is certain he will be doing the other hip in time. Nope, tried it once, didn't like it and am doing everything to rebuild bone and to avoid falls, including wearing snow boarder's armored shorts and knee pads.

Hi Trilby, I am a very active male who had a femoral neck fracture of the left hip in a bike accident. I was 64 yrs old and I was told that I had a clean break. I was hoping to keep my own joints but my surgeon gave me a partial hip replacement. My hip was very stiff for a long time after surgery but loosened up after about 9 months. I still had terrible pain and stiffness if I walked for any distance. At night, I had difficulty sleeping but the pain and stiffness would go away after a few days. I went to my surgeon and we decided to have another surgery for a full hip replacement. This second surgery was 16 months ago. My second surgeon told me I was already starting to develop arthritis from the partial replacement. I suppose I am doing fairly well after the second surgery compared with others who have commented here. I can now walk pretty well without pain, swing a golf club, play tennis. I have lost some flexibility and mobility and I am not able to run as well as I would like. I am doing as much rehab in the gym as possible. I run on the turf for about 20 yards and I have a slight limp. At least I can do jumping jacks and jump roap without any pain. I can do walking lunges while holding a 25 lb dumbell. I am trying to regain all the strength in my left hip. It's a slow process. I am determined to retain my old function, but I still wish I could rehab with my own joints and retain all my function and flexibility. I don't like having a metal joint. I have previously rehabbed frozen shoulder, knee cap pain and hip flexor tendonosis, but I prefer to have my own joints. If Rocky Bleier can come back, so can I. That's the attitude I need to have. Also, my sister, age 86, has a hip replacement. She fell and broke her other hip and femur when she tripped on the side walk. She is doing pretty well considering all her injuries.

@@lawrenceterbell4496 It sounds like you are doing pretty well. My surgeon told me that I had as good a hip as possible but it still would not be as good as my original hip. I can do everything I want to just not in the same way as before the fracture. I still can't squat well so getting things from the floor is awkward but doable. I hike and that is important to me. I have some achiness, still some muscle weakness and a little balance problem so I'm considering another course of guided physical therapy to resolve those issues. I'm at15 months and know that it can take a full 2 years for recovery, Also at 81 age is a factor but I expect to continue my active life for a few years yet. I wish you every luck in your continued progress. Optimism and belief in success I think is essential.

I fell on my hip skating about 2 days ago and it hurt super bad. And since then I can’t sleep on my hurt side it’s super swollen and hurts to put any pressure on it. Thinking of getting it checked out.

Maybe you remember me? Left TKR 3/1/21. As of May 20 through 10 months of pt and constant problems, I bent to 90 degrees and was finally walking correctly and quickly. Then 5/31/22, I tripped at an exercise facility (for knee) and broke left hip. I’m in shock. Total shock. Had repair surgery with three screws in femoral neck. Why don’t they just tell me, “it could be worse” ? So I can scream. I’m 75 in a month. I was paranoid about not falling in managed not to fall through the worst of this. I had JUST begun using the elliptical after finally being able to cycle. If the surgeon thought he’d talk me into a knee revision, that’s never going to happen. I do not have osteoporosis, no dementia, not obese. I’ve taken every measure available to heal the knee. I’ve suffered a hell of ongoing pain with no help. It’s a neck fracture. Had surgery within five hours. I tried to get them to let me go home without surgery. Unsuccessful. I’m told I have PTSD from the knee pain. I’m not sure I can start over like this.

Hello I just came across your video. I fractured my hip 2 weeks ago falling at home and I am walking with crutches . The X-ray shows greater trochanter fracture, Dr said no surgery is needed and to walk as. Much as I can tolerate the pain. Sometimes I am in pain sometimes there is no pain it’s only if I am turning. Please advise if should walk more and gradually add weight and how long will it take to heal. Thanks 🙏

I buggered over a lump of concrete here in Thailand and sustained an inter-trochanteric hip fracture around three weeks ago. I bought a walker and was walking perfectly okay with modicum pain up until my follow up when the surgeon said I must use a wheelchair as the bone had moved slightly 0-0 and, in the case of my soft bones (age 66) may need another op if it worsens. Gah. Anyway, I told him I live alone and wheelchair out of the question. He told me I could carry on using the walker but not to put any weight on my leg. Your vid has just confirmed that which I'd suspected in that NO exercise at all would have resulted in more problems with DVT etc. I've always been very active and don't look (nor act) my age :D A friend in the UK is sending me a 'hip brace', a sort of one legged stocking type thing. You think this might help, or worsen the fracture by maybe shifting the break? Ta :)

Hi tamsinthai. Sometimes with osteoporotic bone fixation of fractures can be difficult and can change position over time. The thought behind reducing or limiting your weightbearing is to allow the bone time to heal before stressing it with walking. Sometimes good enough fixation cannot be attained to allow early mobilization. Every case is very different. Hope that helps.

I know it takes time, I think my doctor worrying too much :D Btw, jettisoned the brace, far too uncomfortable. Thanks again. Btw, he was really keen to get me on an injectible drug called Forteo, incredibly expensive and not much info on the net but the few user reviews there are have cited horrific side effects including pain (more pain), nausea and possible bone cancer! (I wondered why he asked if there were any cancer in my family). You have any experience with it?

My 88-yo mom, previously independent and active without cane or walker, is 4 weeks status post a left anterior wall acetabular fracture with ORIF. She still can hardly get out of bed due to pain in the hip and pain in her right foot with attempting to stand. She is still in the hospital due to no subacute rehab available. Social workers have been so unhelpful.

I hurt my lower back many years ago. If I had taken the advice of my chiropractor at that time, see a qualified orthopedic doctor, and get X-rays, which I didn't. Here I am 40 years later and crippled, needing steroid injections to deal with the pain, which at times is pretty intense. My point is don't think you can heal yourself like I did.

Hi Docs. Love your channel. I’m a young 61 yrs old. 2021 was a challenging year in that I had both knees replaced at the same time in April. Thanks to your videos I did all the pre surgery pt and post op pt and recovered well. I was even golfing at the six week mark. Then after one of my best rounds of golf in December I fell and broke my right hip. Had surgery to repair with the long screws but it was very difficult to stay down for the six weeks after surgery. It’s now March and I’m barely walking without cane/walker. I really hope I don’t have to go back in to have the screws removed but if so how long of a recovery for that? Haven’t swung the clubs yet but hoping all will be ok for the upcoming season. I haven’t checked all videos but any tips on getting me back to my game? Thanks for your videos

This took me back to when my 95 year old father broke his hip. We were in emergency for a few hours before he could be admitted to a bed while waiting for surgery. Whatever pain killers they’d given him in emergency turned him into a raging bull and he had to be restrained on the gurney. It was very difficult to see this and to try and comfort him while we waited. I guess as a result of his agitation he was given a general anaesthetic and the post operative delirium was terrible and lasted for what seemed like weeks. The delirium turned a quiet and dignified man into someone we didn’t recognize and resulted in many, many days and nights of being restrained in his hospital bed due to aggressive behaviour and non-compliance. The delirium eventually lessened with time but he had to be given anti-psychotic medications to try and calm him down. It took 8 months but Dad did recover well enough to be able to walk short distances with a walker but passed away within 10 months of his surgery. His sister, who was in her late 90’s, also broke her hip but a spinal anaesthetic was used instead and there was such a difference in her post operative condition and speed of recovery and there was no delirium. In both cases the fractured hips marked the beginning of the end. Dad’s sister didn’t live more than a few months. Thank you for providing this comprehensive overview of hip fractures - no fun for anyone but so very deadly for the elderly.

I'm not sure if I broke my hip or not. Let's just say I hurt it. I'm 45 and mis-stepped into a shallow pothole in a parking lot in 2020. Took at least 3 months for the worst of the pain to be over. I couldn't even role over in bed. At 6 months I had to walk very careful because sudden pain would stop me if I didn't step carefully. At a year things were mostly better with the rare painful moment. Years later I am still careful and feel the occasional pain start in the middle and stream outward across my right side, but now I can keep moving through it. I never saw a doctor. Is there a way to find out if I had a broken hip? For some reason it hurts tonightt. Feels like my right side is bruised and burning a bit. Is this what arthritis feels like? I kind of wish I knew what happened to me.

I was in a car wreck that resulted in a hip & pelvic fracture. I’m 29. I had to have surgery and had two screws on either sides & two on one of the top sides. Not sure exactly where, didn’t get to see a clear picture of my x rays. They have had me on a wheelchair for going on 5 weeks tomorrow. ☹️ I am to see my dr next week to begin discussing PT & Rehab. I know you couldn’t possibly know without seeing my x rays or knowing my medical history. But what can you tell me? Advice? Thank you. 🙏🏻

Hi Nitin. In Canada where we practice unless it was felt that the patient would not survive the surgery we would lean towards surgical care. Unless the fracture was totally undisplaced and there were other reasons to avoid surgery. It is difficult to treat non operatively in bed as it is associated with many complications like DVT pneumonia and bed sores. Best of luck. Thanks for watching and subscribing if you did

He was 76 years old with heavy diabetic condition and the heart condition was taken into account before operation. He started loosing the Oxyen % and lost heart beat after 3 days which was visible over monitor previous night he expired but nothing could be done as per dr. opinion .

Thank you guys you really helped me in time of distress, my mom is 81 had a pinning of her femur and had a terrible experience in rehab and it seemed to cause her leg to stay in a chicken wing position now for the last 1and a half yrs, and my question is the dr said her life span most likely at best for a 1yr and half expectancy, so is that a time frame that could be delayed if so- how much would be realistic in hoping for as far as a longer lifespan in your past experiences? and shes got dementia in a nursing home now, other then that no other physical aliments.

Hi valerie. Thanks for your question and so glad we could help in your time of distress. It is very difficult to predict life expectancy after hip fractures in the elderly. I suspect what your surgeon was talking about is the fact that mortality after a hip fracture is about 30 percent at six months. The best way to keep elderly patients doing well is to keep them active and engaged. This is often difficult however. Sometimes the hip is held in an awkward position relating to the type of fracture. Hopefully that answers your questions. Good luck and thanks for watching.

I was riding dirt jumps on my bmx bike and my tires where too slippery for the dirt and I was going too fast so when I hit the corner, I slipped out and onto my right hip. I’m 14 years old, and it hurts very bad and when I went in there may be a small fracture

I am a 59 year old male in great health and shape. Fractured my femoral neck with a high impact collision while playing hockey. With my own teammate. Internal fixation with 3 screws and at 8 weeks since surgery and everything is progressing well. I had 6 weeks of zero weight bearing and now I walk with no assistance. Once I fully heal will I be able to resume skating and playing hockey or is the stress of skating too much? I know that it is a difficult question for you to answer but I love playing and hate the thought that I would have to give it up. Thanks for all your great videos!

Hi Rob. Typically once you are healed you will be able to do many of your normal activities depending on the fracture and how the fracture heals. Whether or not people do things like skating or hockey or skiing is a personal choice. It is associated with some risk. Thank you for your excellent videos. I am on month 7 after a hip neck fracture. Non-union with pins revised at 6 months with sliding hip screw for 1 month now. How big of a role does nutrition play with healing and non-unions? Thanks Doc.

Hi docs., watched and liked your video in full,I have a question, what would be a safer environment after surgery for an 87 years old diabetic woman with cirrhosis ,who broke his femur bone and had a 3 screws installed, after one month she's still in pain and takes s few steps with the help of a walker but can't be in a sitting position for too long ,prior to this her most health complications was taking paracentesis every 12 days or so, is there also a closer prognosis of life due to her latter condition ? TIA

Hi, I had femoral neck fracture in Jan 2020, fixed with 3 screws. Still unable to fully weight bear, leg length discrepancy but the pain is not on the hip, but radiates from outer thigh to knee. Could it be that the screws are just irritating the soft tissue? As per the numerous x-rays, they seem to be slowly loosening and I am due for extraction soon. Thanks in advance.

So the treatment for congenital dislocation is typically a hip replacement and can really vary on degree of difficulty depending on the severity on dislocation.

The external rotation is not uncommon. If the bone was broken badly there can be shortening of the bone. A shoe lift will often accomodate for this. Best of luck to him

Hey I had a previous hip surgery in my hip BC my growth plate slipped then three weeks ago the top of my hip I copied a knee on the top of the hip at football and ever since I've had bad pain there I can run if I'm on pain killers otherwise I can't what do U think it is? Hi haydos. It is very difficult to know exactly what is going on. However if you had prior hip surgery and now have hip and knee pain, you should be reassessed to ensure there is nothing more serious going on. At 15 you should not require pain killers to run. Good luck and thanks for watching.

49 years old. I was hit by a car while walking. Broke my pelvis in 6 places including both ball and socket joints, and an H fracture in my sacrum. I have an external fixator. No weight bearing for 6 weeks at all and no weight on the left leg for an additional 6 weeks because that femur was broken in two. They are waiting to see if the blood supply is affected. I was in fabulous shape before this and amazingly, I have no neurological damage.Is there any hope of getting back to normal at all?

Hi Chrys. Sorry to hear about your accident. Your recovery will likely be long and there will be a lot of factors that will determine your ultimate function but they are fixable injuries. It is not uncommon for patients to not get completely back to pre-accident function but often drastic recovery can occur. Wishing you the best during yours. Hi Michael. That is obviously a very difficult question to answer. A Girdlestone is a salvage procedure when there are no viable reconstructive options. This is usually related to chronic infection, bone loss or severe muscle loss/damage. Hopefully that answers the question somewhat. At 13 minutes you talk about getting anyone through surgery, but then 3 months later the patient can die because of a blood clot. Can this be averted by keeping elderly patients longer on oral anticoagulants that have Low risk of bleeding? If clots happen 3 months out or longer (presumably because these elderly patients have slow recovery and are still very immobile) why not keep them on anticoagulant longer than the standard 30 days post surgery treatment? What are the risk in keeping them on anticoagulant for 3-6 months until they have made more significant recovery? Thank you 🙏

Hi t ta. So often at 3 months it is not because of a blood clot. Studies have shown more risk than benefit for prolinged anticoagulation in thus setting. Best advice is to try and regain mobility. However, she is arthritic for last 12 years and bed ridden except for 4 times a day when she walks with a walker for 10 mins each. Also she is under weight. Can we go for for the implants??? Hi Rob. The best thing to do is be reassessed to make sure that the hip healed properly and that you are not developing arthritis which can be responsible for pain after this operation. There are many patients that do take prolia but you should discuss this with your treating physician and usually in the setting of an abnormal bone density test and a failure of vitamin D and calcium. Help guys I fell in my hallway three days ago, I have been in bed ever since. Day 1 could not put weight at all day four I I am walking with walker and putting some pressure on it. It still hurts like sob but it is better than when the accident happened day 1. Due to COVID I am trying to stay out of the hospital, does it sound like it’s broken, what would you suggest?

My mom suffered a hip injury during the earthquake on Aug 14 In haiti. House collapse on her, concretes landed on her side and lower back. I'm watching this video because she is hospitalized in haiti. And I am in north america trying to understand if hip fracture can heal on it's own. She was told she needs hip surgery to fix the fracture but the location is dangerous for her age (67) weight, other medical condition.. Shes able to move her legs, but unable to lift her hip or sit. Shes has massive pain from her lower back with movements. Haitian doctors contemplating on sending her home with a bandage wrapped around her hip. Treatment bed rest. No movements. Which is terrible. I fear I will loose her due to blood clots formation or pneumonia. Can I please gets some advice? I thought of Aspirin for pain and blood thinners. Excess fluids intake, nutrition supplements, and deep breathing exercises. Is that good enough.

HI wasntbad. So sorry to hear about your mom. Dr Weening was in Haiti in 2010 after the bad earthquake that year. Most hip fractures do require surgery and you are correct that if she does have non operative treatment there is a risk of blood clots, pneumonia, bed sores, etc. It is a complicated decision that should be made with the team locally. I WAS taking care of my bones, very dense but small frame. Going through menopause, have femoral neck fracture through running, and I'm USED to exercise, v. active 🙁 So you CAN still get fractures even when taking care of yourself!!
