So for me 230lbs 5'3" 29yrs old working out 3-4 days a week a BMR calculator says my maintenance is 2600 and to lose weight I should eat 2100 and extreme weight loss I should eat 1600. If I'm doing my workouts and fasting 20/4, should I be eating my maintenance or the weight loss of 2100?

Ok, so here's my personal anecdote into the mix. I was following Dr. Sten's program meticulously for 4 months using intermittent fasting (OMAD) and keto diet. It worked great. I lost 42 lbs. (from 232 to 190). Then bam, I hit a very frustrating plateau at 190. Persevered with the program another 2 months but couldn't get the weight to budge. Got very depressed because I still had another 30 lbs to lose. Thought I would have to resign myself to carrying those 30 extra pounds for the rest of my life. Then I found this video. I did 2 3-day fasts during the next month. After my second fast, my weight suddenly dropped to 185 and best of all, I continue to lose now although at a slower pace than during the initial phase. I still do at least one 3-day fast each month. Everything the good Doctor tells you in this video is true - and it works! The key to getting past set points is to lower your insulin spikes drastically by fasting and thereby lower insulin resistance. It's really true that insulin controls the hypothalamus. No more set point worries for me. I won't belabor my gratitude it Dr. Ekberg. It must be obvious. Keep 'em coming, Doc!

I have done the same, on the treadmill every day and ate literally like 300 kalories a day. lost 23kg in 3 months, i kept it off for a good year then it kept creeping up slowly but steady until i gained that weight back and much more. then i have lost weight doing keto, first time i was strict and lost 10kg, that weight i never gained back, then i did another keto diet lost 10kg again, but this time i was not strict and i cheated a lot, and i gained those 10kg back. So my plan now is to do super strict again 10kg. and then take a little break and then do another 10kg. until i reach my weight.

Thank you so much for sharing this Dr. Ekberg. I've never understood why our BMR lowers during calorie restriction but not IF and many lectures I've watched gloss over this important mechanism that is insulin makes the hypothalamus not able to see the available calories and hence it lowers the BMR because the body literally has no access to the fat cells since they are "locked". Now it makes total sense that by lowering insulin, you don't go hungry because the body doesn't need to signal you to add more fuel since you actually have a large depot of energy in the form of lipids and the body knows that it is meant to be used when there is no external intake of fuel. This video is going on my AAA lecture playlist. Thank you so much, bless you, Dr. Ekberg.

I have been on a journey of healing my body from years of abuse since around April 1st. I absolutely love Dr. Ekberg's videos as they are informative and easy to understand. After converting to low card/keto eating, I began to read and study a lot about fasting. Around the end of June, I started experimenting with fasting and lost 8 pounds in two weeks. The lost weight seems to be stable and not related to water weight as in the past. Dr. Ekberg's videos help answer all my questions and provide hope that I can heal my body.

Fasting is a protocol, not a diet. Diets focus on what you eat. Fasting determines when you eat. A lot of people don’t think about it and end up doing fasts incorrectly because they don’t think about the hormonal stress or even know they are doing it wrong.

Excellent presentation. I remember seeing a graph from Dr. Jason Fung that showed insulin doesn't go low enough on calorie restriction but does with fasting. Megan Ramos - partner of Dr. Fung - on Siimland's KZbin channel talked about how even keto and OMAD may not be enough to lower insulin and fasting in the 36 hour range or long may be necessary to bring insulin low enough. Lately there's been some studies that show that the re-feeding period is important for weight loss as well. There was a recent study that IIRC had people do caloric restriction for like 12 straight weeks and then another group did calorie restriction for the same 12 weeks BUT every 3 weeks or so they went on maintenance. It took them longer but the intermittent CR group lost more fat. The intermittent re-feeding helped to keep the metabolic rate up.
@drekberg

Yes. Dealing with a lot of people, I see that as well. Some people have such stubborn IR, that keto or OMAD would shed a pound. Longer fasts are the trick then.

The fasting model is also a calorie restricting model. I agree it's a better one, and have seen it personally by losing 40lbs in the past 5 months. But just for the purpose of explaining it I think it should be said that ONLY restricting calories is not a great strategy, but we need to be aware of the insulin role in the process as well.

@@jillnaughton8083my understanding is that you go on a fasting period. Then you break your fast with a nutritious meal that will not give you insulin spikes. You do not have to eat a whole days worth of calories in one meal because your body already burned the rest of what you needed for the day. You’re only eating for some of your caloric limit, not the full amount. You’re limiting your eating window which give you time to burn stored fat(energy, calories, etc.)

I did a 54 day water fast, went from 258 to 195 lbs, so a 63 lb weight loss, I had a demanding job at the time as well; I did not exercise at all during this fast. I then switched to OMAD keto and went down another 20 lbs (I weigh 170 to 175 now), while eating 3000 calories and fasting 1 day a week (48 hours because I do OMAD) + I was running 30 miles a week and doing heavy weightlifting 3 times a week, so yes, this is definitely true. I also tested OMAD with carbs when I went on vacation and I ate a lot, but I kept it OMAD, probably like 5 - 8k calories a sitting (yes, I have a huge appetite and I can eat a lot) and to my surprise, I did not put on any weight, perhaps a 1lb or so in a timespan of 23 days with the same exercise regime. Insulin is what it is, frequency of eating is key to whether you lose or gain weight, in my experience.

After the first week of Keto-16, I started to not desire to eat. Three months later, I added IF 16-8 and I don't even think about eating and have to remind myself to eat. I'm getting closer to OMAD, with IF window now 20-4 as I approach my goal weight. Never has weight loss been so easy. I started at 247.8lbs and now I weight 195.6 after 5 months. And your other videos are spot on. I have so much more free time because I don't need to shop, cook and eat food all the time. Thanks so much!

Update. Me & hubby have been full OMAD for 5 days now, so far so good. Thank you muchly for taking the time, & sharing this very important information with us all. & Take care too. ❤🙂🐶

Unfortunately, all but one of the winners have gained most or all of their weight back. Metabolism won't correct until you correct insulin.

There is so much misinformation out there & people are stuck in the old mindset & so are the doctors & healthcare, I have applied this information for over 2 years & have lost 5 stone in weight & have maintained that easily with IF & low carbs, without hunger cravings by keeping my insulin low. Calorie restricted diets like weight watchers etc just do not work , it is not about calories it's all about hormones...insulin & cortisol being the main two.. Thank you for educating me...amazing!! It's now taking me into wanted to study physiology.

I’m so glad I found your channel. I wish I had found it a year ago. I knew it was a change of lifestyle that was required, and not some fly by night diet plan. They put me on prednisone three times after I got Bronchitis and they couldn’t get rid of it. The final result I ended up with Chronic cold induced asthma, and a metabolism extremely damaged by those steroids. I never ate like some of those people who got really massive, but I kept gaining weight on my normal diet. I nearly hit 400. I am 6’ or I was and used to strength train years ago. I was benching over 400 so I think the standard BMI chart really didn’t fit everyone, because without any body fat I was close to 200. I started eating a couple of eggs and some toast for breakfast every day, and I came down to about 320. I think my biggest problem now is sweet tea. I quit drinking soda years ago, and will only drink it if I’m somewhere that doesn’t have tea. I started cutting carbs and starches, and was still eating things I discovered were bad from watching your videos. I’m trying to find the right balance myself. Where my body doesn’t think I’m starving, but I can exercise enough to burn a little more than my consumption rate. The hard thing is I used to run, bike, and swim with my father who was an Ironman triathlete. My knees have osteoarthritis, so even finding a way to exercise now is a challenge.

Wow! Wake up call. I was 200 at least for 3 years so that is why my body is resisting the weight loss now that I am at 160 lbs. I am sticking on keto and one day dry fast a week for at least two weeks, and then just not eating more than 50 grams of carbs when I stop. I am just sticking to eating healthy regardless. I have been in ketosis for two days now and feel great. I just make sure I drink alot of water and green tea. I have ten cups of water or green tea a day. I take everything slowly even my workouts. It is working and I haven't had any sweets cravings in almost 3 days now. I also only eat for my blood type diet. I don't believe in hours of constant exercises, and relying on that mostly.. Just eat healthy and exercise daily and take it slow. I am not exercising off 4000 calories a day, it's ridiculous. You are so right. You got to fix insulin. I wasn't hungry on calorie deficit cause I had the amount of grams I needed per protein and fats. I just don't do much carbs and rely on protein more for my energy and I also don't do stressful exercise or any stressful work. Our ancestors only did hit when they were hunting or dancing (and before meal) other than that they walked around slowly. They also didn't do intense workouts for hours. They ate small portions of carbs food and higher portions of greens and protein food and they ate slowly. I follow my ancestor now and feel great.

Started Keto because of you. I'm really excited to see if it works. I've been trying to lose weight for some time (i'm not obese), but everytime i try, it happens as you described and then i feel desmoralized and then go back to junk foods several times a week. I've been oscilating between 78 Kgs and 72 kgs for years now. My normal weight for someone my size would be to 68 - 72 KGs. I decided to do keto a week ago. And i don't remember the last time i felt hungry. Intermitent fasting really works only with high fat, moderate proteing diets, since there's no insulin to trigger hunger.

I've listened to another dr. and changed to your channel a while ago. Currently doing IF, and I'm on OMAD since a while but still mixing it with 2 meals a day with 4-6 hrs window. I eat fruit occasionally- once in season, and I got sweet tooth cravings, which are considerably okay and hopefully will go away over the time. I shop for fresh and organic only i.e. wild caught fish, organic fatty meat ( steak and pork), and I love olives and avocados and any types of oils. I follow a diet based mainly on HEALTHY FAT sourcing from plants and fish, and eat a little amount of carbs in form of all sorts of leaves and salad. I don't eat chicken unless is organic and this is only occasional. I hope to be back to OMAD without any carbs cravings. Great Channel, Thank you 🙂

Thank you very much indeed Dr Ekberg. This was a very helpful presentation - so glad you addressed the common fear and mine too-that fasting lowers metabolic rate. My learning points were the hypothalmus 'setting point' and the insulin resistance and how these two factors lower metabolic rates - I now wish to work out what type of fasting pattern would best suit me and I am going to check out your other videos to see if I can work this out - I don't want to begin a serious eating/fasting regime only to learn for instance that I have lost muscle mass and reduced my metabolic rate. I will check out your 15 mistakes...and see what else you have....

I just started the 42-6 Intermittent Fasting combined with Keto (and steady state walking or biking). I've lost 8.5 lbs. in the first 5 days!!! Will be interesting to see the loss levels as the weeks go on. I know it's easy at the start and a lot of it is water weight. My fridge and cupboards only have healthy choices now. Meat, frozen mixed berries for fruit, and vegetables. No carby or sugary snacks (only nuts and seeds). For drinks it's just coffee, tea or sparkling water. It was tough giving up diet cola but all that aspartame isn't healthy.

Since 12th February 2023,I stopped eating 1. Table Sugar 2. Rice ( All types) 3. Bakery Products ( Refined Carbohydrates) 4. Flavored Drinks ( Nescafé, Nestea etc ) 5. Soda drinks ( Coca Cola, Sprite, Fanta etc..) & adapted to a 12 hour Fasting at Night. Recorded Fasting Blood Glucose Every Morning and graphed with excel & monitored the fluctuations & managed to maintain it two months average of 96.5 mg/dl. Increased intake of Protein (Chicken, Fish, Eggs,Salmon), Olive oil & Green Leafy Veggies (Mainly Cauliflower, Beans, Okra, Brokali & Parsley). During the day I had some milk with added coffee and dark chocolates ( 50% of Cocoa made in Sri Lanka) & occasionally some curd with one or two teaspoons of Honey. My HBA1c dropped from 9.3 to 5.7 by 12th April 2023 (Within exactly 60 days). Weight loss exactly 8kg. My BMI is exactly 22. I am so happy to bring down My HBA1c to None Diabetic level without any doctor fees, Prescription Drug Fees, Unnecessary Lab Report Fees & without time wasting. I am planning to drop it to 5 by 12 th May 2023. Meantime, I got my Lipid Profile, eGFR (Kidney Function Test), Complete Liver Profile, Full Blood Count & Specially the C - Peptide Report that measured the functionality of pancreas & all reports are almost perfect.
