No one is fooling anyone. If I could afford a facelift I would have gotten it yesterday. But since I can't, I'll settle for looking older but well rested and happy with a little Botox and good skincare as opposed to looking like an overinflated blowup dall caricature of myself.

I received my first set of Botox from a nurse practitioner who did a wonderful job. She took her time with me and answered all of my questions as I was a little nervous. I go for my second set on Saturday.

Always very helpful - I had Botox close to an event with an unfavorable outcome. I was able to have it corrected, but with an extra cost and wasn't sure if that was normal procedure and never questioned it.

I have been getting a small amount of Botox for about 15 years from a double board certified plastic surgeon. I would never consider going to an aesthetician or even an RN for that matter. I believe that skin care is just as important as any injectables. I've been focused on skincare (and SUNSCREEN) for about 25 years- and I really see the difference- even in a harsh environment like Colorado. I've done some filler and overall don't care for it...but I have a little bit injected in corners of my mouth, but that's it. When necessary (as these things go), I will get a facelift. My prominent nasio labial folds, had them since I was young , have been drastically reduced by filler-. Injected directly into the lines. Juvederm ultra- done 2021-still lasting to this day.

I went to an RN who dissolved my filler that migrated above my lips from years prior, which never honestly even bothered me and have been dealing with dents above my lip. This has been nothing but a nightmare.. she then months later re-injected that area with an RHA filler and now it’s uneven and the dents are back I will never go to anyone except an actual MD and I don’t even know what to do at this point!

I would love to hear your thoughts on fat transfer versus filler. Also, can anything be done just for the lines around the mouth or on the lips themselves? I don’t want a full mouth, but I want my lips to look more youthful.

I had a physician’s assistant do Botox that made my eye droop for almost a year! She prided herself in injecting without any bleeding. I thought that was great til my eye started drooping and I wanted her to fix it and she put more in and it got worse. So degrees don’t necessarily mean it’s going to go right

Can you expand upon all the commotion online about hyaluronidase dissolver causing permanent/irreversible changes to the face - people claim anecdotally that their “face is no longer like before the cosmetic treatment”? Is there any substance to the claims that this can happen, and if so, how prevalent is it?

There’s no real evidence of it causing irreversible harm to someone’s native hyaluronic acid. We suspect that patients become accustomed to their face with filler (sometimes overdone filler) and once it’s removed it can be hard to remember what someone once looked like. Also, as time passes, people age (and when you dissolve filler that can exacerbate some of the aging changes that were being masked-like wrinkles and laxity).

@@DrSamEllis, there are a lot of people with awful experience after dissolving fillers because hyaluronidaze dissolved also their own tissues as this type of hyaluronidaze using is out of label.

So if I've always had very prominent nasolabial folds but now they are extremely pronounced due to significant weight loss (just under 200 lbs. of loss), could filler be an option to at least get them back to where they were before the weight loss? While understanding I would not want so much as to completely fix it due to my natural face structure? Also if I still have around 30-40 more pounds to lose to get to my goal, is it best to wait? If so, how long after I have reached a stable maintenance weight would be ideal? And finally, I am in my mid 30s and I believe that while loose skin from weight loss has very obviously contributed to the prominence of my nasolabial folds, I also have noticed my face is starting to fall somewhat. So am I just past the age where it would be a good idea to get filler, and instead need to wait 20+ years to eventually get a facelift 😅? I realize no one can answer these definitively for me, but I'd be interested in hearing from Dr. Ellis of course but also just anyone with experience in getting procedures done or seeing a friend get them done after a large amount of weight loss.

I recently had my first “Botox” experience with a very reputable injector/nurs🎉e who used Dysport. (It was only in my frown lines) waited the full 2 weeks with no effect. I went back for a consult as I had full moment. She then used Botox. Same thing happened. No result. Since this was my very first time getting anything done, I am so disappointed and confused that neither worked. Even she said she has never seen or heard of this happening in the 15 yrs of doing it. Should I be trying another brand or injector? I am just afraid to waste money for it not to work.

Dr. Ellis, is there a lab test to see which skincare ingredients will agree with ones skin? I struggle to find products that don't break me out in little water bumps under the skin. I have tried so many, even had a test done for H Pylori (which was negative) as someone suggested that it may be the reason everything breaks me out.

I regards to having your Botox wear off between sessions, does that explain why forehead veins become more prominent? I have been getting Botox for about 2 years now and have noticed my vein is much more prominent now.

It can be due to a little bit of muscular thinning, but also due to the muscles not “squeezing” the vein as intensely, so it gets more full/distended and can appear more prominent temporarily.

I'm in thr group of 'still has volume but less skin laxity and therefore has drooped', meaning I have deeper nasolabial fold lines and marionette lines plus downturned mouth. For this you recommend the best thing to do is get a face lift, but Im only 32! Is it still an option as I dont want to be reliant on filler..

Hi Dr Sam, so random question. I have been getting Botox for 10 years, 8 years same provider(dermatologist) it is on the conservative side, 25 units in my forehead area and 11’s. For the first time my Botox didn’t work, I went back after 2 weeks with full movement, the doctor felt that maybe the syringe wasn’t drawn up correctly and we retreated, and it happened again. At that point I didn’t go back and I am just letting the time pass until my 4 month appt. Any thoughts?

I've heard from a different derm on youtube that you can get accustomed to it and your muscles stop reacting to it. I would try to research that aspect and see if other brands may use different ingredients that may work for you. I don't know much about it to say more.

How to look good using Botox? Well, after you pay you get a treatment and then such dermatologist who claim they use natural methods come and tell you how bad is in fact to use Botox. So, we are all hamsters in a roller-coaster: some might get out, some like it to be in.

I can’t wrap my head around the fact that many women use lip fillers and all end up looking fake or very obvious, kind of like Nicole Kidman’s new lips! With all the money they have or are spending, why can’t they do the naturally beautiful lips! I don’t get it! 😊

If you do something on a regular basis, you get used to how it looks and you start wanting more. It's like men who go to the gym. They are quite muscular compared to regular guys, but since they are used to seeing themselves as muscular, they want even more muscle mass because it's beautiful. Also, add here that a gym guy has gym bros, who support his views on aesthetics. I hope my explanation finds you well.
