YES! We need to do this. Is it ok on the doll? That's what's stopped us so far- the privacy aspect of patients and hospitals!!!

Completely agree. It was helpful going through this talk- a reminder of everything I should be emphasizing. It's easy when parents are talking to you, or there's something slightly off about the exam (a soft murmur etc, a large poop!) to get thrown off. Thanks for watching!!!

Loving these videos, I know you have talked about CBC in a previous video but wondered if you could talk about U&Es, specifically urea, Creatinine and electrolytes on a blood test ✌🏻

Thank you Tina! We all wish we could post more often- we have such a long list of requests at this point. But we all work full time in the NICU and we are all parents- so it’s hard to find the time! Hopefully we will become more efficient :))

love these videos! please do a video of the full physical exam with your tips esp for auscultation. I am an NICU intern and it ca be so hard to tell murmurs with breathing sounds as the backdrop.

Hello Melis! We know- this would have be way more helpful with actual examples. We are having issues with copyright with other peoples' pictures, and patient privacy issues with our own patients. So we'd LOVE it- if you have any pictures (without any identifying data on them) if you could send to us!!! In the meantime we'll ty different avenues! Thanks!!

Yes, the more actual physical images showing the issues you are educating about, especially when a comparison of normal versus concerning, or before and after medical attention, would be extremely beneficial. I would think there must be some avenue to get permission to utilize already published work in the name of the pure educational purpose of this channel??? Hopefully, over time, you will be able to legally coordinate that ability, enabling you to weather up the level of education your already terrific presentations and discussion provide, especially for newer NICU nurses, but really for all NICU nurses of any years of experience!! Please keep passing your incredibly valuable knowledge on in this free format, as my young daughter has found that new NICU nurses have many fewer very veteran NICU nurses still on the job even, and then of course there are a number of veterans in any hospital that are not so willing, or just not blessed with the great ability to teach in a constructive manner! I watch these videos just so I have a bit better understanding of issues she sees often, and am convinced that these should be mandatory viewing for all newer NICU nurses at the very least, if not the vets also! Maybe you could take the extra time to start ordering the videos into chapters playlists, and put the ones most helpful to brand new NICU nurses in Playlist 1, obviously, and move on from there, as that could be very helpful to our new nurses who are lucky and dedicated enough to take advantage of your generous sharing of accumulated learning on this subject! Thank you and your team for your hard work!

Hi Geraldine! We would LOVE to do this video- we're trying to figure out how best to do it that would be helpful without actually doing it on a baby. (Privacy laws etc.) I promise we'll eventually do one though! Even if it's on a simulation doll. There's quite a lot of luck with UVCs. If it is continuously ending up in the liver (or the portal venous system), then somehow it's not going through the ductus venous to reach the IVC. This could be because it's closed, or because it's at a funny angle. Sometimes there's nothing we can do. Other things that have helped me in the past are: a) putting in a second (smaller catheter in at the same time, to hopefully force the second one to go through the DV), b) holding the umbilical stump really taut and pulled downwards as you insert- sometimes this positions the vessels better for insertion and c) twisting the catheter as you put it in. Honestly, often none of these techniques work (and if really really desperate, I'll pull the UVC down for a low-lying line). Good luck!! Addding- Be extremely careful when doing lines- all of these techniques have added risks to them!

Hello! Thanks for watching Dr.! We need to look more into the subtitles- now since we’ve been adding extra information I’m worried they’d get in the way. Maybe we should look into putting a video out with subtitles and one without? We will investigate further! Thanks so much for being here though!

That is a great point, as you could reach nurses in far more countries if able to have these videos translated into every language, and even subtitled for hearing impaired nurses who have found a way to around their challenge, or just for those who retain new information best when reading it at some point. Hope these lectures can eventually help teach nurses around the world!

how do we do such a thorough assessment if the baby is really sensitive to touch and stimulation?

Hello Chris- great question! Obviously you need to do what you can to soothe the baby so that you can examine them- decrease sound, stroke the baby's scalp, keep the baby warm etc- but ultimately- that babies are sensitive to touch is also part of the exam- and so you need to know how the baby reacts when you listen to the heart and feel the belly etc. Sometimes it will take me a couple of minutes to hear the baby's heart sounds in the newborn nursery because the baby is crying. So I stop, warm/soothe the baby, warm the stethoscope better, and try again!
