I believe it's a mod. I don't know about the Baltics too much, but I can give the Sparknotes version of Posen and Poland. Posen being a puppet of Prussia is probably a representation of the Grand Duchy of Posen, a region Prussia annexed after the Napoleonic Wars. It was given some* autonomy per the agreements within the Congress of Vienna but was properly annexed into Prussia in 1948. As for Poland, Congress Poland was a semi-autonomous* kingdom under personal union with the Tsar. It was eventually incorporated into the Russian Empire as a result of the November and January Uprisings.
