it's the county hospital that has to deal with the lack of health insurance in this country. It would be cheaper to provide basic universal Healthcare

We need to take a lesson from veterinary medicine. Ask ourselves before getting aggressive with lumps if there's a 5 year survival chance even with treatment if a dog is 8 years or older, its wise to leave them be. This 77yo man should have been left alone unless he had a goal to live past 85. Sounds like he didn't mind life progressing naturally. ?

These discussions seem so US centric. How does drug usage (Which are approved, used, etc) compares to other countries like in Europe? Looking at Covid and flu shot/booster approvals, the US and European countries could not be more different? How is that possible? Why is it not discussed?

How much of the budget for the county hospital though is driven by quality metrics. Quality metrics, like % of patients who get cancer screenings? In private private practice, I participate with various value based care organizations. There is definitely pressure to perform screenings by means of financial incentives. And it's not only pressure on me to recommend a test. But also pressure on patients to "ask your PCP" about getting screenings done. You're making an argument that a county hospital with a fixed budget should prioritize high value care. And cancer screening is one thing that, in your opinion, is low value care. I don't know how the budget is determined in the county hospital system. But I suspect their revenue is significantly impacted by their performance on quality measures, like cancer screenings. If so, your argument that spending money on cancer screening is a poor use of money makes no sense. Cancer screening is a revenue generator. Rather your argument would be better served for CMS or whichever organization dictates the quality metrics and incentives.
