I more or less did this in Sims 3, with one baby born via the normal means, one adopted, one alien and one plantsim. Definitely a fun one, and I was able to time it so that they were 'quadruplets' all 'born' within the same 24 hours

You can tell if your sim is pregnant without a test when you sims is doing nothing the hunger meter goes down quicker with two arrows instead of the typical one arrow

Check the simology bar. Hover over the "ages up in x days" hover over the next life state. If your sim is pregnant it'll say "sim cannot age up while pregnant"

If you hover over the life stage bar it will tell you if sim is pregnant(will say pregnant sims cannot age up) without pregnancy test. It will auto tell you the second day in notifications.
