How do you toggle item on and off on valve index

It should be the grip on the right controller to pick items up and interact. To drop it should be the b button.

For me, it's Right Grip + Right Stick Up which makes NO sense. It also switches the item to the next item...@@benpascoe323

I did not have to adjust it and yes I was able to see my hands.

I have a pico 4 headset, do you know which controls would be the best suited for that?

I am not sure what controls would best work for the pico 4 so you could try each profile and see if any of them work but first I recommend just trying the default one and see if it works without changes.
