I started doing the exercises you describe last night, after my jaw had been locked for 3 days. I had tried everything, alternating heat and cold, massage, NSAIDS. Even went to urgent care and they were kind enough to try steroid and NSAID injections. I have done the exercises several times since learning of the technique. This afternoon, I was able to open my mouth! Going in to see my orthodontist for a mouth guard so hopefully this doesn't happen again! Thank you so much!!!

Today I was able to open my jaw fully for the first time in months, after following the 6x6 for a few days. Still some discomfort and clicking but I’m seeing a dentist soon so hopefully I’ll be able to figure out how to keep this mobility. Can’t express how relieved and hopeful I feel now though. I can barely fit two fingers in my mouth. But when I move my lower jaw forward, it makes a clicking sound, and then I can open it wider, fitting almost four fingers. Moving my jaw forward and backward keeps making this uncomfortable clicking sound. I can also feel it at the top of both sides of my jaw, and it's been this way for many months. When I first got my jaw locked, I could only take small bites of food. But after I accidentally moved my jaw forward while brushing my teeth, I found out that doing that widens my mouth. So whenever I eat something, I always move my jaw forward. Now, when I eat or drink something, I don't think about it, and my jaw automatically opens without me even thinking about it, and it feels normal, like it's opening naturally.

What happens if you have jaw locked without reduction but you still have the full range of jaw motion? Just that towards 90%+ of the max range of opening, there is a stretching sensation felt. What are your thoughts for this situation?

My jaw has been locked for 3 months. I had clicking in the left joint for the 30 years which started after being out under to have 4 impacted wisdom teeth’s taken out. After that my jaw locked for 2 months and then a serious muscle relaxant fixed it but I was left with terrible clicking and popping of my jaw where when I opened my mouth it would open shift over to one side then click open. Many times over the years or locked but became unlocked within an hour. This time it never came unlocked. There’s no pain involved unless I try to open a lot especial or I chew instead of soft diet. Physio has done nothing. I’m absolutely miserable. Can’t eat a sandwich as I can hardly put two stalked fingers in. The muscle is tight I can feel it. I’m taking a muscle relaxant Baclofen 10 mg twice a day also doing nothing. The pain that goes into my ear when I open my mouth feels like I’m being stabbed with a knife. Luckily when I’m not moving my jaw for any reason the muscle just feels super tight but not painful. I got Botox for it. Helped me not clench but did nothing to loosen the jaw. What can I do? I follow all your videos and do all of your exercises. I’m desperate going on the 4th month of this and having a small child to care for it’s been really tough. Ughh I think what you’re doing trying to help people and teaching other doctors how to treat this is amazing. I wish I wasn’t in Canada.

I fixed my locked jaw (closed) by pushing against the other side while opening. It seemed counter intuitive but I was trying everything and that is what helped first try.

Mam I have tmj anterior disc displacement without reduction as mri reported on both joints. I have clicking and poping sounds for several months but two days ago when I woke up in the morning I found that I couldn't open my mouth more wider than two fingers. My left jaw had locked. Although it had happened with me several times before also, but every time it usually opened itself. When I went to a dentist he advised me arthrocentesis which was very painful. But my jaw suddenly got unlocked after the procedure And next day when I woke up it again got locked. Then the doctor advised arthrocentesis one more time and after that it unlocked again. And now I can open my mouth up to 4 fingers but there is a clicking and poping sound in the jaw. And it also clicks when I try to touch my molar teeth together. And I also feel some pain on touching the molar teeth together. But I don't feel any pain on opening the mouth. Can this exercise help me? And my doctor was telling that if arthrocentesis would not help you then we will have to perform a surgery called arthroscopy. But I can't go for surgery because I recently had an ear surgery two months ago. So please mam tell me what are thealternative treatments for whom should I go for? Please 🙏 mam reply.

It depends on how long the disc has been displaced and how quickly you respond to treatment. On average, my patients are in treatment 5-10 months. Again, it can vary a LOT.

I got trismus after a dental procedure, and my jaw over time became closed, I tried everything recommended, but it was jammed shut, only surgery allowed my mouth to open, even now after two weeks home, my mouth can't open as it did before, but I do daily exercises, and it is slowly improving, when you have teeth removed, don't have more than one done at a time, even if you need more done, space them out, this was the cause in my case, three removed at once and too much injection into the same area numbed my jaw so much the muscle never recovered.

I have had TMJ problems for the last 8 years and went to see a jaw surgeon last month because my jaw locked closed ca 2 months ago (articular disc displacement without reduction). I started low-lever laser therapy which helped with pain, chewing and opening my mouth. But my doctor does not perform any type of manual manipulation/myofascial release, nor have I orthotics. I am scheduled to complete ca 35 sessions of the laser therapy and I am told that the joint will heal on its own within a few months. Do you think laser therapy can solve such problems without manual manipulation? I will try the release techniques you recommended!

Hi there! I don't have much experience with low level laser therapy as I don't use this in my office. In my experience, myofascial release and manipulation in combination with the orthotics has the most success with reducing the disc.

hello, dr priya. i wrote you in a different video about my tinnitus. after a lot of failed treatments and misdiagnosis i finally found a good doctor that recommendend a MRT of my jaw. I do unfortunatelly suffer from "anterior disc dislocation without reduction on the left side when the mouth is open" (no anthristis). Funnily enough I do not have a locked jaw, just all the other symptoms described including the PT tinnitus (unilateral). I watched your video and was wondering will your exercises actually help to bring back the disc to its original position or will my body get used to it and basically create pseudo-disc out of my tissue ? Do you have any timeframe (aprox in months) how long it will take until my normal tissue gets turned into scar-tissue (= pseudo disc) ? I also started suffering from bilateral tinnitus (couple of months before, but I always had a numb feeling in my left face-side. 3 months ago i started getting a unilateral tinnitus (pullsatile /radiating) on the left side where my TMJ issue is. Do you think it s related somehow (the unilateral and bilateral tinnitus) ? I once again know that this is not a professional setting, I just wanted your honest opinion. I am currently in my 3 month of Phyiscal Therapy (tmj/cmd experts) and also had 2. session to adjust my special Mouthgard from my local CMD expert.

My pain is much improved (only with wide mouth opening) however my jaw is still locked closed (31mm mouth opening). Do you know anyone in Toronto, ON that does TMJ? I got my wisdom teeth taken out a week ago and have my check up appointment in a few days. Since the procedure, I haven’t been able to open my mouth all the way (which can be pretty common after the procedure) so at first I just thought it was because of swelling and sore muscles, but as the swelling has gone down and I am in less pain from the procedure, I’ve noticed a lot of pain one one side of my jaw. My symptoms match what you’ve talked about with this. I’m wondering if it’s possible that it may have locked during the procedure? I’m glad I’ll be able to see the Doctor soon and address it to him, but I wonder how it would be treated if that is the case since I need to be careful with the healing wounds?

Yes it’s possible for this to happen. Treating this condition (if it Is in fact disc displacement without reduction) successfully is not easy and requires a lot of skill and experience. I really don’t know how your doctor will treat this - it’s entirely based on their experience level.

Hello and thank you for your videos. I'm in France. I have both discs locked without reduction after I broke my mandibular jawbone in a accident. I can open my mouth about 20mm. The orthodontist wants to put specialised resin on rear teeth to help my jaw stay open and relax my muscles, hoping maybe the discs will go into place. She will adjust the orthotic. But my original surgeon who placed titanium plates on my jaw, a specialist maxillo-facial surgeon, wants to try to pull discs back into place with minimal surgery at the same time she removes my plates. Which option in general would you go for? I know you can't comment on an individual case. Thank you.

@@theTMJdoc im in scotland, it’s unbelievably painful and can only have fluids. I dont know if this is an emergency or not or if theres something they can do immediately to pop it back. They did not seem well clued up on TMJ and would not treat it because it was ‘muscular’ when I know its the disc thats slipped :(

Thank you for the video! My jaw made a huge piping sound at a dentist visit and then started clicking. After a while it stopped clicking but the jaw was locked closed and pain all over the face, neck, shoulders and throat became massive. I’m seeing a TMJ doctor in LA and am wearing a day split and a night guard for 9 weeks now. The situation improved a little but I’m far from good. We do regular adjustments of the guards but no jaw manipulation and there was no measuring of where the muscles are loose before making the guards. I feel that my night guard is doing more harm than good. Do you have any recommendation or know a doctor in LA that works with your methods for jaw locked closed??? Thank you so much!

Yes I treat disc displacement with reduction the same way but the clicking/popping doesn't always resolve completely. Dr. Pamela Marzban is in Burke, Virginia and comes highly recommended. I don't know her exact treatment methods.

I don't wanna live anymore. My jaw is locked. I feel claustrophobic, restricted. This is happening for years. I even did surgery like TMJ arthocentesis. It went away only for a year. I've talked with multiple maxillofacial surgeons. & I've come to know that this situation can never permanently get treated. I've cried a lot. No one understands it. People make fun of it thinking like it's nothing. It's not like life wasn't hard before. Or that didn't have any other health issues. Im sorry but I don't see point of living like this. Where I've to constantly battle just to keep my mouth open. Right now I'm almost unable to breath. My nose is also stuffy. My mouth doesn't open. I wish I could just end it.

@@theTMJdoc I didn't visit oral surgeon, some of them couldn't even understand my issue properly. They were dismissive. This is what led me to purse doctors with more deeper knowledge in it. I visited maxillofacial surgeons. It is a dental specialty above Orthodontists who specialize in entire face, jaw, head, and neck. They were the only ones who understood my exact issues and told me the reality of it.

Have you seen a TMJ dentist or TMJ physical therapist? Unlocking the locked jaw takes a skilled and experienced provider and cannot usually be done with at home exercises/massage.

I just realised I probably have tmj, without reduction, cos my mouth won't open more than 2,5 cm (although I do not experience any sort if headache, pain, toothache only when I chew but it's more of a discomfort rather than pain) and my jaw shifts slightly to side......and my muscles around neck, mouth tend to get tence.....

Thankyou so much for this video - I had a traumatic jaw dislocation (open lock) 2y ago and had 7 months of physio and splinting - it was 90% better, happily and painlessly clicking but normal mouth opening. I had a general anaesthetic 2 weeks ago and the day after I just woke up with a closed lock (stuck at the point would normally click) and a stuffy ear like im on a plane. I dont know what happened??!! Maybe from the GA. Your 6x6 exercise painlessly unlocks it almost everytime with a click. But when I eat - the lock gets worse as i eat and cant be unlocked and i get muscle pain all over the temporalis. I tried the release video but was so sore it throbbed for three days after i did it once for 15 secs. Any suggestions until I can see someone? (Im in Australia and as its summer most places shut down for Jan) Is it recommended to keep the disk moving with the 6x6 or can I make it worse, I dont want to stretch things more and make this permanent? Do you think it will just settle on its own given it came out of the blue?

Is this “normal” after having teeth removed? As I know it can be from trauma but wouldn’t that need to be a little excessive to displace a disc in someone’s jaw causing lock jaw?

I Definitely wouldn’t say it’s “normal”, but it does happen. Usually there’s an underlying issue - an imbalanced bite, postural imbalances, upper cervical/neck issues, hyper mobility, etc - and the extraction is the straw that broke the camels back.

i wake up every day with mine locked closed and lose sleep because of it, i just woke up and what do you know it’s locked again and it hurts usually having an ice pack on and off of my jaw helps unlock it but this isn’t working

When my jaw locks I always have the uncontrollable urge to want to yawn. Does anyone else feel the same way? I also learned many years ago do not play tug of war with your dog while your on all fours acting like a dog. I almost couldn't release the rope and of course the dog thought i was playing still.

hi! my jaw is only locked on the left side and there is no pain unless i forcefully try to open it. is this still the same thing? there’s inflammation on the left side as well. usually it unlocks on its own after an hour or two of being awake, but it’s been locked for over 24 hours now which is new for me. additionally, is the 6x6 exercise still effective if i cannot bring my teeth edge to edge? the way it’s lock doesn’t allow me to bring my jaw all the way forward.

Yes, it's likely the same thing. Doing the 6x6 when unlocked may help prevent locking. The usual progression I see, however, is that if you're locking frequently but able to unlock it will eventually reach a point where it locks and CANNOT unlock.

@@bhasithavanneti4803 yes i went to a tmj and sleep therapy centre (dr steven olmos branches) and got it fixed without anything invasive (no surgery or injections) basically through exercises and day and night orthotic/splint. It's similar in design. But it's not so much about the design, it's more about the prescription built into it. The best analogy is prescription glasses - they all look similar and fit in a similar way, it's more about the prescription built into them that makes the difference for the wearer.

Hello Dr. Mistry, I used to have clicking noises especially when I wore my night-guard for bruxism, so I stopped wearing it for a while. Things got better and I don't know when the last clicking was. Was meaning to get another mouth guard that is a little less thick -- hoping that it would not bother my jaw as much. In the last few weeks, I started having alarming episodes where it hurt a lot when I tried to open my mouth - just felt like something was out of place. Thankfully these episodes were brief -- usually a repeated jaw manipulation would cause the situation to resolve. Then about a week ago, it did not resolve... I have had limited range of motion since then. I am now researching what it might be and its probably displacement without reduction. Given that quick fixes worked until recently, it is possible there may be a quick fix for this with a proper adjustment? I am up in Vancouver, BC, in case you can refer anyone up here.

Hi Dr. Priya! Your videos have been so helpful! However, my tmj dentist/specialist i have been working with, got an MRI of my jaw, and it has shown that BOTH sides of my jaw.. the disks are fused. Thats what he said. Im guessing its something like what you are explaining here where i have without reduction going on. But on both sides. We knew my jaw deviated more to the right sometimes, but other times it would switch. Now i guess we know why. Anyways, he is suggesting surgery. I go back and see him in 4 weeks to discuss said surgery. Ive been locked now for over a year now. Do u have a link where u talk about surgery where you "break apart fused disks"? Thanks!

I have two questions if you don’t mind: I was doing your 6x6 method and I noticed the right side (which is my locked side I believe) popped and then I got relief for 2 seconds and was able to open wider. Why did this happen? I have always had disc displacements with reduction and only in the past month has it become without reduction (I think!) when they reduced I was wearing my orthotic nightly and most of the day but since stopping my orthotic is when I’m experiencing a limited opening in my jaw. Could it have been from stopping the orthotic ?

It sounds like your disc reduced and you unlocked after doing the 6x6 which is why you were able to open wider. As for why you're experiencing limited opening after not wearing your orthotic, it's likely because you stopped wearing it.

Have you ever treated patients that were completely dislocated meaning disk was so far out that the condyle was rubbing on the fossa ? Orthotic, myofascial release and osteopathic manual jaw manipulation all help. It takes time and effort. Sometimes we check after with MRI. Other times it’s obvious the disc was recaptured and we don’t.

@@theTMJdoc okay so with all my research I definitely have a dislocated disk without reduction, before my jaw would pop eating hard foods, I could also open my mouth super wide to the point where it locks but I could always unlock it right away on command so I was doing that before bed one night & I think the disk didn’t come back with the joint 😭 I wish I could see you sooner before my disk shapes a different form it’s been 8 days now

I don't know if that splint will help. It depends on where it holds your jaw. Also, in my experience, if the jaw is locked closed, gentle manual manipulation is needed as well.

Mam just wanted to ask my tmj seems to be disc displacement WITH reduction and i wonder does modified splints would recapture my disk and exactly what is disc recapturing i mean does disc gets back to its original position and mouth opening is back to normal and also for how long will it last...(just a guess... according to ur experience) and can i take preventive measures so that disc doesn't gets displaced in the future (Btw i don't hv tmj pain right now but when i open my mouth my lower jaw vibrates due to some obstacle feeling on my left jaw)

In my experience, we can recapture the disc with orthotics, myofascial release, and osteopathic manual jaw manipulation. As for prevention, once the disc is recaptured, we recommend that our patients wear the orthotic nightly to help keep the disc in place and stable.

Because i had an injury when i was 13-14 yrs old on my jaw...and i experiencing these problems now (20 yrs old) can traumatic problems will be resolved...no more questions ahead 😅

@@omkarsil9206 yes these problems can be resolved. A nightguard is very different from an orthotic. My patients typically wear a nighttime orthotic indefinitely to keep their joints stable.

Mam I also have jaw locking problem from a year but I don't have any pain. My jaw get locked at morning time when I wake and I know how to unlock it. And it don't have any pain. But it is a daily routine that my jaw get stuck at the morning and I locate my dislocated jaw with the help of my 2 fingers. And gets at his right position only in 2 sec. And I don't get any problem all the day. So please help me.

Can a locked jaw be caused from not wearing your retainers 24/7 I have noticed that when I wear my retainers only at night I wake up with a locked jaw (but it isn’t painful) I did the 6x6 method and it worked immediately but when I started eating I heard my jaw click every time I’d bite down???

I have locked jaw closed since Oct 2021 I have no pain and I’m able to take care of my oral hygiene and eat soft food. I was doing physiotherapy for a month but it did not help open my jaw.. in addition now I have started to feel heaviness around my ear occasionally and sometimes pulling sensation in my jaw when I sleep on my back and if I’m stressed. Is it ok to just live with a locked jaw while I do my stretching exercises at home?

It's really all about quality of life - if you can manage with it locked and you have low pain levels, then it could be OK.

I have no pain luckily..I’m just hoping that doing stretching exercises will help me open my jaw.. the intra oral massage at physiotherapy only started adding some ear related issues, like numbness or fullness in my left ear.. before that I had no pain, no ear issues at all no pulling sensation in my jaw. I have been thinking a lot about which makes it worst. So I’m thinking to give it a break.. hope this doesn’t get bad or painful later if left untreated?

@@amodbh Any updates on it now? I was told the longer one waits in a closed lock the harder it gets for it to reduce due to the ligaments and muscles being in that new position for longer periods of time. I am in a closed lock without reduction

@@tapsubscribe I’m still closed lock.. I have an appointment in March, I am thinking to get an MRI and know what’s happening. Do you also have no pain with closed lock? How long for you?

@@amodbh I have some pain but mostly inflammation more, mine happened since july 2021 at least. Get an open and closed MRI, not just closed, I couldnt open my mouth enough to do open so it was not the best bang for buck basically

What a rude comment. I say EXACTLY how I correct this in this video and In other videos if you would bother to watch. there is no home remedy for this condition that I’m aware of. If you don’t like what I have to say, don’t watch my videos.
