Hi Jason, someone in your comments sektion raised an really interesting concearn about the supposedly "positive effects" in airflow data seen from using v-com. That it's actually mostly a artificial effect from "tricking" the airflow algorithm in the Resmed CPAP machine, so that it actually reports inaccurate data when the v-com is disturbing the airflow. This is kinda important if true....have you heard of this before and verified if there's some truth to this?? 🤔 Thanks for helping out the papper community! 😉👍

Uhh, wasn't the v-com device later found out to do the same thing, as just changing the mask setting of the Resmed machine from pillow to nasal/full face? Or am I remembering incorrectly? 🤔

I don’t know if I trust the data on the card with the V-Com in line. Yes some of it is better with it but some is worst. I did have to raise the pressure by 1 but had to turn EPR on to 1 to get a good night sleep without swallowing air or chipmunk cheeks.

still confused on what someone on low pressure (6.8) and EPR2 is supposed to do if they get aerophagia and also can't handle higher expiratory pressure. If pressure is higher aerophagia is unbearable. Can't exhale without EPR though 🤷🏼♂️

I find it harder to take a breath with V-Comm. Not sure what to do with that ? I don't get it.... some nights my AHI is in the 5 range - and other nights - 14-to as high as 22 ?

My Dr told me to look at a V-Com for air swallowing. I did I will be drilling a nickel with 4 7/32” drill holes inside an~ .65” circle for my diy $0.05 v-com hack just to try it out. It fits between the heated hose and nose mask hose perfectly. ResMed 10. Not sure of the long term oxidation complications from the coin though. I tried using a hernia workout belt at night to keep pressure on my lower ribs and stomach from feeling bloated. It helped the first night but not the second. I will keep trying. I think it was too high up the second night. And like the CPAP I don’t notice it much. Lastly a racket ball on the end of a 4” or 5” cylindrical neck roll covered with my clean sock. For me its 2.5” from where my collar bones meet to backside of the front of my lower jaw bone. I tuck it in my shirt when laying on my back and it keeps my mouth closed without any pressure on my Adam’s apple area. It also straighten out my bite in two weeks. It works-ish laying on my side but I may drape an arm over it for that but I get more mask leakage in that position.

Can you tell me where your tongue should be when using a BPAP machine, either nasal pillow mask. Should my tongue be positioned at the top of my mouth or the bottom to avoid leakage and dry mouth. Ps: you do a GREAT job

Jason, I just watched this video on 8/17/24. I bought the VCOM several months ago and tried it for awhile and when I changed hoses it got stuck. A friend helped me get it separated from the hose today. I have the Air Sense 11 and I don't use the heated hose (I hate hot air) so I put the VCOM on the outlet of the machine like you showed in the video. I wasn't using water until my DR. suggested it. So, I have been using ice cubes made from distilled water in my water tank. It keeps the moisture cool enough for me to fall asleep. My question is will water droplets form in the VCOM and cause any issues with the machine? Thanks for the assistance. Dawn

I appreciate your channel. I've watched a lot of your videos. I am new to CPAP; 3-5-24. I have a Resmed air sense 10 with the heated tubing. Will the VCOM be able to be installed between the machine and the tubing being that there is an electrical connection between my machine and the tubing? Looking at the back of my machine I can't figure out if it will work. Thanks.

No, you have to install it before the mask on the opposite side of the tubing if you use heated tubing.

I bought a vcom from CPAP supplies and used it for 3 days on both apap 10-13 and cpap 12.8 with epr off. I found my rera went way up. Very strange.

@@microstar22 APAP usage is a no no. Select a static pressure 2-3 above your 95% when using APAP…..for a ballpark starting point.

Thank you. I have greatly improved the quality of my sleep by watching your videos and making some changes. Over the past month my social-media and internet browsing has been overwhelmed with mouth-tape ads. Could you compare some of these that are flooding the market (and with exaggerated claims) and review them. One ad claimed it would make me look "more attractive" and i suppose being well rested may do that. My thought was to first try 3M Nexcare "Gentle Paper First Aid Tape." An expensive way to see if mouth taping works for me? Then perhaps try a more expensive possibly better product that is made for this purpose. I have an emotional aversion to "Hostage Tape." Some folks might like the name? If they would market the same product under another name I might try it.

I have diagnosed sleep apnea, will continue to use CPAP but may want to try adding mouth-taping to my therapy. The list of benefits claimed on TikTok, Facebook, etc. many of which are covert advertisements for tape sales - border on comical? Stops snoring, cures sleep apnea, improves complexion, keeps chin-line and cheeks firm (gets rid of your double-chin?), prevents bad breath, prevents tooth decay, and promotes weight loss… I do understand how some of these claims may have some truth. However, I suspect that if someone has sleep apnea (obstructive and/or central) that the use of mouth tape without simultaneous use of CPAP support is likely dangerous.

You created a resonant system (the human airway doesn't have an iPad in it) that will naturally dysregulate the control loop algorithm running in the CPAP. You put the VCOM damping factor on it, yeah less bounce. Not representative at all for a CPAP during normal use. Nice job second-guessing decades of algorithm development by ResMed. Remember: SD card data is invalid while VCOM is used, it puts a damping factor on the derived flow data as well.

i'm curious as to how the flow data is invalidated by using the vcom. are you saying that my flow rate data isn't correct if i use the vcom? since beginning to use one (and boosting my cpap pressure a couple cms) my flow rate numbers have been very low. as has my ahi numbers. more importantly, my events are way down. if i now have 5 total events in a night, that's a lot. i'm just trying to get the best therapy, since i haven't seen a sleep dr in two decades. i have an as10, with the pressure set to 10.8, with the vcom attached at the machine. i wasn't happy using epr, and my numbers reflected that. i have heard and read all the various reasons that epr (especially on resmed machines) is janky. the lack of effort to exhale was more disconcerting to me than the perceived extra effort it takes to exhale with the vcom. thanks for any input.

@@GK-vj9dz The effect on the flow waveform is that it's "squeezed down" (similar to dynamic range compression in music, amplitude compression) and smoothed. This means that less hypopneas are detected since the contrast in amplitude is lost (ResMed detects hypopneas with 50% amplitude decrease) and Flow Limitation will be smoothed away.

Normally, no. The machine thinks someone is attached when you get it started with a little “bounce”. The way it looks is a great visual of how it feels.
