Firstly you make the intention of which has 4 forms: performing wudu to remove hadath(ritual impurity), performing wudu to perform an action that necessitates it(like salat), making wudu to perform a sunnah action like reciting Quran, or performing wudu as a renewal for the pre-existing wudu (for example optional wudu before an obligatory prayer) while ignorant about having broken it. Then you say Bismillah. Then you wash your hands up to the wrist thrice. Then you rinse the mouth and nose. Thrice each, using your right hand to insert the water for both and then using your left hand to expel it from the nose. It is best that you perform it with one scoop of water(washing your mouth three times with it and then taking it into your nose three times with the same scoop), but you can do it taking water thrice (washing the mouth and nose with one scoop then repeating it three times) and six times(washing the mouth three times with one scoop each and then cleaning the nose three times with one scoop each) as well.

You need it to touch the Qur'an, but not to recite it without touching it.

as salamu alaikum may Allah bless you brother. please do not refer to the "ayat" of the quran as "verses" as verses refers to lines of poetry and such. Rather just refer to it as "ayat" or [singular] "ayah"

Not washing it six times, he meant taking water in your hands six times, so you take water in your hand three separate times and wash your mouth thrice then take water thrice and wash it thrice So a total of six takes of water
