Palpatine had skilled spies. Luke did not keep his origins secret. Biggs, a long time friend of Luke's, whom died during the first Death Star battle near Yavin, was a long time member of the rebellion. It is likely within a week of the first Death Star's destruction, Palpatine's spies had backtracked Luke's origins to Anchorhead on Tatooine. Cross reference with the droid search leading to the burned Lars' residence, Palpatine probably put all the pieces together with less than a month, long before Vader did. Palpatine probably let things play out due to a sense of humor over the whole matter.

I can appreciate that, like 90s Palpatine, he just "let the Force tell him". All the resources, spies, informant networks, etc., and it was simply using his powers that did it, but it does lead me back to wondering, why was Luke still Luke Skywalker? Various individuals had taken significant steps, and at great risk, to hide him, and protect him, yet once he joined the Rebellion, it didn't take a spy network, or Imperial/Rebel double agent, for the name of Skywalker to spread far and wide, even though several people did know how bad it could have been, early on. I know Tatooine is remote, but I'm surprised, between Ben, who wanted him hidden, and Owen, who didn't like Anakin, or Kenobi, much, that they didn't change his name? It's not like Luke was a young boy who already knew his name, when Kenobi dumped him off; he was a baby, and Skywalker isn't exactly a subtle name. King Arthur wasn't initially told of his heritage, and even Frodo Baggins adopted an alias, well before certain threats were even known to be a problem, yet the name of Skywalker was left to spread, and so there are times I am honestly surprised that it took either of these two Sith Lords, wirh their resources, and connections, to discover the truth of Luke.

I guess there are so many Skywalkers (it is common for many unrelated people to have the same last names) in the Galaxy that all it took was a dream of Vader's of Luke being his Sith apprentice and son for Sidious to realize Vader's deception and the next steps forward for the latter's replacement. Also, Sidious seeing Vader's dream is really taking the invasion of privacy on another level.
