I give you for FREE, my opinion about how to best measure and fix a Leg Length Discrepancy. This measurement does NOT have to be perfect. Taking the measurement in standing can have more carryover into functional ADL's.

It's amazing that 3/8 discrepancy can offset or appear to cause an inch or more tilt in the body, and with stretching or muscle tightenss makes it even more challenging...good luck to all you come across this.

For years I had back pain!!!It turned out to be hip problems and my left leg was shorter than my right! When I got two new hips yay and now wear a heel lift on the left, no more back pain whatsoever. Thank you UCD orthopedic surgeons!!!!!!!!!!!

Many people who have hip arthritis are treated for back pain. Hips refer pain to the back a lot. Glad to hear you are doing well Phyllis!

Can i ask you if you had issues maintaining a neutral leg alignment (outside of foot making a straight line forward), as i seem to struggle? Also did you find on one leg you had a lot of weight on your heel, whereas on the other foot, you had trouble really pushing your heel into the ground? I also tend to stand like that, with all my weight over my left leg, and half of the time, id only have my right forefoot on the ground with the heel floating. My entire body leans diagonally to the left from the feet all the way up too. Getting xrays and stuff finally too, thats why im interested, ive always kinda asumed my leg length discrepancy was functional rather than structural but im now reconsidering somewhat.

Thank you 🙏🏻 so much for this video! My left leg is longer than my right leg confirmed by a standing x-ray. I also have a fused 6th vertebrae on my left side, and I’ve been told I have Bertolotti Syndrome. I’ve been in pain since my late 20’s after child birth. It wasn’t until 7 years ago that a chiropractor took the standing x-ray. Up until then, various chiropractors would just adjust me which was always painful. Anyway, 7 years ago, a new chiropractor gave me a heel lift; and for the first time in 30 years, my lower back pain was relieved. But after a short time, as you so accurately stated in this video, my knee started hurting me first, and then both of my hips started hurting too. So I ended up in more pain and unable to sleep. So thanks 🙏🏻 to your video, I’m going to try and insert a full wedge in my shoe and see if that helps. Thank you!😊

I also was diagnosed with Bertolotti syndrome about 3 years ago and have been suffering with the same issues back,knee pain . Did the full wedge help ?

@@ezeehc22 No I never did. I had gone to a specialist, and it was a disaster. He caused me a lot of pain. I told him about the wedge, but he thought that he knew everything. So, he had me do a different insert. It took over a month to recover. Then I found another one, and it didn’t work out with him either. Then I found a third chiropractor who is amazing. He is the best chiropractor I’ve ever had, and my pain improved a lot with him. He had previously been a massage therapist for years, so he is different from anyone else I had ever seen. Anyway, I just moved from the area he was in, and I found a new chiropractor who is good. After everything that had happened, I completely forgot about this video. So I’m glad you messaged me because I’m going to discuss this with my new chiropractor, and I will let you know. But, I definitely think that you should try it.

Thank you. I got very painful metatarsal bursitis, because a podiatrist gave me heel lifts. I gradually lost my ability to walk and stand.

Putting any wedge and trying to adjust leg length can have huge impact on a body. The impact cab be positive or negative. The only accurate way of measuring is through scan (x-ray, MRI, CT). The rest of methods can cause more headache in longer term.

Hi sir, I'm a new physio and I appreciate your great work , sharing information with us. I have a question, I examined a patients foot and he has sensational spots under foot arch, some increases pain locally when pressed and especially one spot gives tingling to whole body even to hand and fingers! the question is he has a callosity on his right foots lateral malleus but left foot looks fine in terms of callosity, both feet lost eversion a little bit. what is significence of right foots callosity on lateral malleus?

Something has to be rubbing there to make a callus. If no rubbing then it is not a callus.

@@OrthoEvalPal yes sir, his shoe is rubbing, I thought one little abnormality like little weight bearing or eversion etc. Triggers a chain reaction resulting in big compensations on lower back and torsum. Which means pain.

Hello. I get headache due to structural LLD, the difference is 0.75. Will full sole lift alone potentially fix it?

That evaluation does not take into account sacroiliac dysfunctions or pronation. As far as lifting the heel with or without the sole, without measuring dorsiflexion you don't know. Raising a heel only increases forefoot pressure when there is no equinus. There is a lot more that is missing in this evaluation.

Thanks for the demo! Just one thing I was thinking.. couldn't a tight QL also pull upwards on one side of a hip, causing the appearance of a shorter leg even if the leg isn't technically shorter? I thought this was a reason for the tape measurement. Thanks again :)

Yes and so many other : check SIPS, torsion pelvis, correct this first than ‘leg length’ disappears..

Yep there are functional and structural discrepancies and its quite hard to tell which you have. If its functional a lift on the shorter leg will exaserbate symptoms, so you do have to be aware of how your body handles them. Its been a while and at this point im pretty sure my leg length discrepancy is structural, but it could still easily be an issue at the ball joint itself or many other things. After 15 years im finally getting xrays done on my hips. Its a joke, that its taken this long.

But if the lift is only in the shoe, doesn't it cause issues when walking with a different pair of shoes without lifts or barefoot/slippers? Doesn't changing leg length so frequently, possibly cause more issues long term if not always in correct shoe, with lift 24/7?

The lift is moveable from shoe to shoe. Many patients who have a permanent leg length issue buy several lifts for each shoe.

Everybody says to measure from the ASIS. But can't the ASIS be shifted higher or lower when one has lateral pelvic tilt? Thereby not being a reliable anchor for a "true leg length" measurement?

I think I need 2 get bk 2 using my orthotic, esp my rt 1, as I slope over n my rt slipper. I'm going 2 try just the rt 1 1st, since I'm convinced that leg is definitely shorter, even though my chiropractor doesn't agree. But since he judged my leg length as being even only while I was laying down, I'll have him check yr way. He's an excellent Dr otherwise. Over the yrs I've lost 4 3/4" - my grammy was shorter than I n my 20s too. I started questioning that my rt leg was shorter when I was walking along the seaside & while walking when the sand was higher on my rt, it didn't hurt as much 2 walk as when walking flat. Then when I turned around on the beach & walked bk w my rt leg on the downward side, it hurt a lot more than on the flat. I also understand a lot more v recently how my sensitivity 2 sugar, dairy & grains puts my balance etc off even more + how they interfere w my body's ability 2 access the minerals 1s bones erc need 2 b healthy. I've recently lost 46 + lbs by eating v mostly just fish chick meats veggies grn tea & water, though its v difficult 2 cut out that 1 c coffee/day! I realize that is my problem food that leads me bk 2 the other bad 1s, so I have 2 often correct things. But I do want 2 say, I've gotten a lot more insight as 2 how the body wks, than I already knew abt myself & my issues, frm watching a heart surgeon talk on my cell, as well as, this Australian Dr n Vancouver, who has a v deep & basic knowledge of the body processes & how doing the rt & wrongs things impact the body, including using poltices & other natural things/wats that really work. Absolutely amazing both of them & u! So far, what I had already learned, not only have I saved my own life a few times, but our daughter who came 2 - 3 days from passing away frm an antibiotics she was allergic to, but also my hubby, who was having an allergic reaction 2 a Covid booster. I've also been able 2 stop him frm having any more kidney stone issues, stopped my gallbladder issue, stopped my hubby frm any more broken bones when realized he had osteoporosis frm the coke cola n his rum drinks - only the odd beer now lol - + fixed or manage a lot of our other health issues. Problems I'm still working on: hubby smokes, nurse daughter w cyst n fallopian tube - she has same food sensitivities as I but still eats some things she shdn't; concerned mostly abt grandson, who i know is greatly impacted at school etc cause he's eating 2 many grains & why his ADHD is way out of control. The adults know what they need 2 look after, the grandson @ 11 doesn't understand most of it & I'm 2 far away 2 b much influence. Oh well, no matter what, my glass is always 1/2 full; that @ least away makes me feel 10% better + even w cane, I do 1 - 2 hrs/wk Latin Line dancing + 1/2 hr ea a.m.stretching b4 I even get up, + gardening - even w worst bk GP has ever seen, hip worse, hypothyroid, fibromyalgia, SAD, hiatial hernia etc!!!😊 Thank u 4 yr help. I just reached 75 & surpassed my mom who passed needlessly @ 66 frm a hiatial hernia for 2nd time n same place/2 diff Drs wudn't listen 2 her, so guess I'm doing some things rt - secret was/is 2 becoming my own avocate. God bless - I'm a down 2 earth/grounded person, but I must tell u I was physically saved by my Guardian Angel n my 20s frm a fake that wud have killed me; instead I was made 2 blank out & woke up abt 20' away w tailbone hurting, sitting on a different big boulder, having no way 2 get there & no people around. It did happen & no I wasn't drinking liquor nor do I use drugs (except 4 the odd antibiotic etc) - it was abt 10:30 on a Mon a.m. & I was going 2 work. It just wasn't my time to pass & prayers do help, just need 2 say them out loud.❤

Thank you for the video, however the method doesn't distinguish between a functional leg length discrepancy and actual leg length discrepancy. If you start adding heal raises etc for a functional then you're never going to address the real problem

Iliac crest is the top edge of the pelvis to the sides. Medial malleol is the protruding bone on the inside of the ankle. ASIS is the forwardmost projection of the iliac crest. Hope that clarifies 😊

Great video- My question is a clarification question. If the right hip hikes up, does that mean the LEFT leg is shorter and needs the lift.. So my question is in how to read the assessment.

Hi, not sure if you ever found your answer but to maybe shed some light on your question, you’d look at the hips and whatever what is lower it would be the shorter leg so he’s recommending if you have 1/4 templates or heel/shoe lifts (you can find a bunch on amazon that are cheap and come in 1/4in sections) you’d then put it under the the shorter leg and keep adding until the hips are even. He explained it pretty well in the video! hope I was able to help out!!:)

Why 3/4 shoe fill lift? Why not just full shoe lift? Those partial shoe lifts always fall forward.

When I do custom orthotics I do only full length. I've never had a 3/4 length lift move forward. I always put it under the shoe's insole so it stays in place.

I've got a 1 inch descrepancy... What about for athletes with specialized shoes like cleats? You can't really put much of a lift in or get them specially made. I require a generic insole to prevent knee pain which doesn't leave any more space in most shoes. I'd like to be able to walk barefoot/ do yoga once in a while without being uncomfortable.

It is structural so the LLD will always be there when you don't have shoes on

I am 73 and in my youth was in competitive sports. I spent the evenings in bed and wish I had known more. My discrepancy is now 3/4 inch based on my testing on my left sole build up. I correct all of my shoes with some hacked up effort to make my soles adequate. I unconsciously compensated by tightening muscles in the opposite hip, throwing the spine out, twisting the short leg to walk on the outside of my foot (which injured that knee). Now I'm undoing all of these kinks. Know that it will get worse as your muscles spasm trying to avert pain if you don't make corrections.

Why can’t more drs be thorough like this not one dr has examed me physically. I’ve had PT, injections, MRIs, X-rays ect showing only some bursitis which drs think is no big deal. I am in so much shock that I’ve been left to rot like this for 21 years no help. I’ve had severe pain on the side of my hip and hamstring with severe tightness and snapping near my bum line like severe. I’ve been to so many drs have had so many MRIs it’s not funny my life ruined. I know whatever the problem it’s likely something rare or I just haven’t run into a skilled enough surgeon to diagnosis it. This all started around 21 I’m now 43 still going dr to dr to dr finally had to go on pain meds 4 years ago. The pain was so bad that I lost my job after 6 years at the age of 25. And I’ve basically been disabled ever since going dr to dr to dr with no answers. I haven’t slept in 21 years literally I’ve slept like 1-2 hrs wake 4-5 hrs sleep 1-2 hrs wake 4-5 hrs repeat. I feel like the lack of sleep is going to kill me eventually along with the pain and tightness. I never imagined when I first started going to drs that I would never be diagnosed and cured. I’m suffering so much mental unwellness from being left like this and I also know a lot has to do with money. Although I’m at least $100,000 over these years into going dr to dr basically the research to be diagnosed. I don’t know where to turn anymore I’ve seen every kind of surgeon there is. It’s obviously something extremely hard to diagnosed and drs rely heavily it seems on MRI’s unfortunately. I’ve begged for exploratory surgery but can’t get that either I don’t know what to do anymore the pain is so bad. I know there are 3 types of common snapping hip syndrome but I’ve found a few articles of rare case publishing’s by a few drs. That diagnosed a few other snapping hips that are supposedly rare which I think means under diagnosed not rare. One being snapping hip if they hamstring and also of the satorius muscle and the one next to it. And this area is exactly where the large deep snap occurs 50 times a day for the last 21 years. I’ve recently the last 3 years have become scared it may be hereditary. As my 16 year old started presenting with a very light snap in hip on some movements. I have asked her if she has any pain, tightness or awkward feeling like I do she says no. But I also didn’t have pain at 16 then suddenly at 21 the nightmare began. So I’m getting her checked over again soon as 3 years ago they said if no pain present just keep eye on her. This is the dumbest analogy I’ve heard when online it says snapping hip left untreated can disabled a person. Can you pls tell me if you have any tips as far as tests I can ask for that can help me finally be diagnosed. Oh and I also get a pop in my knee and ankle as well my whole leg just feels off. The tightness in the hamstring is so painful it burns like fire.

I been having this problem ever since I had my daughter , my right side hurts so bad my hip and leg . I never have pain on my left side . In what shoe should I wear it ? I’m confused

Hi Vanessa, It is very important that you be properly evaluated first to make sure you don't have something else like a herniated disc or anything like that before adding a lift.

Yes I do. The neoprene material that I use comes in different densities. Good question!

This method doesn't tell you if the leg length discrepancy is functional or structural. If you put a lift under a functional issue then you can exacerbate the problem.

Hi Melissa, you need to be measured for the correct height. You should bet able to find something locally to lift you up to that height. I will look to see if there is a product you can order more easily.

I have a functional leg length discrepancy. Do I just use a shoe lift for the rest of my life? There should be a way to reverse the muscular habits that cause the pelvic imbalance.

Yes you should have an evaluation to identify any muscular imbalance first. If there is a true leg length issue like a with a short tibia or cartilage loss for example then a lift is very appropriate.

I have the issue where my right leg is longer and therefore my right hip is a bit higher And the oblique muscle is more developed due to imbalances when i workout(uneven flexing of the core) I have only a heel sole but od doesn't do too much, any advice on streches etc?

@@lokooooooooooct I was losing my mind trying to figure this problem out. I was seriously thinking that one of my legs was longer than the other and was about to get a heel lift. I've been dealing with this problem for four years now and had to resign from my job because of it. I will definitely be watching all of Neal's videos. How long did it take you to fix the problem? Did you need to do a consultation with Neal or did you just follow the four exercise techniques? Thanks.

I have a leg discrepancy that was caused by a car accident. Now I have a limp which I hate, I do wear a heel lift which helped but I still limp. What more can I do to completely eradicate the limp whilst wearing a heel lift

My mother’s left leg is 2.5cm shorter. I hv been taking her shoes to a cobbler to increase the sole but the problem the shoe becomes very heavy compared to the other shoe. What advice can u give? Is there any material tht is light? Pls help.

I would not know but you could consult with a prosthetist. They use different materials all the time for things like this

I have about a quarter of an inch discrepancy and have been suffering from severe lower back and hip pain forever. I finally bought a lift, but hate having to add so much in my shoes to be able to wear them out. I also have to wear arch supports. I really wish there was a simpler solution for me.

Hi Bri, I usually build the lift into the orthotic when I do custom supports. I may have to jump up a size in shoes.

I'm 73 and have had back pain since a teenager. I started adding lifts, building up my left shoe sole about 15 years ago after a tip from my chiropractor. My build up is slightly over 3/4 inch and has been steady for over a year. This much discrepancy has kept me from taking physical work all of these years.. In spite of these corrections I am still working to release the painful spasmed tissues that resulted from this condition but the progress continues steadily. I have often adjusted the lift height daily due to muscular release or tightness. The spasms are the biggest issue, not leg length corrections. I have used many instruments to relax these tissues; cold laser, chi lamp, blue emu cream, etc. The best is lately the use of "Terrahertz wand" ($400). I use the licensed version from Prife and do recommend it. Progress has increased rapidly since using it. sosls@@user-vk2ei5ze4y

I have the same. I have learned to add soling material to the bottom of the shoe and it works pretty well.

My discrepancy goes up and down as my back slowly straightens from the years of compensation in the low back. It's still slightly over 3/4 inch. But, I am sensitive to even 1/8 inch off so I add insole layers to keep it within that margin. @@Xavierzzzzu

I need help after 3surgeries on my knee my leg is 3inches shorter. Im bout to have a 4th surgery. My hip an leg kill.i just want to walk more normal again.

A 3 inch difference requires a lift under the shoe.

I'm a poor person with lld femur is about two inches shorter on one side due to a leg break when I was a kid I went 25 years without a proper shoe now that I have a proper shoe to even me up. I'm noticing a lot of atrophy in the muscles and my hip is locked my knee won't fully straighten and as I walked more I'm noticing a lot of muscle cramps. Life is misery right now I'm doing a lot of stretching but something is always hurting goes from my back then the pain was in my thigh now the pain is in my calf I can't sleep

I'm sorry. You can read my story above. Yes there are pains up and down the spine and leg from discrepancy. As you say, releasing the muscle cramp is the biggest issue with leg length discrepancy. I am using the Prife terrahertz wand but it is costly ($400). I also like blue emu cream ($14) U.S. I have used acupuncture needles as well- of course be careful here. Good luck.

After I got myself a proper pair of lifted shoes for my left side I got a job as a mechanic right to be on my feet for 9 hours a day if not longer and lifting heavy things I started not being able to stand for more than 15 minutes at a time and it was misery nine months later I dropped 90 pounds I'm more flexible a lot of the pain is going away and I'm building a crapload of muscle with a lot of muscle cramps

@kulcousy I haven't been x-rayed in a while but I think I'm dealing with a fused disc or two I went 20 years without the proper orthopedic shoe. Right now I'm dealing with a little bit of sciatica pain but I just can't seem to fully stretch out one kink that's right dead center above my tailbone it just feels like it wants to move but it won't. And everytime I try to stretch it too much I feel electricity shoot through to my toes

I have a leg discrepancy on my right leg and it’s about 1/2inch but my heal is also stepping at an angle as if there is a angled rise from the toe to the outer heal that wears on the right rear of the heal on my shoes at an angle. How do you measure the rise from the toe to the outer heal to get an accurate angle to correct the gait and step correctly?

Hi pal hope you are enjoying, I just want to have your expert opinion. My mother have 3 cm short left leg which happened due to an accident. I met with ortho expert and he gave me a insole of 3 cm. Kindly suggest best way to use that insole and reliability of that sole

I like to use a lift that goes from the heel to the ball of the foot and it should be placed underneath the insole in the shoe.

I have a 1.5 inch leg discrepancy and went to the doctors which gave me an insole for my shoes (the only down side is that it’s made of cork which with it hurts my foot).

Putting in a lift will make an immediate change. A true leg length discrepancy will never correct itself.

Great video thank you!!My doctor said that I would need orthotics bec.of leg length discrepancy, they working with FOORMAX company - and the price is $500 for 1 pair. Is there anyway I can find alternative solution>?like for example buying some inserts from local pharmacy stores>?

When I don't make a custom orthotic, I use Tread Labs. The orthotic alone will not correct the leg length issue but you could add a separate lift under it. bit.ly/3P3o6Z4

Hi Vinod, Hard to tell. If a leg is short though, it may never get longer. Once a person uses a lift for a leg length problem they usually have to always use it.

I had inserts and my pelvic shifted where a chiropractor had to put me back into alignment I was measured like you show so then another doctor stated the leg shortness was all you could put in the lift not the rest of the body like hip degeneration etc I have had hernia surgeries where a doctor associated this with the shifting What are your thoughts since I was left this way

Yes it will if the bone length is shorter on one side than the other or if there is significant degenerative changes on one side over the other

Lmfao "im going to show you a better way to measure leg length discrepancies without any errors", 1st thing you do is stand the patient up then you find the iliac crest and I like to "ballpark it"
