Body recomposition is very real for me, though this could vary widely depending on your age, history and particular physiology. My weight hasn’t gone down that much but my pants are a lot looser and people regularly remark about how much better I look. My personal advice is to pay more attention to your waist measurement than the scale.

I force myself onto the scale each and every morning. This is because the scale doesn’t lie. Maybe it’s inaccurate, but the same scale constantly tells the same inaccuracies. Weight is least important, but, if I don’t loose weight and my skirt is falling off and it’s actually fun to go for a walk, I’m making real progress. Especially because I’m a woman, weight loss day by day is a foolish measure. And, especially for the younger ladies, it’s important to note, a growing waist measure and no movement on the scale can signal a happy addition soon, especially as you move from overweight and unhealthy. I’m no longer married, and I’m beyond that age, but, still my hormones shift, and I gain 8-12 pounds of water weight each month. Even when I was in a two week fast, that time came, and I gained weight, although at the end, I had a net loss. So, I completely agree, weight is not everything. It’s possible to be a normal weight and still be fat. You don’t want to be rail thin, weak and miserable. Healthy, happy women are a slight bit padded, and some are heavily padded in the hips and thighs.

Thank you so much for this encouraging video. Years ago I battled hypoglycemic headaches. Since changing to low carb & intermittent fasting, I'm very pleased with how my body is healing & getting healthier.

I love protein and I love how I feel on a high protein diet, but I'm worried about longevity. Sinclair is so clear about this.

It's fine, protein won't harm you. As long as you feel good and you're in good shape, don't worry about it. The only way to get protein toxicity is if you only eat lean meats and litterarely nothing else because then your body will burn that little fat that's in your meats, quickly break down the glycerol into glucose then it will use a large portion of that protein to convert into glucose to keep your blood sugars normal and then you get too many of the byproducts of converting amino acids into glucose, forgot its name but that's the only way to get it.

Hi Dr. Scher, First of all, thank you for all you do to help us in our weight loss goals. Since starting VLCHF and at-home resistance workouts, I've dropped a little over 5 BMI points, going from class 2 obesity to overweight. Your insights are invaluable. As a third-year medical student, I am dismayed to see the lack of good insight and advice in most of primary care/cardiology/endocrinology settings where I've rounded. It seems almost impossible to break through the lipophobic (to plagiarize a term) guidelines. I have a few questions: 1) Does a PCP have to essentially open one's own independent practice (difficult in modern HC settings) in order to practice keto-friendly medicine? I see doctors gossip about providers who recommend keto, and they seem like they're professionally sidelined. It's very unprofessional, but, forgive my jadedness, but pettiness is a big problem I notice among HCWs in general (but that's another topic). Opening an independent practice was never what I had envisioned when I went into med school. 2) Is it worth it to even try to give an honest answer to my attendings about what my real plan would be for a patient? I feel like a liar when I recommend low-fat, low-cholesterol, plant-based blah blah blah, but I know it's what they want to hear. 3) Are there legal hurdles to not delivering USPSTF- or AAFP-sanctioned advice/treatment to patients? The current standard of care should not be the standard of care, but medico-legally, it's still the standard of care. Is there a way to protect oneself from frivolous suits in that regard? Hi Joey, I enjoyed reading your post. I cannot answer any of your questions since I am not a medical provider. It struck me how open you are to what many call alternative medicine, and I hope you continue down that path after you graduate medical school. My doctor is a DO in a setting with hundreds of MD’s, and she is the only one in that insurance setting I have found that I can discuss my alternative medicine opinions with. More and more MD’s are also realizing that there are alternatives to what they have learned and practiced. Keep up the good work, and my best to you and your medical career.

Also, I bought a pair of 30" jeans today for the first time in a few years. In 3 months, I've gone from a 33" back to a 30" waistline. I was borderline pre-prediabetic, and now I feel like I've caught myself before any serious problems occurred. This community has truly been key in helping me change my lifestyle choices. Went on Keto & Intermittent fasting & so far lost 42 kgs, has cut my blood pressure meds in half & my doc is happy with eveything. Only downside i can think of was buying a leather tool to make holes in my belt. But i payed that price gladly.

I've read in literature that bioelectrical impedance has a large margin of error (like ±10 points), and that even DEXA has a significant error margin (up to ±5 points). Thoughts?

I've just received my blood test results, everything was great except for my cholesterol. It came in at 7.5, one part of that test came in at 4.3 instead of 2.5. I don't have the results in front of me, but just spoke with my doctor. I told her that I'm on Keto (1 yr) so have been having cream in coffee, chicken with skin on etc. Prior to Keto I ate only veg and fruit with small amounts of cheese, we discussed meds, but I suggested cutting back on my fat intake so will need to remove skin from chicken and modify the cream then have another test in 3 months. My cholesterol has always been on target in the past, so was disappointed with the results. My doctor said that as we get older the process gets slower for breaking down the fats. I do OMAD, I certainly don't have a bigger meal and don't have all those fat bombs and desserts. Can you advise if possible how much good fat is okay to avoid this increase. I recently thought I might be able to try your website program but as I have issues with mast cell activation and need to eat a lower histamine-containing diet, it would not work for me. I got a DEXAscan a few months ago. I expected to get more information about visceral versus subcutaneous fat. No. I did get 30% as my fat composition, which seems plausible but very disappointing after already losing 45 pounds over the past year.

I am 55, At three months of lifestyle change diet and exercise primary meat/fish/eggs, modest dairy, nuts and seeds, low glycemic veg and fruit diet and 30 lbs of weight loss my fasting blood sugar still is not getting into the 80s. Im tending to be in the mid 90s. Wearing a cgm for over two weeks my blood sugar is pretty flat but tends to average up in the low 100s in the evening up until i go to bed. In November three weeks into my new diet and exercise my blood tests were 96 fasting glucose, 4.6 fasting insulin, A1C 5.7, C-peptide 1.56, all other tests including thyroid T3 &T4 were all normal exception low Vit D. What is a reasonable time expectation for seeing my fasting blood sugar in the 80s and what is a reasonable daily blood sugar between meals snd in the evening? I am shooting for an A1C of under 5.0 but that doesn’t tell me the answer to the question of what idle blood sugar between meals and in the evenings should be if optimum in a normal healthy individual.
