What if you just have pain at the tip of the middle finger? Is the median nerve being pinched at the elbow?

It hurts my hand to make a thumbs up. I can move my arm and everything normal only thing hurts is doing thumbs up. Sensation lost on thumb. I fell in a gym with my hand flat . Please help
